Objectives• Identify various types of EKG Monitors
– 3 Lead– 12 Lead
• Describe various EKG functions– Monitoring– Defibrillation– Transcutaneous Pacing– Cardioversion
• Describe the use of Pulse Oximetry in the pre-hospital environment
• Describe the use of Capnography in the pre-hospital environment

ECG/EKG Monitors
Foothill College EMTP 60BFall 2012
• Electrocardiography (elektrokardiography)a trans-thoracic interpretation of the electrical
activity of the heart over a period of time.
• EKG monitor A computer designed to recognize, record and
display electrical activity within the heart.
• Electrocardiogram (elektrokardiogramm)The waveform produced and displayed by an EKG
monitor

Monitoring Terms• ElectrodeElectrode
Electrical detection device attached to the skin “the sticker”
• Monitor Cables wires that connect electrode to monitor
• Lead A tracing of the voltage difference between two electrodes.

3 Lead Vs. 12 Lead• 3 Lead EKG
– Created through the combination of 3 (limb) electrodes• Right arm (White)• Left arm (Black)• Left Leg (Red)
– Offers 3 views (leads) of the heart• Lead I• Lead II• Lead III

3 Lead Vs. 12 Lead• 12 Lead EKG
– Combines the 3 limb electrodes with 6 chest electrodes
– Creates 12 views (leads) of the heart• 3 limb leads• 6 chest leads• 3 “augmented” leads

Interventional Functions• Transcutaneous (External) Pacing
Providing an electrical signal (stimulus) to the heart when it’s own built in (natural) pacemaker or conduction system has failed.Goal: Control the heart rate and/or rhythm
• Synchronized CardioversionDelivering a selected dose of electricity to the heart at a specific period in the cardiac cycle.Goal: To reset the hearts intrinsic firing rate.
• DefibrillationDelivering a selected dose of electricity to the heart at any period in the cardiac cycle.
Goal: To terminate disorganized electrical activityFoothill College

Pulse OximetryWhat it is..
A non-invasive method of monitoring a patients oxygen saturation.
What is does..• Pulse Ox probe measures
amount of light absorbed by oxygen saturated blood hemoglobin
Foothill College EMTP 60BFall 2012
![Page 11: Cardic Monitoring Devices](https://reader030.fdocuments.in/reader030/viewer/2022013105/55a756bd1a28abb15d8b480c/html5/thumbnails/11.jpg)
Pulse Oximetry• Clinical significance
– Provides a clinical picture of a patients respiratory status.
– Especially useful for patients in respiratory distress.“Is the patient getting
oxygen into the blood?

CapnographyWhat it is..
A non-invasive method of monitoring a patients ventilatory status, perfusion and metabolism.
What is does..• Sampling devices measure
the amount of expired CO2 at the end of each respiratory cycle
End Tidal CO2
Normal Range 35-45 mm/Hg

Capnography
• Clinical Significance– Mainstream
• Verification of ET tube placement
• Viability• ROSC
– Side stream• Narcotic Overdose• DKA• Head Injuries• Chronic Respiratory Conditions
