This schedule of Rule s and Regulations is a part of all contracts for receiving electric service from the Cooperative and applies to all service received from the Cooperative whether the service received is based upon a contract, agreement, signed application, or otherwise. No employee or individual director of the Cooperative is permitted to make an exception to rates or Rules and Regulations. Rates and service information can be obtained from the Cooperative's office.

REVISIONS

These Rules and Regulations ma y be revised, amended, supplemented or otherwise changed from time to time without notice. Such changes when effective shall have the same force as the present Rules and Regulations. 5. RELOCATION OF LINES BY REQUEST OF MEMBERS

The Cooperative's established lines will not be relocated unless the expense for moving and relocating is paid by the member, except in instances where it would be to the advantage of the Cooperative to make such relocation.

The Cooperative's personnel is prohibited from making repairs or performing services to the members equipment or property except in cases of emergency or to protect the public or member's person or property. When such emergency services are performed, the member shall be charged for such service at the rate of time and material.

B. SERVICE PROCEDURES

APPLICATION FOR SERVICE

Each prospective member desiring electric service will be required to sign the Cooperative's form, "Application for Membership and for Electric Service," before service is supplied by the Coope rative and provide the Cooperative with .necessary easements or right-of-way permits.

MEMBERSHIP FEE

The membership fee in the Corporation shall be $5.00 (Five Dollars.) One member­ship must be held in connection with each person receiving service. The member­ship fee will be refunded if all bills are paid or applied against any unpaid bills of the member at the time service is discontinued which will be automati­cally terminate the membership. Service covered by each membership shall be metered and billed separa tely. An additional $10.00 required for non-propert y owners deposit. ~

Any member desiring service discontinued or changed from one location to another shall give the Cooperative three (3) days 1 notice in person or in writing pro­vided such notice does not violate contractua l obligations.

RECONNECTION CHARGE

The Cooperative will make no charge for connecting service to the member's premises for the initial connection of service. When service has been terminated and the Cooperative is requested to reconnect service to the same member at the same location a $5.00 (five dollar) reconnection fee will be charged. Reconnect charge will be due and payable at the Cooper­ative's office upon notice of said charge prior to connection. No reconnect­ion shall be made after the regular working hour s unless in the judgment of the·management there exists circumstances that will justify the addition­al expense.

RESALE OF POWER BY MEMBERS

All purchased electric service used on the premises of the member s h a ll be supplied exclusively by the Coopera~ive, and the member shall not directly or indirect ly sell, sublet, or otherwise dispose of the electric servic e or any part thereof.

C. METERS

METER TESTS

All new meters shall be checked for accuracy before installation. The Cooper­ative will, at its own expense, make periodical test and inspections of its meters in order to maintain a high standard of accuracy and to conform with the regulations of the Public Service Commission. The Cooperative will ma ke additional test of meters at the request of the member and upon payment of a fee of $5.00 (Five Dollars) paid in advance. When the test made at the member's request shows that the meter is accurate within 2% slow or fast, no adjustment will be made to the members bill and the fee paid will be forfeited to cover cost of requested test. When the test shows the meter to be in excess of 2% fast an adjus tment shall be made to the member's bill by recalculating the monthly bills for a period equal to one-half (~) of the time lapsed since the last previous test, but in no case shall this period exceed three (3) months and th~ cost of testing the meter found to be inaccurate will be borne by the Cooperative and the $5.00 (Five Dollars) fee paid by the member will be refunded.

FAILURE OF METER TO REGISTER

In the event a member's meter should fail to register, the member shall be billed from the date of such failure at the average consumption of the member based on the three (3) month period immediately preceding the failure.

RULES AND REGULATIONS

BILLING

Members will receive statements for electric service on the 1st day of each month for service rendered for a thirty-day period ending the 1st day of the preceding month. All statements are due and payable upon receipt and shall be paid at the office of the Cooperative within ten (10) days from date of bill. Failure to receive electrical statement will not release the member from pay­ment obligation. Should the statement not be paid as above the cooperative may at any time there-after on a five (5) days• notice to the member discontinue service. Should it become necessary for the cooperative's representative to call at the Consumer's premises or other locations for the purpose of collecting a delinquent account, a charge of $5.00 (Five Dollars) will be made to the member's account for the extra service rendered, due and payable at the time such de­linquent account is collected. If service is disconnected for non-pa yment, an additional charge of $5.00 (Five Dollars) will be made for reconnecting service due and payable at time of such reconnection.

