The DIVISION OF HEMATOLOGY/ONCOLOGY at Vanderbilt University Medical Centeroffers training programs in all areas of hematology and oncology. Training opportunities include afellowship program that provides superior training in all aspects of clinical hematology and oncolo-gy, and research opportunities in molecular biology, cellular biology, genetics, signal transduction,pharmacology and clinical investigation. Faculty in the division are in the forefront of cancer researchand treatment.

PURPOSE

Provide superior training in the disciplines of Hematology and Medical Oncology leading toAmerican Board of Internal Medicine Certification in Medical Oncology and Hematology

Instill trainees with the necessary skills and professional attitude that will lead to a lifetime commitment of excellent patient care and scholarship

Develop productive investigators in clinical and/or laboratory-based research in cancer andhematology

ORGANIZATION

There is a single program administered by the Division of Hematology/Oncology overseen by a stand-ing committee of faculty from the division. Program trainees, with rare exception, are expected toenter into a 3-year period of training for combined board certification. Applicants to theHematology/Oncology Training Program must be eligible for ABIM certification in Internal Medicineby the time they plan to enter fellowship training.

Step1: Preparation

If you're a 2006 graduate and you think finding a job is somethingyou won't need to worry about until next year - you're wrong!

Start now to avoid the pressure that builds as graduation nears.Assemble the basics: A CV, education about the market andscanned documents to expedite the credentialing process.

Keep your CV simpleIf you're just graduating from your first program, education,accomplishments, and honors will be the theme. Include dates(month and year) to make it easy for the reader to follow your CV'stimeline. If there is a time gap explain that in the cover letter.

A 12 point font is sufficient. Bold and underline sparingly tohighlight key points. Allow plenty of space around each section.Make sure it is error free - have a non-medical person read it forunderstanding. Use action words to describe your skills. This is also the time to identify any job fairsin your area. Medical societies frequently offer opportunities tomeet recruiters one-on-one during lunch or half day session.

Scan your credentials Keep diplomas, certificates, licenses, etc. in a file that you useEVERY time you apply for a position or to a third party payer.The credentialing process can take from 3-6 months and serious-ly impact your start date. If you're going to relocate you may,inadvertently, pack needed paperwork. Scanning or filing keydocuments now will save time when you need it.

As a rule, there are two recruitment cycles. Many positions areposted in September and hired by November with a second waveof openings in January and filled by March. It is generallyassumed by recruiters, that the more serious (and therefore moredesirable) candidates start early. Some simple preparation nowwill assure you're part of that group!

Step 2: Ready, Set, Interview!!

Over the next few months you'll probably interview more thanyou will any other time in your life. For you, this is an opportu-nity to land that perfect job. For the interviewer it's a chance todetermine why you should be hired over anyone else.

To be successful you'll need to ooze self confidence (not arro-gance), high energy, initiative, and good interpersonal skills.They'll want to know how you handle conflict, accept responsi-bility and, above all, if you have leadership potential.

Start by identifying your goals. Prepare answers to the standardquestions you know you will get that refer to your training, com-petence and ultimate career path. Know your strengths, weak-nesses, reasons you want this job and something about youremployer. Be prepared to discuss these - rehearse with someoneif necessary. If you're the nervous type remember that, while theyare interviewing you, you are also interviewing them.

Beware the most common pitfalls!Never submit a sloppy or error laden CV Never be lateAvoid the limp handshake. (Cultivate a strong, look-your-interviewer-in-the-eye handshake)Don't be arrogantNEVER speak ill of past employers or make excuses for pastbehavior

If you know of something that negatively impacts your applica-tion, don't wait for it to come up - address it first. For example, ifyou can't commit to a long-term position because of a spouse intraining, suggest that hiring you now will instill loyalty to thepractice and/or hospital in the event you BOTH end up staying inthe area.

Finally, if the interview isn't going well consider: Asking theinterviewer some questions. Clarify that you're on the right trackwith your answer. Smile and, if all else fails, keep in mind that youmay not be the reason the interviewer is having a bad day.

Step 3: Benefit Terminology

If you're like most residents or fellows you haven't given muchthought to the benefits you have. As you hunt for your new job,however, you have the opportunity to "shop" your benefits and seewhat's available. Learning some simple terminology will help.

Malpractice Insurance Terms There are two types: "Occurrence" and "Claims Made". The dif-ferences are important. "Occurrence" coverage is for all malprac-tice cases that arise while you are working in your practice or afteryou leave. "Claims made", on the other hand, only covers youwhile you are working in the practice and you must pay for "tailcoverage" to continue the insurance after you leave.

