SOP UniversalLocking Plate SystemWhy have we developed the SOP™?

The use of bone Plates and Screws to repair fractures is well established. Although widely used, conventional Plates have a number of significant limitations. For example:-

• Plates can only easily be contoured in a single plane, hence the development of the reconstruction Plate. • Conventional bone Plates impair blood flow in healing bone, hencethe development of the limited contact dynamic compression Plate.

• Bone Plates are particularly weak around Screw Holes, hence the development of shaped implants.

• Conventional Plates depend on friction between the Screw head/Plate and the Plate/bone interface for stability, which is inefficient, hence the development of locking Screw systems.

• Minimally invasive fracture surgery is an attractive concept but existing Plates do not lend themselves to this application.

What makes the SOP™ so unique?

• The SOP™ is a novel implant system which answers many of the shortcomings of conventional Plates and consequently has a wide range of applications in orthopaedic and spinal surgery.

• It comprises a series of non-deformable apertures (pearls) connected by short internodes with a circular cross-section; standard bone Screws lock into the pearls.

• The pearls are stiffer and stronger than the internodes. They are manufactured to extremely high tolerances to allow the use of standard Screws.

• The connecting internodes have a circular cross-section which gives the implant a consistent stiffness profile along its length – there are no ‘weak points’ analogous to the Screw Holes in a conventional Plate.

• The SOP™ was designed to be stiffer than the 3.5 LC-DCP (Limited Contact Dynamic Compression Plate) and the 3.5 LCP (Locking Compression Plate) - confirmed by mechanical testing performed at Cummings School of Veterinary Medicine at Tufts University, Boston. Data on file.

• The SOP™ can be bent in any plane or twisted along its axis.

• The SOP™ is a locking system providing angular stable fixationof fragments regardless of bone quality, minimising the risk of lossof reduction, even under high dynamic loading and allowing secure fixation even when bone stock is poor.

• The locking system (patent pending) uses regular bone Screws and provides two points of contact (Screw thread and Screw head) between implant and Screw for optimum stability and security. It also eliminates the need and cost of a separate range of interlocking Screws.

• The footprint of the SOP™ on the bone is extremely small when compared with conventional bone Plates so the impairment of blood- flow will be minimal.

• The SOP™ system is designed for use in conventional and minimally-invasive fracture surgery.

• The locking system is more ‘surgeon-friendly’ and ‘bone-friendly’ than existing systems.

Castless PancarpalArthrodesis PlatesOrthomed are delighted to announce the release of the CastLess Pancarpal Arthrodesis Plate (PCA). The Plate is currently available for 3.5/2.7mm Screws and for 2.7/2.0 Screws.

PCA Rationale

Introduction

Pan-carpal arthrodesis is an established surgical procedure used to salvage otherwise irreparable injuries of the canine carpus. Traditionally, the surgery has involved removal of articular cartilage prior to placement of an autologous cortico-cancellous bone graft and fixation using a bone Plate applied dorsally to the distal radius, radial carpal bone and the third metacarpal bone. After surgery, it is generally recommended that a cast is applied to provide additional support during the early convalescent period.

Although usually successful, pan-carpal arthrodesis is technically demanding and complications, either directly related to surgeryor associated with the cast, are not uncommon. Over the years consideration has been given to alternative surgical techniques: external fixators are effective but technically very difficult to apply and a second anaesthetic/surgical procedure is inevitable; a ventrally applied bone Plate is more appropriate biomechanically but significantly more difficult to place; and medially applied a bone Plate has been advocated but this requires considerable additional implant contouring and an undesirably straight arthrodesis. Most surgeons favour a modification of the traditional technique using a modified bone Plate featuring 3.5 mm Screws proximally and 2.7 mm Screws distally for the metacarpal fixation.

Design Considerations

The incidence of minor complications related to the use of the cast, whilst nowhere documented, is likely to be considerable. Catastrophic cast complications are not unknown. An implant system which avoids the need for post-operative casting is, therefore, desirable.

