Project: 0081-011-727, P101, N501 From: MM 168.25

To: MM 168.00 UPC: 93462 County Botetourt Length: 0.25 Miles

The roadway is classified as a Rural Interstate with a posted speed limit of 70 mph. The grade is approximately 2.5% throughout the project limits. The AADT in 2011 (SB only) is 18,110 with 32% trucks. The existing typical section consists of a four-lane divided roadway with two twelve foot lanes in each direction with a graded median. The outside shoulder width is approximately 15 feet with 10 feet stabilized (paved). The inside shoulder is approximately seven (7) feet with four (4) foot stabilized (paved) shoulders found through the project limits. This information was taken from the original construction plans generated for this section of roadway and field observations. The superelevation of the existing roadway will be corrected within the project limits using flexible pavement planing and overlay on the outside lane and complete pavement/subgrade removal and replacement of the inside lane to meet current transition guidelines. Combination underdrains will placed in the inside lane to correct an apparent subsurface drainage problem. The outside shoulder will be improved for maintenance of traffic purposes during construction. In addition, the inside edge of the bridge deck and damaged concrete railing will be replaced. During construction traffic will be maintained at all times throughout the project limits although motorists may expect delays as a lane closures may be required for extended periods of time to reconstruct the inside southbound lane. Strict erosion and sediment measures shall be enforced throughout the life of the project. The project will be constructed within the existing right of way. There is no bicycle or pedestrian accommodations intended or proposed with this project.

TRANSPORTATION MANAGEMENT PLAN

INTRODUCTION

Improvements will be performed in Botetourt County. The roadway is classified as a Rural Interstate with a posted speed limit of 70 mph. The grade is approximately 2.5% throughout the project limits. The AADT in 2011 (SB only) is 18,110 with 32% trucks. The superelevation of the existing roadway will be corrected within the project limits using flexible pavement planing and overlay on the outside lane and complete pavement/subgrade removal and replacement of the inside lane to meet current transition guidelines. Combination underdrains will placed in the inside lane to correct an apparent subsurface drainage problem. The outside shoulder will be improved for maintenance of traffic purposes during construction. The bridge over Purgatory Creek (Structure No.1021) within the project limits is included in the project. The bridge will require partial deck repair in order to properly replace the existing damaged median side concrete railing with an approved parapet wall. The bridge deck, which carries traffic throughout construction, will need repair requiring type B and C patching.

will be affected at times, during milling, bridge repair, guardrail adjustments, and resurfacing/marking operations. During construction, the Speed Limit will be reduced to 55 mph.

Temporary Traffic Control Reference:

The work zone shall be maintained according to the Maintenance of Traffic” document in the contract. Refer to the following traffic control typical applications from the “Virginia Work Area Protection Manual, dated May 2005.

Single lane closures will be allowed Sunday thru Thursday from 7:00 pm in the evening to 6:00 am the following

morning. In Phases 2, 3, and 4 traffic will run on the outside lane and outside shoulder providing continuous work behind the barrier wall.

In Phase 5 traffic will run continuously in the left lane for 4 days.

Entrances and Intersections Affected:

The on-ramp at exit 168 (Arcadia) and the off-ramp at exit 167 (Buchannan) will be affected during lane closure operations.

PUBLIC COMMUNICATIONS PLAN

During construction, south bound traffic on I-81 will have only one travel lane available during certain work activities. Goals:

Interstate 81 carries large volumes of traffic daily. Salem District Public Affairs will issue a traffic alert and post online in advance of the work. Also,

notice of the work will be included in the weekly Roadwatch report that is issued each Friday and posted

TRANSPORTATION OPERATIONS PLAN

This plan is not required on this project. However, a contact list of local emergency response agencies must be kept and maintained throughout the project life cycle. Traffic Backup Notification The Traffic Operations Center (540-375-0170) shall be notified of traffic backups related to the work by the project staff. The TOC will make entries into systems that feed this information into 511. The TOC will also make notifications to department staff. The staff notifications shall include but not be limited to the Area Construction Engineer, Project Maintenance of Traffic Coordinator, Project Manager, Resident Administrator, District Work Zone Safety Coordinator, Regional Traffic Engineer, Regional Traffic Operations manager, Regional Incident Management Coordinator, and Public Affairs Manager of any incidents and expected traffic delays. Emergency contact for this project shall be 911. A review of all major incidents, as determined by the Regional Incident Management Coordinator, shall be accomplished within 72 hours of clearance of the incident. SEQUENCE OF CONSTRUCTION PHASE 1: Prepare outside shoulder for traffic Sta. 103+00 to Sta. 115+00 (Excluding the bridge)

A. Traffic in inside, (left) lane, behind barrels. The Arcadia ramp (Exit 168) will be channeled into the left lane.

1) The outside shoulder will be trenched using flexible pavement planing 9.5 inches and replaced with 8 inches of base and 1.5 inches of surface.