If you are offered "claims made" insurance, make sure you ask thecost to continue the tail coverage when you leave the practice.This will be an important part of any exit strategy when you lookfor your next position. "Tail coverage" is also referred to as "nosecoverage" when you are joining a practice and have these pay-ments from your previous malpractice coverage.

Health Insurance Terms COBRA: When you leave a job, you and your dependents havethe right to continue health and dental coverage through theConsolidated Omnibus Reconciliation Act (COBRA) for up toeighteen (18) months. Before you leave residency check to seewhat this will cost. If your new employer doesn't provide imme-diate health or dental coverage, you may need to sign up forCOBRA in the interim.

Ask at your new position if there is a "qualifying period". (This isthe length of time you need to be on the job before the benefitstarts.) If there is a lag between beginning practice and coverageyou may be able to maintain coverage through COBRA.

Ask your new employed about "pre-existing conditions". Someinsurances will not cover medical conditions for which you havereceived previous treatment. This term may also apply to a pre-existing medical condition such as a pregnancy. Having this infor-mation can save you from unexpected medical expenses.

Salary Terminology Having salary "at risk" is very common. When you are consideringan opportunity, make sure you understand what the expectations arefor productivity, patient satisfaction, and any other quality or pro-

ductivity measures. Failing to meet these requirements can result ina portion of your salary being reduced ("at risk"). By the sametoken, if you exceed expectations you may receive and incentive.

Contract Terms While not always easy to enforce, many practices include a "non-compete clause" in their contract. Generally this clause is addedto protect the current practice. The clause will prohibit you frompracticing within a certain radius from the current practice (ie: 10miles) for a certain period of time (ie: three years). You can askto have this clause removed from the document, but if it's not, beprepared to comply with the requirement. As with any new endeavor, learning the terminology is critical tosuccess. While this is not a comprehensive list of terms, you willfind that familiarity with these simple phrases will help as younegotiate your first offer. Read information thoroughly and askquestions when in doubt.

Today it's no secret, physicians are in short supply.

According to the Bureau of Labor Statistics (BLS) there will be atotal of 212,000 physician openings by the year 2014 due togrowth and net replacement of retiring physicians. This numberrepresents more than 25% of the current physician work force. Itis important to note, as well, that the BLS statistics do not factorin the other outside influences, such as the tightening restrictionsaround immigration which may impact the potential supply ofInternational Medical Graduates who account for more than 22%of the physician labor force, or the increasing subspecialization inphysician training which delays "time to market" year over yearfor graduating residents.

In traditional economic theory, applying the law of supply anddemand, the shortfall in physician supply would bode well fornewly graduating Residents. Entering the workforce you wouldanticipate the stronger the need (tighter the supply) the higher theprice (salary) which may be offered. Unfortunately, the health caremarket is an anomaly, where the rising costs to provide care are notbalanced by increased reimbursement. And medical practices, hos-pitals and health systems alike are challenged to find new ways towork smarter and deal with the bottom-line while providing ade-quate access to and quality of care for the communities they serve.

So why is this important to you as you consider your employmentoptions? Primarily, because understanding the broader issues yourpotential employer faces, market to market, will enable you to bestnegotiate a contractual agreement which will ensure you maintaina professional/personal life balance, while attaining the greatestpotential financial earnings.

Important Things to Consider When EvaluatingPotential EmploymentWhat is your primary goal in taking this position? The majority ofemployers with whom you speak will want your goal to be longterm. Though this may not always be realistic based on your cur-rent personal situation. For example, if you have a partner who isfinishing training, and upon completion you are fairly certain youwill be moving out of the area. This presents a short term goal foryou to discuss - and there is nothing wrong with this. Be upfrontwith the employer, while staying may become an option, right nowmoving on in a year to a different geography is your focus. This doesnot mean you will not work hard to build a practice or fulfill clini-cal duties. Positioning it upfront, however, enables the employer toplan accordingly, continuing to search for a long term hire whileyou are in practice. Be forewarned though, this type of arrangementmay not be suitable for all practices, and the fairest option may beto explore a hospital or academically based position which is moreapt to look favorably upon your needs.

What is the goal of the practice in hiring you? Is the practice inexpansion-mode, needing a new associate to meet increasingpatient demands? Or are you replacing a physician, and if so why?Has there been significant turnover at the practice? Is there aplanned retirement? And if a retirement is planned what is thetime to and plan for transition, and how will this impact thegrowth of your patient panel? Has the practice or affiliated hospi-tal conducted a physician-community needs assessment recently?These types of studies are regularly performed now as the FederalGovernment seeks to monitor the level of relationship betweenhospitals and ensure proper access of care to patients. Should thepractice and hospital be aligned in the recruitment, it will beimportant to understand the relationship of the practice to thehospital and the overall growth strategy of the hospitals as itrelates to physicians. In these latter situation, where a hospital isplaying a significant role in supporting community or even hospi-tal owned practices, you want to be certain that the hiring goals orfocus will not saturate the market making it difficult to build-up apractice. Ideally, you should get a sense of everyone who is hiringin the market (in you area of specialty) to truly determine anyshort-term impacts on your success.