METER READING

Each member receiving service will be required to supply the Cooperative with reading of each meter on the form as furnished by the Cooperative on such date designated by the Cooperative. Upon failure by the member to supply the Cooperative with such meter reading for three (3) consecutive meter reading dates the cooperative will act by having it s representative read the member's meter a service charge of $5.00 (Five Dollars) will be made for the extra service render­ed. The service charge will be made to the member's account and will be due and payable upon notice of said charge. Classification is a means for treating without discrimination all members having similar characteristics in their u s e of service.

A commercial consumer will be any business esta blishment such as a store, office, garage, tavern, creamery, factory, hotel, touri s t cabin, mill, quarry or institu­tion other than schools, churche s or community h a lls. Cons umers qualifying under this classification shall be billed under Rate Schedule 2 or LP.

RESIDENTIAL AND COMMERCIAL ESTABLISHMENT OCCUPYING SAME PREMISES.DESIRING SERVICE THROUGH A SINGLE METER.

A member operating a commercial esta blishment on the same premises as his residence may be served through a single meter and shall be classified as a commercial consumer. If the consumer perfers, his bus iness establishment be served under the commercial rate and his residence served under the farm and home rate, two (2) meters will be required.

26. SERVICE TO SCHOOLS, CHURCHES AND COMMUNITY HALLS

Schools, churches and community halls and other small non-profit community welfare institutions shall qualify under this classification and be billed under Rate Schedule 1. The special rates available to these small institut­ions shall be limited to KVA of transformer c apacity. Larger institutions shal l be served under the commercial or large power schedule and · will be classified as commercial u nder r ate schedule 2 or LP.

added to the bill. TERMS : In those cases where additional invest­ment is required for three-phase service, it will be rendered only after the following conditions are met:

A. The customer will give satisfactory assurance by means of a written agreement as to the character, amount, and duration of the business offered. '

B. The customer will guarantee a m1n1mum monthly bill for the three-phase service which will be computed on the basis of 1 ~ per­cent of the additional investment, which includes the additional cost of transformers necessary to make three-phase service availabl This minimum bill will be effective for a period of three (3) years from the date on which service commences. After this period the regular minimum monthly charge will be effective.

C. In no case, however, will the minimum bill for three-phase service be less than the previously specified "minimum monthly charge."

SPECIAL RULES:

A. Single-phase customers having their homes on the same premises may include farm and home service on the same meter. B. Service under this schedule includes lighting, incidental• appliances, cooking, heating and motors. C. Service under this schedule is limited to customers whose load requirements can be met by a transformer having capacity exceed 25 KVA. D. Service under this schedule is not available to sea~vud-~ customers unless otherwise provided for. E. Motors having a rated capacity in excess of t~n hors~~~~~-m1 be three-phase.

This rate schedule is available, for street lighting serviceF ! shown herein, in any community in which the coopera tive operates. Service is subject to the provisions herein and the provisions of the Cooperative's standard contract. Should the service not meet the standard provisions, then the Cooperative reserves the right to revise the charges listed hereinafter so as to include any unusual cost involved.

CONDITIONS OF SERVICE: 1. All night service shall be from dusk to dawn. . j 2. Sta~dard Overhead System: Street lighting equipment furnish-, ed under the standard overhead rate shall cons ist of wood poles, brackets, appropriate fixtures for the lamps being used, the necessary overhead street lighting, protective equipment, control and transformers. The corporation will install, own, operate and maintain the entire street lighting service, including the stand­ard overhead rate. The Cooperative will, upon request furnish under the ornamental service rate, monolubar aluminum poles of it 1 s own choosing, together with overhead wiring and all other equipment and provisions set out above. The customer will pay the Ornamental service rate. 3. Other than Standard Overhead System: Should the customer re-

A. Since mobi'le homes and trailers are readily and easily relocable, they must be classified as non-permanent electric service. A charge for providing service, therefore, is to be made as follows:

If the applicant installs sanitary sewage facilities which meet the requirements of the State Health Department, certified by the County Sanitarian within one year after service connection, then the Cooperative will credit or refund the amount charged for a maximum of two primary poles, or the service charge.

II. Service to Mobile Home Parks

Cooperative will extend primary service to an established mobile home park meeting Kentucky Board of Health requirements if in reasonable proximity of existing primary facilities. Secondary service may be provided under one of the following options:

A. If park operator desires one metering point, all facilities beyond this shall be furnished by the owner.

B. If operator desires individually metered service at each mobile home, the Cooperative will install secondary facilities to the meter pole.