2) The cross-slope of the shoulder shall be raised to match the outside lane. 3) Beyond the limits of milling and paving for maintenance of traffic purposes,

the shoulder shall also be reconstructed for traffic, 200 feet to the south and 1260 feet to the north of the project limits. See plans and typical sections for additional information.

B. Guardrail will be removed and reset as part of phase 1.

1) Reset, each night, what is removed each night, so as not to create a hazard for daytime traffic. See Guardrail General Notes for additional information.

2) If possible, the GR-FOA terminals should be adjusted not replaced in this phase.

3) Beyond the limits of milling and paving for maintenance of traffic purposes, guardrail will be removed and reset 200 feet to the south and 600 feet to the north of the project limits.

NOTE: For resetting Existing Guardrail: If possible reset the existing guardrail so as to provide a consistent full width travel lane on the shoulder. NOTE: Prior to Phase 2, 3, and 4: The striping alignment for the on-ramp and gore area of the on-ramp southbound at Exit 168 will be adjusted and shifted to accommodate an acceleration lane which will tie into a shifting taper that will be moving traffic onto the right shoulder and the right lane allowing for a long term stationary left lane closure. PHASE 2: Repair bridge overhang and wall

A. With traffic still in the left lane apply temporary pavement markings to the right lane and shoulder for traffic.

B. Using night time operations: shift traffic to right shoulder behind barrels. 1) Set barrier wall MB-11A on the bridge and upstream of the bridge using a

taper of 22:1 to seal the inside lane. 2) Mark the left edge of the travel lane 6 inches off the barrier wall. 3) The traffic side of the barrier wall will be attached to the roadway and to the

bridge.

C. Shift traffic to outside lane and outside shoulder. D. Make repairs to the bridge.

C. Repave travel lane and shoulder to intermediate course. D. Attach a new GR-FOA-2 to the new, safety shape, outer wall and reset the guardrail north

of the bridge and tie to existing guardrail.

PHASE 4: Perform “Deep Milling” operation south of the bridge Sta. 103+50 to 107+42 (Plan sheets 1 and 2)

A. Close the Buchannan Exit: Exit 167. Install 2-Type III Barricades with RAMP CLOSED signs on the barricade.

B. Add barrier wall to the south of the bridge to cover the milling operation. Extend

markings to cover the length of barrier wall extension.

C. Remove existing guardrail in the deep mill area.

D. Accomplish deep milling (south of bridge approach slab) 1) Trench 20 inches for both travel lane and shoulder.

E. Repave to intermediate course. F. Attach a new GR-FOA-2 to the new, safety shape, outer wall and reset the guardrail

south of the bridge and tie to existing guardrail. PHASE 5: Mill outside lane and shoulder

A. Using night time operations: shift traffic to right shoulder behind barrels. 1) Set barrier wall MB-11A for the length of the project (103+50 to 114+50).

Provide a taper of 22:1 to seal the outside lane. 2) Mark the right edge of the travel lane 6 inches off the barrier wall. 3) The traffic side of the barrier wall will be attached to the roadway and to

the bridge.

P-2 The pavement materials on this project will be paid for on a tonnage basis. The weight will vary in accordance with the specific gravity of the aggregates and the asphaltic content of the mix actually used to secure the design depth. TRAFFIC CONTROL

1. See Sequence of Construction notes for additional information.

2. The Contractor shall be responsible for all traffic control. All traffic control devices shall conform to the 2005 Virginia Work Area Protection Manual.

3. All work performed under this contract shall be coordinated with any other contract

and/or state forces work being done in the area.

4. The Contractor’s equipment and stored materials shall be stored outside of the Design Clear Zone and deflection area behind physical barriers.