What is the payor mix, reimbursement rate and percent of collec-tions for the practice? More important than "salary", which formany private practices and a growing number of employed hospi-tal positions is simply offered as a first year safety net, these threefactors will determine your earning potential well into the future- even at partnership levels. Evaluating these areas now will helpyou identifying some key features - is the practice currently runefficiently? Is the support staff appropriate for the number ofphysicians in the practice? How many patients will you need to seeto produce target revenue levels? What is the current overhead ofthe practice? While overhead is typically assessed as a percentageof revenue, it is simpler to understand it as a whole dollar figure,rolling all cost elements into the assessment and subtracting themfrom collected revenue to get the "real" picture on profit. Requestthis information for at least a three month period of time - it willbe most accurate if the snapshot does not included the summermonths or typical cold and flu season.

Approaching your job search by evaluating the broader factorswhich determine the long-term viability of the employment oppor-tunity is best for both you and the practice. Let's face it, inherent-ly you know the location where you want to be - it usually hassome level of tie back to you through family, a personal or profes-sional connection. We can also agree that anybody seeking a newposition wants the highest offer possible. The reality is, however,while the landscape of physician "employment" is continuallychanging, the fundamental basis for clinical opportunities into theforeseeable future will be production oriented. Basing decisions onsimply an initial salary offer is a short-sighted strategy which mayleave you with fewer options than reasonably expected.

A good rule of thumb, narrow down your top two locations bygeography, interview three to four times in both areas to get a truesense of the market. Evaluate the pros and cons, assess the longrange viability of the practice . Then make the decision that willbe best for you over the next five or more years.

Introduction Selecting the right practice opportunity can be over-whelming whether you are just coming out of residency or mov-ing from an established practice. By following these steps, youcan make the process a little more manageable.

Pre-Visit Before you begin contacting other practices or head-hunters, it is important for you to decide what you want. Thatmay sound obvious, but many physicians have never taken thetime to decide where (geographically) they would most like to live,which may be the most important question to start with.

After location, focus your attention on things such as group size.Are you interested in a small or large group? This will have impli-cations for your call schedule. How often would you be comfort-able being on call?

You will also want to think about whether you want a hospitalpractice or whether you would prefer to turn patients over to aHospitalist group.

How about nursing homes? Do you feel comfortable coveringnursing homes for other members of your practice? Or wouldyou rather not do this? Be honest!

Finally, consider your productivity. How busy would you like to be?How many patients per day do you feel comfortable seeing? “Busy”means different things to different people. Think through all of thesethings prior to interviewing with a group.

The Visit Pay attention to and look for group chemistry as one of the mostimportant factors in selecting a practice. Remember that pickinga physician partner is not like a marriage, it is a marriage. Yourphysician partner or partners will help determine your financialand professional future. You will also be spending a large amountof your waking hours with your physician partners.Pay attention to the general environment. What is the generalatmosphere of the practice? What is the group’s style and effi-ciency of handling patients? Does it appear to be well managed?Learn about the structure of the group from a legal, financialand decision-making standpoint.Ask questions about the group’s expectations of the newphysician. How busy does the group expect you to be, andhow quickly? How does the group plan to promote the newphysician to build the practice?

After all of the above questions are answered to your satisfaction,you can begin to investigate the compensation package includingsalary, bonus, benefits and other perks of the job. Ideally, groupswill be able to answer these questions, but don’t be surprised ifthey have a hard time getting specific with their answers. Manygroups have not added a new practitioner for a while and have notthought through the details of their particular offering.

Additional Thoughts on the Interview The following tips will helpmake your visit more successful:

Pay attention to your appearance. Many groups get immedi-ately turned off by a great clinician simply because that clini-cian is dressed unprofessionally or too casually. Rememberthat you are being interviewed for a six-figure job, and yourpotential employer wants to make sure that you will upholdtheir reputation. Be sure to be on time.Rehearse your talking points so you can cover your philosophyof care, give some background on your clinical training andyour work ethic.

As you wind up the visit, get clarity on the decision-makingprocess. Find out who will be making the decision and in whattime frame. Determine who your main point of contact will beand let them know what your decision-making process is.

Settle on the general business terms before going to the time andexpense of hiring an attorney. Once the general business termshave been agreed to, you should expect a formal, legal contractthat you can review with your attorney. Don’t make the mistakeof trying to save on legal fees by doing this yourself. For a fewhundred dollars up front, with good legal advice, you can avoidmajor complications and headaches down the road.