In the normal standing dog, the carpus has 10-15° of extension. However, a review by Whitelock, Dyce and Houlton (1999) indicates that excellent cosmetic and functional results are obtained with an arthrodesis angle of approximately 8°. Achieving an acceptable arthrodesis angle involved bending the Plate over the carpus which will subsequently be exposed to considerable stress.

An implant which achieves an arthrodesis angle of approximately 8°

without contouring is, therefore, desirable. The review by Whitelock, Dyce and Houlton (1999) revealed that fracture of Metacarpal III through the most distal Screw was the most common complication causing failure of the arthrodesis. Further, it was shown that Plates covering more than 50% of the metacarpalwere less likely to cause fractures. An implant which covers a large proportion of the metacarpal is, therefore,desirable.

Vigueur and colleagues (2001) using cadaver limbs demonstrated that, when loaded incrementally, Plate arthrodesis invariably failed by fracture through Metacarpal III. An implant which attaches distally to two metacarpals is, therefore desirable.

Design Features

The implants are machined from surgical grade 316LVM stainless steel; batch-marked and manufactured to internationally recognised ASTM standards. Each batch of raw material also undergoes an intercrystalline corrosion test in accordance with ASTM A262.

There is a range of three, 3.5/2.7 implants varying in length from 120-140 mm. Proximally there are four Holes designed to accommodate 3.5 mm Screws, two round Holes and two ovals to permit compression of the radius against the radial carpal bone. A single centrally placed round Hole accepts a 3.5 mm Screw to be placed into the radial carpal bone. All three Plates are identical proximally - it is the distal (metacarpal) part of the Plate which varies in length, incrementally by 10 mm. The variation in distal length allows a Plate to be selected which will give maximal coverage of the metacarpal bones.

Distally, there are three pairs of Screw Holes designed to accommo-date 2.7 mm Screws. The proximal pair of Holes is placed close to midline and Angled at 10°; the middle pair of Holes is slightly more offset and Angled at 30° and the most distal pair of Holes is significantly offset from the midline and Angled at 30°. This arrangement optimises the Screw - bone contact and allows for the anatomical divergence between metacarpals three and four distally.

Arthrodesis involving fixation of the implant to two metacarpals, each of which is substantially covered by the implant should minimise the risk of metacarpal fracture.

Two small (0.8 mm) Holes perforate the Plate distally. During surgery a small K wire or hypodermic needle can be placed through these Holes and between metacarpals three and four to maintain accurate alignment of the implant.

The underside of the implant is contoured. Proximally, a curved under surface is present to conform to the distal radius and the radial carpal bone. Distally, the surface features a “keel” to accommodate the dorsal aspects of metacarpals three and four. Additionally, the implant is shaped to impose an arthrodesis angle of approximately 8° without the need to bend the Plate.

Method of Application

A standard dorsal approach is made to the distal radius, carpus and metacarpus. The tendons of the digital extensors along with neuro-vascular structures are retracted. The dorsal joint capsules are excised from radio carpal, intercarpal and carpal metacarpal joints before articular cartilage is removed from all joint surfaces using a curette or bur.

The pancarpal arthrodesis Plate is selected which most completely covers the metacarpals. The carpus is aligned and the implant placed such that Screw Hole 5 directly overlies the radial carpal bone and the distal portion of the Plate lines up over metacarpals III and IV. Small Kirschner wires are placed through the Plate and between metacarpals III and IV to maintain alignment distally. Autologous cortico-cancellous bone graft is collected from the proximal humerus and packed into the joint spaces. After bone grafting, the alignment of the carpus is reviewed and once the carpus is straight, the metacarpals aligned and the proximal portion of the Plate lying comfortably over the distal radius.

