Chief complaint

ปวดหลงเปนๆหายๆ 1 ปกอนมาโรงพยาบาล

Present illness

- 1 ปกอนมาโรงพยาบาล เรมมอาการปวดหลงบรเวณตรงกลางสวนกลาง ปวดแบบราวๆเปนครงคราว เปนมากขนเวลายกของหนก ผ ปวยจงใสทพยงหลงมอาการดขน

- 10 เดอนกอนมาโรงพยาบาล มอาการปสสาวะแลวแสบ ปสสาวะบอยขน จงไปโรงพยาบาลกบนทรบรพบตดเชอในทางเดนปสสาวะ ไดรบการรกษาจนอาการดขน

Present illness

- 6 เดอนกอนมาโรงพยาบาล ยงมอาการปวดหลงแบบเดมอย มอาการไขเปนๆหายๆ ปสสาวะตอนกลางวน 3-4ครง กลางคน 3-4 ครง ไมมอาการปสสาวะแสบขดเปนฟองหรอตะกอน ไมมปสสาวะเปนเลอด ไมปวดทองหรอทองนอย ไมมคลนไสอาเจยน ไมมทองเสยถายเหลว ทโรงพยาบาลกบนทรบร ไดท าการ x-ray พบเปน abnormal

calcification ท left kidney ไดท าการวนจฉยวาเปน staghorn calculus และไดท า IVP (17/7/57)พบวา left

kidney ไมม excretory function จงสงตวมารกษาตอทโรงพยาบาลศนยการแพทย

Past history

- มโรคประจ าตวเปน Dm type 2 ,hypertension,thyroid

goiter,mild aortic stenosis

- Para 3-0-0-3

- มประวตการใชยาสมนไพรและยาเสน - ปฏเสธประวตการรบเลอดหรอการไดรบอบตเหตรนแรง - ปฏเสธประวตการแพยาแพสารอาหารหรอสารเคม - ปฏเสธประวตโรคมะเรงหรอโรคตดตอทางพนธกรรมในครอบครว - ปฏเสธประวตการดมสรา สบบหร และการใชสารเสพตด

Current medication

- metformin 500 mg 1x3 po pc

- folic acid 1x1 po pc

- amilodipine 5 mg 1x1 po pc

- levothyroxine 0.1 mg 1x1 po ac

- cyproheptadine 4 mg 1x3 po ac

- simvastatin 40 mg 1/2x1 po hs

- glibencamide 5 mg 1x1 po ac

Physical examination

• Vital sign : BP 160/100 mmHg , PR 92 bpm ,

BT 36.7 c , RR 20/min

• General appearance : A Thai female , good

consciousness , not in pain , not pale , no

jaundice

• Skin : no rash , no abnormal pigmentation

Physical examination

• HEENT ;

– Eye : not pale conjunctiva , anicteric sclera

– Ear : normal shape , no lesion on the auricle

– Nose : no deformity , no sinus tenderness

– Throat : moist and pink buccal mucosa ,no tonsil

enlargement , no glossitis

• Neck : Multiple thyroid nodule , firm

consistency , smooth surface , not movable ,

not tender

Physical examination

• Lymph nodes : no palpable enlarge lymph

nodes

• CVS : JVP 3 cm from sternal angle , PMI at 5th

intercostal space mid clavicular line , no

heaving , no thrill , normal S1S2 , systolic

ejection murmur at right upper parasternal

border , no S3S4 gallop , no carotid bruits

Physical examination

• Respiratory system : trachea in midline ,

symmetrical chest expansion , resonance on

percussion , normal & equal breath sound , no

crepitation , no wheezing , no vocal and tactile

fremitus

• Abdomen : no abdominal distention , no

superficial vein dilatation , no visible hernia or

mass , normoactive bowel sound , soft , not

tender , no guarding , liver & spleen are not

palpable , liver span 8 cm

Physical examination

• Kidney : bimanual palpation – not palpable ,

no CVA tenderness

• Extremities : warm hands and feet , no

clubbing of fingers , no pitting edema ,

capillary refill <2 sec

Physical examination

• Nervous system : E4M6V5 , intact memory ,

no dysarthria , normal tone , motor power

grade V all extremities , Deep tendon reflex 2+

all , no sensory loss , normal gait , cranial

nerves are within normal limit , Cerebellar

signs are normal

• no osteolytic/blastic letion, no bone fracture

• no subcutaneous emphysema, no soft tissue swelling

side เรมเหนexcretion ชดขน และเหนลกษณะของ ureter ยาวลงมาถง pelvic area โดยเหนไมตลอดความยาวของสาย ureter

เนองจาก ม peristalsis

• แสดงภาพ IVP ท 20นาทเรมม สารทบรงส ใหเหนใน urinary bladder โดยความเขมของbladder

ทางขวาเปน radiopaque

มากกวาทางซาย และยงไมเหน

ureter ดานซาย

• แสดงภาพ IVP ท1ชวโมง สารทบรงสมาอยใน urinary

bladder หมดแลว เหนเปน well-define bean

shape radiopaque pattern ทbladder

แสดงภาพ IVP ท 1 และ 2 ชวโมง ท2ชวโมงพบวา bladder แฟบลงไป คดวานาจะจาก voidding ของคนไข

แสดงภาพ IVP ท 2 และ 3 ชวโมง ท3ชวโมง มสารทบรงสใหเหนใน

bladderอกครง คดวานาจะมาจาก kidney ทางฝงซาย ทคอยๆ excrete ออกมา

แสดงภาพ IVP ท 3 และ 4ชวโมง ท4ชวโมงเปน post-void

ส าหรบประเมณความสามารถในการขบปสสาวะของคนไข

Patient Education

• ใหความรเกยวกบตวโรค

• บอกถงการรกษาทใหกบผ ปวย

• แนะน าการปองกนการเกดนวซ า – ดมน ามากๆ วนละ 8 แกว – หลกเลยงน าอดลม

– รบประทานเคมใหลดลง

– งดอาหารทม oxalate สง เชน ถว ชอกโกแลต ฯลฯ
