19:45, theCarletonville Policewere cal-led to the home of young Sizwe Bene-dict Moleko (17), one of twin brothers,who startedGr 12 as a prefect this year.Upon arrival, EMS (Emergency Medi-cal Services) staff was already at thescene, but it was too late. It seems theyear had not started well for Moleko,as he decided to end his life on Sunday,hanging himself with a blue tie and belton thewashing line of the family’s homein Lang Street, Carletonville.A message to Moleko from a friend

on Facebook reads;“Ah! Boy Boy, remem-ber how you were soscared of playing rug-by when you came tohigh school and thenI came and spoke to uand Schaba about it.Then you ended upwearing the first teamjersey in 2012? Ahmy, my boy that wasan overwhelming fee-ling. Ah, now you’regone, gone for goodwhich kills me too.Friend, if you thoughttwice about what youwanted to do hey,your future was everso bright. But now you left all of that

just because of that oneproblem you had. Nowyour parents, sister, yourbetter half Sechaba, yourgirl and all your friendsand family are hurting be-cause of what you did.”The Carletonville Police

Five accusedmen fromKhutsong, arre-sted on December 19 for the murdersof gangsters in the townshipon Novem-ber 3 last year, applied for bail thisweek.In an already heated case, the defen-

ce advocate for the five accused men,Mr Katlego Mofokeng started by rea-ding affidavits by the men who havebeen charged with, amongst otherthings, murder, attempted murder, ar-son and public violence.The affidavits by the accused men

state clearly what their fixed addressesare, that they all either have their ownbusinesses or are employed and are so-le-breadwinners with dependents.Furthermore, the accused men offe-

red the court an amount of R 3 000each for bail, are all South African citi-zens and pose no flight risk. They alsostated that they will adhere to the con-ditions of bail, should they be grantedbail by the magistrate, Mrs AdeleMaass.The accused men all deny having

been around or participating in themurders of six alleged gang membersof the Cassanovas gang in Khutsong onNovember 3 last year. One of the accu-sed, regularly in question, Mr Happy

Lefakane, stated in his affidavit that hewas not in Khutsong on the day themurders took place, but was, in fact,at an EFF Launch in Muldersdrift andonly returned to the township at 19:30.His advocate has a supporting affidavitwith regards to this.Similarly, another accused alleged

that he was called by someone knownto him, about the murders taking pla-ce. He says that when he got to the sce-ne, police officers were already there.The other accused men have similardefences.In various press releases to the natio-

nal media by Brig. Neville Malila, Pro-vincial Head of Corporate Communica-tion in Gauteng, it was clearly statedthat six people were murdered duringthemob-justice events on November 3.The ages of the deceasedwere also pro-vided by Malila, the oldest deceasedman being James Magagula, 61 yearsold from theChrisHani informal settle-ment.When called to the witness stand, ho-

wever, the investigating officer, Col. Jo-hannes Mokoena, who has 30 years ofexperience as a police officer and isheading this investigation by a team ofthe Gauteng Provincial InvestigationsUnit, spoke only of five murders andsaid that the oldest deceased victim

was 42 years of age.Mokoena opposed the bail of the five

accusedmen, saying thatwitnesseswe-re scared of the accused and that somewitnesses stayed in the same areas assome of the accused men in Khutsong.He said that some people had leftKhutsong in fear that the accused menwould get bail and that he would haveto call and inform them of the outcomeof the bail application. He said themenalso tried to intimidate police officersbefore their arrests were made.Mofokeng askedwhyno intimidation

cases had then been opened, turned tothe gallery filled with Khutsong peopleand said, “Isn’t it strange that the entiregallery is here to support these men,”Mofokeng said. “Usually when a com-munity is angry and they want to oppo-se bail, they will protest outside thecourt, yet it is evident that these menare supported by the community all theway. Mokoena said that he had a wit-ness statement, but could not disclosethe name of the witness, in order toprotect him/her. When asked how farinvestigations are, Mokoena said thatthere are still outstanding witness sta-tements. He said thatwitnesses arewil-ling to speak, but do not want to givestatements inwriting, out of fear of inti-midation. “This is a matter of Commu-

nity against Community” is how Mo-koena referred to the case. He also saidthat more arrests were going to be ma-de in connection with the mob-justiceevents.When asked whether he knew of any

rival gangs in Khutsong, Mokoena saidhe knew of none and also said that hedid not have experience in dealing withcases such as these.The only “group” in question, cur-

rently, is the group known as the “shit-cops,” butMokoena says that he has noproof that the five accused men all be-long to any formation, as he has no-thing in writing which confirms this.The prosecution’s, Mr. Victor Sime-

lane aswell as the head of theKhutsongDetectives, Col.OckertBritz, producedprofiles of the accused men. Althoughone specific accused man’s profile wasmore thoroughly discussed than any ofthe others, various arrests and convic-tions of the accused men were used bythe prosecution to oppose bail.At this point, the evidence possessed

by the prosecution to justify the arrestsof the accused men as well as keepingthem in custody for almost a month isone witness statement and one Carle-tonville Herald newspaper article.The case was postponed to 24 Janu-

ary.