A 2.5 mm Hole is made into the radial carpal bone through Hole 5. The Plate is secured to the radial carpal bone using a single 3.5 mm bone Screw. Carpal alignment is reviewed then, using the “load” Guide, a pair of 2.7 mm Screws are placed into metacarpals III and IV through Plate Holes 8 and 9. Similarly, using a load Guide and a 2.5 mm Drill, Screws are placed into Plate Holes 1 and 4. The placement of remaining Screws is routine, though care must be taken to respect the angle of the most distal Screw Holes, 10 and 11.

Retracted tendons and neurovascular structures are replaced prior to routine wound closure. A light dressing is placed to control post- surgical swelling and the patient is restricted to limited leash exercise until there is radiographic evidence of significant bony union.

This dog jumped a 3ft wall not realising there was a 15ft drop on the other side. He sustained a fracture-dislocation of metacarpals IV and V and dislocated metacarpals II and III from the carpal joint itself.

Immediately Post-op:

The Castless PCA plate was applied to metacarpals III and IV, thereby stabilising the fracture at the same time. A small ‘K’ wire was used to stabilise fractured metacarpal V. An additional screw was used to secure metacarpals II and III back into alignment.

Post-op 7 weeks:

Complete fusion and bony healing.

3 months: Dog returned to full exercise with only mild stiffness reported.

6 months: Dog remains sound.

Manufacturing Process

1. The TPLO Plates begin life as large sheets of 316 medical grade stainless steel. 2. We use an industrial LASER to cut out the TPLO blanks. 3. We then de-burr the Plate ready for the milling process. 4. The round and compression Screw Holes are milled. 5. The Plate is de-burred after the milling process. 6. The Plate is pressed into shape. 7. The TPLO Plate is then manually polished. This is a highly skilled procedure as in the wrong hands it is very easy to “polish in” magnetism. This is the last thing that we require in a stainless steel implant.8. The second polishing stage involves the Plate being placed within thousands of tiny vibrating ceramic pieces. 9. The Plate is finally electropolished, laser marked and then packaged for use.

Screw Process

1. All of our bone screws begin as a bar of 316 medical grade stainless steel.2. We turn the bar to produce the major dimensions (and in this case the self tapping part is also milled).3. The hexagon slot is milled into the screw head.4. The de-burring process removes all sharp edges.5. The 1st polishing process is applied (prior to the threads being turned).6. After the threads have been turned the screw is now ready for final cleaning and polishing.7. Electropolishing finishes the process. The screws will now be laser marked to identify their origin and packaged.

Predisinfection: For predisinfection immerse the telescope in an aqueous disinfecting solution after use (see disinfecting solutions described on the right side).

Crude Soiling: Clean the telescope thoroughly after use. Remove crude soiling with plenty of water. Do not use ultrasonic cleaning.

Disassembling: Disconnect the adaptors from the light guide connector.

Mild Detergent: Further cleaning should be done by immers-ing the telescope in a mild detergent solution. Use cloths and soft brushes.

Rinsing with Distilled Water: After cleaning, rinse the telescope with deionized (distilled) water to prevent coating. Encrustation on optical surfaces may be removed by using polishing cotton.

Optical Surfaces: In general, optical surfaces should be cleaned using a cotton swab soaked with 70% Ethanol.Never apply metal tools to the telescope.

Drying: Finally dry the instrument carefully with tissues, gauze or compressed air. Reconnect the adaptors to the light guide connector.

Cleaning:

To achieve the sterility assurance level (SAL) of *10–6 MGB recommends the following steam and gas sterilization methods.

Steam sterilization

After disinfecting and cleaning, the telescopes have to be packed into special sterile containers for endoscopes or designated packaging. MGB telescopes qualify for the following parameters (steam sterilization according to ISO 111134-1993).

• Temperature: 134°C• Pressure: 2,2 bar• Effecting time: 5min to max 7,5min• Batch time: 20 to 30 min

Remove telescopes from the tray after they have cooled down (room temperature). Do not chill them.

Sterilization:

Only thoroughly cleaned telescopes should be disinfected. We recommend disinfectants by the following manufacturers:

The telescopes are also tolerable to 100 % alcohol.