5. The Contractor shall remove any lane closure upon any queue longer than 2 miles

from the lane restriction provided all hazards are reasonably protected and safe travel is possible. After traffic flow has returned to normal flow with no delays, the Contractor may resume operations and install lane closures during the allowable hour herein. The Engineer or designee will notify the Contractor when queue length is greater than 2 miles and deemed necessary to remove the lane closure in his sole discretion. Any interruptions to the Contractor due to the release of queues longer than 2 miles will not be cause for additional compensation or schedule delays.

7. Shoulder Closures will also be allowed during the following daytime hours: Monday through Friday 9:00 AM to 3:00 PM

GUARDRAIL

1. Guardrail materials removed during this contract shall become the property of the Contractor and shall be disposed of at a licensed landfill, recycled, or be retained by the Contractor.

2. The contractor shall perform all work such that there are no fixed objects or blunt

ends exposed to the traveling public. If an approved NCHRP 350 terminal cannot be installed at the conclusion of a workday, then a Truck Mounted Attenuator (TMA) shall be placed in front of the fixed object or blunt end. If the shoulder is insufficient to accommodate a TMA, Group II channelizing devices shall be used to delineate the fixed object or blunt end as directed by the Engineer.

3. All new guardrail installed under this contract shall be protected with an approved

crashworthy terminal before the end of each workday.

4. Pavement markings damaged by the Contractor during shoulder or guardrail operations shall be replaced by the Contractor at their cost.

5. All stored material and equipment shall be kept out of clear zone and deflection

area behind guardrail at the discretion of the Engineer.

6. The Contractor shall submit shop drawings for each terminal used & an FHWA NCHRP 350 approved letter.

7. Cracks in the shoulder as a result of driving posts shall be repaired. In soil or

aggregate stabilized shoulders, cracks and voids around the posts shall be filled with like material and thoroughly compacted. In asphalt paved or surface treated shoulders, cracks and voids around the post shall be filled, compacted and sealed with fine asphalt plant mix no larger than SM -9.5A, or filled with fine aggregate No. 8 or 9, sealed with asphalt emulsion CRS-2 or CRS-1 at the rate of approximately 0.35 gal. Per sq. yd. All cost for this work shall be included with the price of guardrail.

8. The Contractor shall backfill all unused post holes. Guardrail post holes shall be backfilled to existing ground level with approved material placed in layers not more than 4 inches in height. Each layer shall be compacted by tamping.

GENERAL NOTES FOR SEQUENCE OF CONSTRUCTION 1. Unless otherwise approved or directed by the Engineer, the Contractor shall Plan and prosecute the work in accordance with the following: 2. The phases in this suggested sequence of construction shall be followed unless the Contractor submits and secures the approval of the Engineer for a sequence which will both expedite construction while lessening the effect of such construction upon the traveling public. The intent of the contract and suggested sequence of construction is that portions of the roadways in the different phases may be allowed to be constructed provided they do not interfere with existing traffic flow and/or do not cause any delay in the project completion. 6. Construction and permanent pavement marking shall be installed in accordance with manufacturers recommended practices. 7. All areas excavated below existing pavement surfaces, at the conclusion of each work day, shall be thoroughly compacted to form a 6:1 wedge against the existing pavement surface for protection of vehicular traffic. All costs for placing, maintaining, and removing the 6:1 wedge shall be included in the cost of other items and no additional compensation will be allowed.

8. All barrier wall used on this project will be traffic Barrier Service Concrete Parapet (Double Face) MB-11A. The barrier slope transition ratio will be 22:1 based on the 70 mph posted speed limit. At the break in the barrier wall a Type B warning light will be used. (See TTC-16.0 Pages 6H-36 and 6H-37).

FLASHER INSTALLATION NOTE:

1. Two FB-2 Flashing Beacon structures each equipped with one W1-4 Reverse Curve – Oversized sign, and one W13-1 Advisory Speed (60 MPH) – Oversized sign (36”x36”) shall be installed for the SB lanes of Route I-81 and will be located 321’ north of mile marker 168.2. One FB-2 structure shall be installed on the right shoulder of I-81 which will be behind existing guardrail, and one FB-2 structure will be installed in the median of I-81 directly across from the one on the shoulder. FB-2 structures are break-a-way.

