Refer a client to Lifemark’s Cardiac Rehabilitation Program. Here’s when and how to make a referral.
What is cardiac rehabilitation?An interdisciplinary and comprehensive care plan that address a clients’ cardiac, exercise, orthopaedic and psychosocial concerns, raises their confidence and helps them get back to life by providing them with the knowledge and tools they need to support safe and sustainable participation in functional life activities.
Indications for a referral• Medically stable following a myocardial infarction
(heart attack)• Stable angina• Coronary bypass graft surgery• Stable heart failure• Heart transplant• At risk of Coronary Artery Disease with co-existing
conditions or diagnoses of diabetes, dyslipidemia, hypertension or obesity
Contraindications for a referral• Unstable angina• Uncontrolled hypertension
• Over 180 mmHg resting systolic or resting diastolic over 110 mmHg
• Uncontrolled atrial or ventricular arrhythmias• Uncontrolled tachycardia >120 beats/min• Uncompensated heart failure
• Worsening of heart failure symptoms, including dyspnea, leg or feet swelling and fatigue
• Active pericarditis or myocarditis• Acute systemic illness or fever• Uncontrolled diabetes mellitus• A severe orthopaedic condition that would
affect participation in an exercise program