Put the telescope into the disinfecting solution. The telescope has to be completely moistened with solution. Follow the soaking time and solution concentration prescribed by the manufacturer. A higher soaking time or concentration can lead to damages of the telescope. Rinse off disinfecting residues on the telescopes with sterile water, after drying with sterile cloth swabs wrap it into sterile cloth. For temporary storing, keep the telescope wrapped in cloth in a closed and sterile container.

Disinfection:

Gas sterilization

ATTENTION: The safety instructions for gas sterilization have to be observed and biological indicators have to be used. After disinfecting and cleaning the dried telescopes have to be packed into the designated sterile packagings. MGB telescopes qualify for the following parameters (ethylene oxide sterilization according to ISO 111135-1994).

• Pressure: max. 2,2 bar

Both overpressure and negative gas sterilizers can be used.

Terms and Conditions Definitions1.1 Company means Orthomed (UK) Ltd1.2 ‘Customer/ means the veterinary practice or any person to whom theCompany has agreed to supply goods.1.3 Goods’ means the products which the Company is to supply to the customer in accordance with these conditions.1.4 ‘Writing’ includes telex, cable, facsimile transmission, e-mail or similar means of communication.

2. Breakages and ShortagesIn the case of obvious shortages or suspected damage to deliveries of goods, the drivers delivery note must be signed accordingly by the person accepting the delivery on behalf of the customer. All claims for broken or missing goods should be reported to the Company no later than three days from the date of delivery. Where independent transport is used failure to sign or indicate on delivery notes any shortage and/or damage will invalidate the Company claim against the carrier involved and therefore the customers claim against the Company.

3. Returned Goods Policy3.1 Under normal circumstances, goods will not be accepted for return for credit other than for the following reasons:a) To correct a Company delivery or order error,b) In response to a recall instigated by a manufacturer.c) Where the products or packages are alleged to have been faulty, damaged or tampered with prior to delivery.d) Where the products have been offered on a pre-arranged sale or return basis.e) Where the Company agrees, in advance of their return, to accept goods not covered by any of the above criteria.3.2 Goods Returned to Correct a Company Errora) The goods must be returned within 7 working days of receipt. Goods requiring temperature-controlled storage must be notified to the Company no later than the next working day and must have been stored in accordance with the manufacturer’s recommendations whilst on the customer’s premises. Goods must be adequately packaged to ensure that no damage is caused during their return to the Company. b) All returned goods must be in perfect original condition; not marked in any way; should not show any evidence of being tampered with.c) To be considered for return for credit, the goods must be accompanied by a copy of the original invoice or delivery note, or the number of the specific invoice on which the goods were supplied, together with a reason for their return. If this information is missing, there will be a delay in issuing a credit. 3.3 Returns where the products or packages are alleged to have been faulty damaged or tampered with prior to delivery:a) Details of the fault or damage should be notified to the Company as soon as possible in order that the manufacturer can be alerted if appropriate.b) Goods must be packed separately and accompanied by a copy of the invoice or delivery note (as in 3.2(c))3.4 Where the Company agrees to accept the return of goods not covered by any of the criteria recited in clauses 3.2- 3.4.Goods will only be accepted by prioragreement with the Company and must be accompanied all the information detailed in 3.2(c) in addition to the name of the person who has authorised the return. Please note’ Goods returned under this category may be subject-to a handling charge.3.5 Goods returned to stock.Goods returned by customers for any of the above reasons will only be considered for credit if;a) It is known that they have been stored in accordance with the manufacturers recommendations.b) They have been assessed and accepted by a person authorised by the Company so to do. The assessment will take into account the nature of the product, any special storage conditions required and the time elapsed since the goods were supplied.3.6 Special Orders. These products will not be accepted for return or credit unless supplied as a result of Company error. In such cases, the product must be returned within 7 working days of receipt.3.7 Packaging. All returned goods must be securely packaged to prevent damage during the return journey.3.8 Date Expired Good. Date Expired Goods will only be accepted for credit in accordance with the terms laid down by the manufacturer concerned.

