Manypeopleexpressthewishtobecaredforinthefamiliarityandcomfortoftheirownhome.PlanningthiscarewiththeProvincialIntegratedPalliativeCareTeamcanhelppatients,familiesandcaregiversgetthesupporttheyneedtobecomfortableathome.Avoidinganunplannedtransfertohospitalduringpalliativecarewillbepartofthesupportprovidedtoassistpatientswhowanttoremainathome.
Thisservicealsoprovidesanadditionalsupporttothosepalliativepatientswhowouldprefertoreceivecareathome.

The Paramedics Providing Palliative Care at Home project is a joint initiative between Health PEI, Island EMS, Nova Scotia Emergency Health Services (EHS), Dalhousie University (Division of EMS) and Cancer Care Nova Scotia with financial support from the Canadian Partnership Against Cancer.

Paramedics Providing After HoursCare at HomeShouldapalliativepatientexperienceanunexpectedeventwhileathomeduringafter hours*,in-homesupportisavailablethroughIslandEMS.Bycalling9-1-1,trainedparamedicswillhelpprovidecareandsupporttoyou/yourlovedoneathome.
Aspartofthisservice,IslandEMSparamedicsaretrainedinpalliativepainandsymptommanagement.Thisensuresthataqualifiedemergencyresponseteamisabletosupportandmanageunexpectedpalliativeevents,soyou/yourlovedonedoesnotneedtogotothehospitalifthepreferenceistostayhome.
Thereisnofeeforthisin-homeservicetopatientsregisteredintheProvincialIntegratedPalliativeCareProgramwhohaveavalidPEIhealthcard.
Patients Registered in the Provincial Integrated Palliative Care ProgramOnceyouareregisteredwiththeProvincialIntegratedPalliativeCareProgram,ahealthcareteamwillworkwithyoutodevelopacareplan.Shouldaparamedicbecalledtoyourhomeafter hours*theywillalsofollowthesamecareplanwhilehelpingyoutoremainathome.Ifyouarenotregisteredintheprogramandwouldlikemoreinformation,pleasephone(902)620-3045orvisitwww.healthpei.ca/palliativecare.
*After hours is the time when your Home Care Nurse is not regularly available. Please consult with your Home Care Nursing team.
How the service works...IfyouareregisteredintheProvincialIntegratedPalliativeCareProgramandyou/yourlovedoneneedsafter hours*care:
• Call9-1-1**
• Tellthemyou/yourlovedoneisregisteredwiththeProvincialIntegratedPalliativeCareProgramandprovidethepatient’slastnameandregistrationnumber.
• Theambulancewillarriveatyourhomewithnolightsorsirens.
• Theparamedicswhoarriveatyourhomewillworkwiththecareplandevelopedbyyouandyourhealthcareteamtoassistyouinremainingathome.
• Yourcareplanmay,forexample,instructtheparamedicstogiveadditionalpainmedicationorinitiatetherapytoassistyoutobemorecomfortableinthehome.
• Theoptionwillbeavailabletotakeyoutotheemergencydepartmentifthatiswhatyouwanttodo.
• Insomecases,theparamedicsmaytellyouthatitwouldbebesttotakeyoutotheemergencydepartment.
• TheparamedicswillworkwithyourhealthcareteamtoensurethatyourHomeCareNurseisawareoftheevent.
** 9-1-1 is available at all times for anyone in an emergency situation. For patients registered in the Provincial IntegratedPalliative Care Program it is recommended that you contact your Home Care Nurse if help is needed to manage pain and symptoms during regular Home Care hours.