A 15-year-old girlwas mercilessly ra-ped and then robbedin the veld betweenKhutsong’s Extensi-on 5 and Carleton-ville.The girl, who is

from Extension 2 inKhutsong, was hea-ded to Carletonvilleon November 30 lastyear, and arrived atthe taxi-rank ataround 14:00. Afterthat, she walked tothe Gateway Mall.She had been givenR 150 by her motherto purchase a newcell phone. There she bumped into a

friend who offered to help her shop fora cell phone. The phone she wanted,however, cost R 400 which she could

not afford. The girls thenwent to the Wimpy forlunch. The friend thenasked her to accompanyher to Extension 5, Khut-song, to collect moneyfrom her brother who li-ves there. The visit to Ex-tension 5, however, tookmuch longer than the girlwas expecting, becausethe brother was busy andit has started raining. Al-though the brother volun-teered that she must ra-ther sleep over, the girl de-cided towalkhome.Therewere no taxis available.On her way back to Car-

letonville, an unknown man, armed

with a knife, threatened the young girland started undressing her, first hertrousers and then her shirt. He thenproceeded to rape her three times. “Iwas crying,” she says. Whilst rapingher, he allegedly told her not to look athim. The man also went as far as rob-bing the young girl of the R150 she hadbeen clinging to for a new cell phone,aswell as the blue Nokia she possessedat that stage. The suspect is a young

man of around 20 years of age andabout 1.75m tall. He has a coffee-co-lour complexion, a beard, speaksTswana and was last seen wearing ahooded jacket as well as a greyish jac-ket with blue lines on it. He is armedwith anOkapi knife. Anyonewith infor-mation regarding the whereabouts ofthis man is asked to call the investiga-ting officer, Const. Annelize Cloete at078 960 9760 as soon as possible.

An employee of the Carleton-ville ABI (AmalgamatedBeve-rage Industries) depot, wasconned by men posing as po-lice officers.On January 10 at around

11:20, the complainant leftthe ABI depot in Carletonvil-le’s new industrial area anddrove toward town, with alarge sum of money which hewas going to bank for the de-pot. Before the man couldthink of stopping at a bank,he heard sirens, looked be-hind him and saw a black/dark blue Volkswagen Golf 5pull up behind him with bluelights on the dashboard. Themen in the “police” vehicle in-dicated that the complainantshould pull over and stop,which is what he did.The men got out and intro-

duced themselves as policeofficers, asking to search hisvehicle. They found the mo-

ney in thebagon thebackseatand asked him what it wasfor. “The complainant explai-ned that it was the ABI De-pot’s money,” tells Col. Wes-sel Prinsloo. “The con-policeofficers said that they did notbelieve him, and that heshould drive back to the de-potwith themtoconfirm this.Once the complainant was

in the “police” vehicle, onesuspect pointed a firearm athim. They drove out on theP111 road with the complai-nant, took the bag of moneyand dropped him off in thefield near Westonaria.Although no other inci-

dents involving this vehicleand it’s con-occupants havebeen reported in the area,anyone who knows anythingabout the vehicle or spots it inor around Merafong mustcall DetectiveWarrant OfficerSoon Theron of the Carleton-ville SAPS immediately at082 854 1017.

Although the situation at Blyvooruit-zicht Gold Mine has stabilised, the mi-ne’s problems may still not be at anend.The problems at Blyvoor, the oldest

mine in the Carletonville area, startedinAugust last yearwhen themine’s pre-vious owner, the company Village MainReef, also known as Village, indicatedthat it will not take responsibility forthe mine’s losses anymore. In themeantime, DRDGold, the companywhich had owned Blyvoor before, alsoindicated that does not want anythingto do with themine. Since this time Vil-lage has also claimed that the sale ofBlyvoor was not fully completed, an al-legation denied by DRDGold.As a result of the mine’s troubles it

wasplacedunderprovisional liquidati-on in order to see whether the mine orsome of its assets could not be sold. InDecember a disaster was averted whentwo prospective buyers were able to ne-gotiate with Eskom not to cut the elec-tricity supply to the mine. If the electri-city was cut, residents of the villagewould be without power, water and se-werage services while water wouldflood the mine.National government departments

involved in mining did not put a stopto Eskom’s action while the MerafongCity Local Municipality also indicatedthat it was not going to get involved in

the matter.According to one of the provisional

liquidators appointed for Blyvoor, MrLeighRoering, allwas, however still notplain sailing at the mine.Roering told the Herald that the sale

