St. Josephs Rd Black Canyon City AZ 85324 December 12, 2012 PARTIES AFFECTED BY THE APPLICATION The subject parcel and proposed use is located atop a ridge at the Our Lady of Solitude Catholic Church Retreat, with surrounding uses as residences, horse stables, storage yards and vacant parcels. The proposed use will not have a negative impact on those in the area as the proposed use is currently in place. The requested Use Permit is for a site modification of the existing 101' tall wireless communications facility lattice tower. AT&T plans to collocate on the extended tower at the 110' level. AT&T plans an expanded ground space area for additional AT&T equipment cabinets. NOTIFICATION AND INFORMATION PROCEDURES The surrounding property owners will be notified in writing via 1 st class U.S. Mail with a notice of the pending or submitted Use Permit Application. The County requirement of public notice to the real property owners within 2,500' of the subject parcel will be adhered to. The 2,500' notification list will be prOVided by Yavapai County. A copy of the map showing the surrounding property owners and list will be included in the follow up case submittals to the County. RESPONSE PROCEEDURES A neighborhood meeting will be held in the event that there is community concern that cannot be otherwise resolved; or if deemed necessary by County Staff. The neighborhood meeting would be held at the subject parcel. The notified property owners may contact the Applicant via email, telephone, fax or by sending a letter with their questions, comments or concerns. The contact information will be set forth on the neighbor notification letter. A report of such activity, if any, will be submitted to the Planning Department prior to the Planning Commission hearing agenda. SCHEDULE FOR COMPLETION A summary of notifications and the public response will be compiled and delivered to the Planning Department prior to the Planning Commission nearing.