4. Price of GoodsGoods will be invoiced at the prevailing price at the date of despatch to the customer. The price of goods shall be subject to alteration by the Company without prior notice to the customer; the price is exclusive of any Value Added Tax for which the customer shall be additionally liable to pay the Company.

5. Terms of Payment.5.1 The customer shall pay the amount of the invoice nett 30 days from the date of the Company’ invoice unless by prior agreement in writing from the Company. If the customer fails to pay on the due date, the Company reserves the right to:

a) Suspend any further deliveries to the customerb) Appropriate any payment raised by the customer to such of any other invoices raised by the Company for goods supplied to the customer as the Company may think fit, notwithstanding any purported appropriation by the customer and:c) Charge the customer interest (both before and after any judgement) on amountsunpaid at the rate of 2.5% per month until payment is made in full. Notice ofinterest charges will be served in writing on the customer.

6. DeliveryAny dates/times quoted for delivery of the goods are to be treated as estimates only and the Company shall not be liable for any loss, injury, damage or expense arising from delay In delivery of the goods howsoever caused.

7. Force Majeur7.1 The Company shall not be liable to the customer or be deemed to be in breach of the Contract by reason of any delay in performing, or any failure to perform, any of the Company obligations in relation to the Goods, if the delay or failure was due to any cause beyond the Companies reasonable control, without prejudice to the generality of the foregoing, the following shall be regarded as causes beyond the Companies reasonable control:a) Act of God, explosion. flood, tempest, fire or accident;b) Strikes, lock-outs or other industrial actions or trade disputes (whether involving employees of the Company or third parties);c) War or threat of war, sabotage, insurrection, civil disturbance or requisition; d) Import or export regulations or embargos:e) Difficulties in obtaining raw materials, labour, fuel, parts or machinery;f) Power failure or breakdown in machinery.

8. LiabilityGoods are sold on the understanding that the customer is solely responsible for determining the suitability of the goods for the purposes for which the customer intends to use them. Accordingly the Company liability for any loss or damage suffered by the customer and arising by defects in the goods, shall be limited to the invoice price of the goods in respect of or in relation to which loss or damages is claimed. The Company will not accept under any circumstances liability for consequential loss, damage or loss of profits arising from the use of the goods, service, information and advice,

9. Title and RiskUnless otherwise agreed in writing between the parties prior to delivery, the titleto the goods shall not be transferred to the customer until the full purchase price has been paid to the Company. The risk of loss or damage to the goods shall pass to the customer on the date that the goods are received and accepted by the customer

10. Solvency10.1This clause applies if:a) The customer makes any voluntary arrangement with its creditors or becomessubject to an administration order or (being an individual or firm) becomesbankrupt or (being a company) goes into liquidation (otherwise than for thepurpose of a solvent amalgamation or reconstruction);b) An encumbrance takes possession, or a receiver is appointed, over any of theproperty or assets of the customer;c) The customer ceases, or threatens to cease, to carry on business;d) The Company reasonably apprehends that any of the events mentioned above is about to occur in relation to the customer and notifies the customeraccordingly.10.2If this clause applies then, without prejudice to any other right or remedy available to the Company, the Company shall be entitled to cancel the Contract or suspend any further deliveries under the Contract without liability to the customer. If the Goods have been delivered but not paid for, the price shall become immediately due and payable notwithstanding any previous agreement or arrangement to the contrary.

11. IndemnityThe customer agrees to indemnify the Company against all loss, damage, costs, legal costs and professional and other expenses of any nature whatsoever incurred or suffered by the Company or by a third party as a result of the use of the Goods. The breach of any legislation relating to the use of the Goods or as a result of any breach of the Contract by the customer.

12. JurisdictionThese conditions shall be construed in accordance with and governed by English Law. Each of the parties irrevocably submits for all purposes in connection with the Conditions to the exclusive jurisdiction of the Courts of England.