of the Blyvoor village to a property de-veloper from Rustenburg, Ms Liz Ge-ricke, was still in the process of beingfinalized. AlthoughGericke has startedto take over the running of the village,some prerequisites still had to be metbefore the village could officially besold.One of these conditions are that Bly-

voor’s must officially be proclaimed asa town.In the meantime, the company Gold-

richHoldings is also still in the processof buying Blyvoor’s mining activities.“We wrestled the gold plant back

from the zama-zamas and they also puta new security company in place,” saysRoering.He added that Goldrich Holdings al-

so indicated that it wants to continuemining at the one shaft that is still inoperation at the mine.To show that they have the capacity

to run the mine, Goodrich Holdingswas however supposed to pay over acertain amount of money by the end ofDecember.Although the company has not been

able to do so, the deadline for the pay-ment has been extended after unionsrepresenting workers at the mine

agreed to such a step.“I hoped that everything at Blyvoor

would have been sorted out by now butit is still not the case,” Roering told theHerald.

He added that, if Goodrich Holdingsis not able to come up with the amountof money that is required, the processto sell the mine to them “would revertto square one”.

An elderly resident of Khutsong had arather unpleasant festive season as sheand her family had to endure sewageleaking into their yard.According to Ms Miriam Malete (67)

of house no 4855 in Extension 2, shehas been experiencing problems since1996 with sewage leaking into heryard. Each time sewage spilled into heryard after the main sewer line becameblocked.Since September last year the pro-

blems became worse as Malete’s yardwas constantly flooded with sewage.Although municipal workers came tofix the problem during October, the se-wage leak soon started again.“I went to report the problem to the

municipality two times,” says Malete,

and explained that one of the times shewalked directly to the offices of the de-partment that is responsible for dea-ling with sewerage issues. Despite hertrouble, no-one came out to assist overthe Festive Season. “I am so tired. OverChristmas the problem was very bad.I had to close the house and went tomyfamily in Tswana Section just to getaway from the stench,” a frustratedMa-lete told theHerald lastweekThursday.Malete added that she also believes

that the stomach trouble from whichshe and her two grandchildren, Prince(15) and Princess (9) are suffering, areas a result of the sewage constantly lea-king into their yard.

“It is so bad we cannot sleep,” Male-te’s neighbour, Ms Miriam Letlhake,told the Herald on Thursday.At the time sewage had not only

floodedMalete’s backyard but was alsostreaming into the road in front of herhouse. Somuch of the sewage had stre-amed onto the road that some of thefoul-smelling discharge pooled at theend of the street.According to Malete municipal wor-

kers had told her previously that theproblem would only be sorted out if anew pipe was to be installed, but thatthere is curently not enough money forthis task.As themunicipality had said, various

times in the past, that residents wereaddressing their complaints to thewrong officials and that they had to re-port such issues to the municipal callcentre, the Herald’s journalist called

this centre to again report the matterlast week Thursday.In the past the municipality had dis-

puted residents’ claims that they hadreported their problems if they couldnot provide reference numbers, whichwere supposed to be given to them byofficials at this centre.When the Herald however spoke to a

municipal official who answered at thecall centre and particularly asked forsuch a reference number, she deniedthat any such numbers could be givento the public.“We have never given out any referen-

ce numbers for complaints since thiscentre has opened, the official, whosename is in possession of the newspa-per, claimed.The same Herald journalist had, ho-

wever, obtained reference numbers forcomplaints from the call centre in thepast. The call did however lead to re-sults as Malete indicated that munici-pal workers did visit her home last Fri-day and Saturday. By Monday sewagewas however still leaking into her yard.“The sewer line in Extension 2 which

services Mrs. Malete collapsed and af-fected many houses in the area. As aninterim measure the manholes on thesewer line are cleaned through suction.A contractor has been appointed to re-route the sewer line. It is expected thatthe projectwill be completedduringFe-bruary. The operator who did not givethe number will be identified and ap-propriate action taken. All call centrecalls must be logged with a number,”a municipal spokesman responded tothe Herald’s questions regarding thematter on Tuesday.

