CARROLL UNIFIED SCHOLARSHIP FORM APPLICATION(The following scholarships are available for Carrollton High School and Carroll County Seniors and Graduates by

using the unified form. The complete application, with all attachments, is due March 1.)

ASHTON MEMORIAL SCHOLARSHIPSEligibility: Carroll County High School Seniors (may be renewable)Information: Renewal letters will be mailed, no need to reapply. One scholarship will be

renewable for four years. $5000.

BELL-HERRON FOUNDATION SCHOLARSHIPSEligibility: Carrollton High School seniors or graduates who plan on attending any post-

secondary education training or college. Information: Students need to reapply each year, priority will be given to high school seniors

and college freshmen.

CARROLLTON BUSINESS WOMEN SCHOLARSHIPEligibility: Female seniors of Carrollton High School who wish to continue their education

past the high school level in a business related field.

CARROLL COUNTY FARM BUREAU Eligibility: Carrollton High School seniors or graduates who plan on attending or have

entered any accredited institution offering post-secondary education.Information: The applicant or his/her parents/guardian must have been a Farm Bureau member

for at least two years. Two letters of reference are required.

CARROLL COUNTY HISTORICAL SOCIETYEligibility: Carrollton High School seniors who plan on attending any accredited institution

offering post-secondary education.Information: Scholarship is NON-renewable and recipient must submit an oral report to

the membership of the Historical Society at the annual meeting in the year following the receipt of the scholarship.

CARROLLTON ROTARY SCHOLARSHIPEligibility: Carrollton High School seniors who plan on attending any type of post secondary

education.

DELLROY INDEPENDENT ORDER OF ODD FELLOWS SCHOLARSHIPEligibility: Carrollton High School seniors and graduates who attended Dellroy Elementary.

DELLROY RURITAN SCHOLARSHIPSEligibility: Carrollton High School seniors and graduates who attended Dellroy Elementary

School and plan on attending any type of post secondary training/education.

MARY KEAN WHITEEligibility: Seniors of Carrollton High School/Malvern High School planning to pursue an elementary education with a 3.0 GPA.

MOYNE SMITH MEMORIAL SCHOLARSHIPEligibility: Carrollton High School seniors and graduates who plan on attending (or are

attending) any type of college training/education.

MR. & MRS. JOHN NEIDER SCHOLARSHIPEligibility: Carrollton High School seniors attending West Virginia University. Priority will

be given to students pursuing a field in agriculture.

PERRY TOWNSHIP SCHOOL MEMORIAL SCHOLARSHIPEligibility: Perry Township high school seniors and graduates planning to enroll, or are

currently enrolled with at least a two year course of study to pursue.

RUSSEL AND HENRY COLE SCHOLARSHIPEligibility: Carrollton High School seniors planning to attend an accredited institution of

study.

SCHOLARSHIP 19Eligibility: Carrollton High School graduates who are currently attending a college or

university to complete a Bachelor’s Degree program.

THE RICHARD C. AND JOYCE J. HANNON SCHOLARSHIP FUND FOR THE ARTS, LITERATURE AND COMMUNICATION

Eligibility: Carrollton High School seniors and graduates who are planning to enroll in college and study in the field of performing arts, which could include speech, theatre, music, drama, speech and theatre education, performance and theatre arts, and television and radio broadcasting.

CIVIC CLUB SCHOLARSHIPEligibility: Carrollton High School graduates who are currently attending a college or

university to complete a Bachelor’s Degree program.

MELVIN L. AND FRANCES E. TOLSON SCHOLARSHIP TRUSTEligibility: Carroll County seniors and graduates who are seeking training and instruction to

become licensed pilots. Also, Carroll County seniors and graduates who are enrolled or accepted into a college/university and majoring in a program of art education or fine and preforming arts.

1. Compose an autobiography of at least 100 words and not more than 500 words in which you include all pertinent information concerning your place and date of birth; parent’s names, age, occupation and family size; your membership in local organizations such as the 4-H club, boy/girl scouts, youth groups or other such clubs and your goals in the area of higher education. This should be constructed grammatically correct with correct spelling, and should be typed on regular 8 1/2 by 11” paper with correct margins and double spaced. Make every effort to present quality writing because it is your introduction to the selection committee. Three copies should be made of this biography. (This will replace the personal essay requirement)

2. You are to submit two recommendations from professors of your choice. If you are already in your major field of study one must be from a professor of that department. The other one may be from a professor of that department. The other one may be from any professor or instructor you have had for at least one semester. Please ask them to provide three copies of the recommendation.

3. One character reference is required. This should not only attest to character but also address the issue of your moral priorities and commitment. This reference can be from a youth advisor, 4-H advisor, minister, etc., Please ask them to provide three copies of recommendation.

4. Also include college student ID number, total mount of loans already incurred and a student photo.

1.