swembad, was daar net ‘n enkele toilet,een in die dames-kleedkamer, wat ge-werk het. Die vloer van die manskleed-kamer was onder water as gevolg vanpype wat sover vasgestel kan word,reeds sedert die einde van Oktober, toe

vandale op die ter-rein toegeslaanhet, gesteel is. Diemeeste wasbakkeis ook tydens die-selfde voorval ge-steel maar is nooitvervang nie en wa-ter het uit pype aandie buitekant vandie gebou gespuitna dié ook in die-selfde voorval be-skadig is.Glasstukke van

die gebreekte ven-sters van ‘n buite-gebou, wat reedsmeer as ‘n jaar ge-lede beskadig is,het ook in die grasgelê.Intussen was die

gras by die swem-bad ook lanklaasgesny en het dit opplekke bykans kuithoogte op die ter-rein gegroei.Intussen was daar ook geen toiletpa-

pier in enige van die kleedkamers nie.“Die munisipaliteit kan bevestig dat

daar gedurende Oktober skade by dieswembad aangerig is na diewe toege-slaan het. Daar is ‘n totaal van ses toi-lette, drie vir mans en drie vir vroue.Van dié ses is daar vier wat werk. Dietoilette wat as gevolg van die diefstal niewerk nie sal vinnig herstel word. Onsis ook bewus dat daar probleme metgrassnyers was, maar dit is uitgesor-

teer en die gras is nougesny. Die gebreekteglas is ook verwyderen toilette word daag-liks nagegaan. Daarbehoort nie ‘n tekortaan toiletpapier tewees indien dit nie op‘n spesifieke dag ge-steel is nie,” het ‘nwoordvoerder vandie munisipaliteit inDesember aan dieHerald gesê. Perso-neel van die Heraldhet die swembad eg-ter weer op Maandag,13 Januarie besoek.Met hul aankomswasdie swembad gras-groen. Hoewel daaraandieHerald-perso-neel, wat hulself niegeïdentifiseer het nie,gesê is dat die swem-bad nie vir die pu-

bliek oop is nie, is kinders wat aange-dui het dat hulle van Kokosi kom, weltoegelaat om te swem.“As gevolg van baie reën was die ph-

balans in die swembad nie reg nie enhet dit groen geword. Dit is egter regge-stel en die swembad is nou weer vir al-mal oop. ‘n Tydelik plan is met die toi-lette gemaak aangesien dit deur vanda-le beskadig is en in die afsienbaretoekoms herstel sal word,” het die mu-nisipale woordvoerder die afgelopeweek aan die Herald gesê.

Fochville-swembad laat veel te wense

The scope of Internal Audit work not only entails evaluating the adequacy and effectiveness ofinternal control, IT management, risk management and governance processes of the Company butalso the verification of operational effectiveness and efficiencies and devising innovative waysto improve overall performance of the organization.

Internal Audit Function reports to the Audit and Risk Management Committee and the ChiefExecutive Officer.

The successful Bidder might be requested to meet with the External Auditor from time to time.Notwithstanding, both the Internal and External Auditors have to play specific roles within theorganization to improve performance, governance and compliance.

The proposal should include:

1. All relevant perceived strengths and weaknesses of the firm bidding for the service

2. The summary of the Bidder’s mission statement, the vision statement , values and long termstrategies and objectives

3. A proposed plan of action to achieve the objectives of Internal Audit function.

4. A list and curriculum vitae of members of staff who will be allocated to this Internal Audit work.Changes to staff allocation must be made in consultation with Midvaal Water Company.

5. The Broad Based Black Economic Empowerment status and plans in this regard.

6. A break-down of the hourly tariff of staff members to be involved in the performance of the InternalAudit and expected hours to be spent.

7. A comprehensive budget showing service activities proposed, charge-out rates and budgetedhours per activity, travelling costs and all other costs to be charged.

8. A list of references of previous and current Internal Audit appointments.

9. A copy of the Audited Financial Statements for the past two years.

10. The original valid tax clearance certificate, the Company registration certificate, BBBEEcertificate are to be submitted as a mandatory requirement without which the bid will be disqualified.

11. Methodology to be applied in the Internal Audit.

12. A statement concerning the independence of the Bidder, including any relationship of theproposed audit team to employees of Midvaal Water Company and /or members of the Board ofDirectors of Midvaal Water Company. If there is any relationship, explain it and elaborate howobjectivity will not be affected.

13. The name or names of external quality control review organization which the Bidder is a memberand the date on which the last quality checks were done.

14. Tenders should be signed by the authorized person. Full name, surname and designation shouldbe written on the tender.

15. Tender number must be included.

The Bidder is required to confirm that it will hold its proposal valid for 60 days from closing date of thesubmission of proposals.

The bidder might be requested to make a presentation with regard to the bid to the Audit and RiskCommittee

The successful Bidder will sign a contract with the Company for one year which will be reviewedyearly.

Performance of the Successful Bidder will be evaluated by the Audit and Risk Committee yearly.

Midvaal Water Company reserves the right not to appoint the lowest Tenderer as well as the right toreject any or all tenders. Reference checks of a successful bidder will be conducted.Successful bidder will be required to sign a service level agreement.

The Company intends to award the tender by the middle of February 2014, after which the successfulTenderer will be notified.

Internal Auditor might be required to perform ad hoc projects as may be required by Management orAudit and Risk Committee. Such projects will be outside the scope of Internal Audit Function, and willbe negotiated as such.