2. The electrical service supply by Dominion Power will originate from Route 611

(Greyledge Dr.) that tees into Route F54 (Frontage Rd.) which runs parallel to I-81 and is approximately located at mile marker 168.3 on I-81 (to the west of I-81). A 40’ class 4 wood pole shall be installed on the right shoulder of Route 611- 215’ from F054; one SE-4 Type A Electrical Service, and a Flasher cabinet shall be installed on the 40’ wood pole; a JB-S2 Junction Box shall be installed at the base of the wood pole; a 2” conduit will be installed from the junction box by ECI-1 Trench Excavation approximately 215’ to Route F054 to a junction box where it will turn south on F054 and run approximately 571’ along the right shoulder to a JB-S2 junction box. A Bored 3” Conduit will be installed under Route F054 and under the limited access fence to the right shoulder of I-81 behind the existing guardrail where the FB-2 shoulder structure is located. From that FB-2 structure/JB-S2 Junction Box; another Bored 3” conduit will be installed under the SB lanes of I-81 to a JB-S2 Junction Box in the median and then to the FB-2 structure in the median.

10. A description and schedule of procedures to maintain vegetation, erosion and

sediment control measures and other protective measures in good and effective operating conditions during construction are identified in Sections 107.16 and 303.03 of the VDOT R&B Specifications.

11. All engineering calculations supporting the design of erosion and sediment

control measures are contained in the project drainage file located in the VDOT Salem District Transportation and Land Use Section and will be made available for review upon request during normal working business hours.

12. The temporary erosion and siltation control items shown on the Erosion and

Sediment Control (ESC) Plan for this project are intended to provide a general plan for controlling erosion and sediment within the project limits. The ESC Plan is based on field conditions at the time of plan development and an assumed sequence of construction for the project. The contractor, in conjunction with the VDOT Project Engineer and/or ESC Inspector, shall adjust the location, quantity and type of erosion and sediment control items required based on the actual field conditions encountered at the time of construction and the actual scheduling and sequencing of the construction activities. Significant changes to the proposed ESC Plan (e.g., those that require an engineering analysis) shall be submitted to the applicable District Hydraulics Engineer for review and approval. Any changes to the proposed ESC Plan must be noted on a designated plan set (Record Set) which shall be retained on the project site and made available upon request during normal working business hours.

13. The areas beyond the project's construction area are to be protected from siltation. Perimeter controls such as filter barrier, silt fence, diversion dikes,

turbidity curtains, etc. shall be installed prior to any grubbing operations or other earth moving activities.

14. Temporary earthen structures such as dikes and berms are to be stabilized

immediately upon installation. Stabilization may include temporary or permanent seeding, riprap, aggregate, sod, mulching, and/or soil stabilization blankets and matting in conjunction with seeding.

15. All channel relocations are to be constructed during the earliest stage of

construction and shall be constructed in accordance with all applicable permit requirements and shall be constructed in the dry wherever possible.

SECTION III SWPPP

1. All documents related to the SWPPP for this land disturbance activity shall be maintained at the activity site and shall be readily available for review upon

request during normal working business hours. Such documents include, but are not limited to, the construction plans (or other such documents), the ESC Plan, the post construction SWM Plan (if applicable), the VDOT R&B Standards and Specifications, Supplemental Specifications, Special Provisions and Special Provision Copied Notes. Documents related to stormwater pollution prevention which is not a part of those documents referenced above, such as copies of the VSMP Construction Permit coverage letter (when applicable), the DCR General Permit For Discharges Of Stormwater From Construction Activities (when applicable) and those required to be developed by the contractor for stormwater pollution prevention associated with any support facilities for this land disturbance activity are to be maintained at the activity site with the other SWPPP documents. Where no facilities are available at the activity site to maintain the SWPPP documents, they are to be kept by or with the designated RLD at a location convenient to the activity site where they would be made available for review upon request during normal business working hours.

2. Other than construction at the activity site, there are no discharges associated

with industrial activity (e.g., from dedicated asphalt plants or dedicated concrete plants) produced by this land disturbance activity or (where applicable) covered by the VSMP General Permit For Discharges Of Stormwater From Construction Activities for this land disturbance activity.

* 3. Documents identifying the locations of off-site waste or borrow areas or material or equipment storage areas associated with or (where applicable)

covered by the VSMP General Permit For Discharges Of Stormwater From Construction Activities for this land disturbance activity shall be provided by the contractor in accordance with Section 107.16 (as amended and/or revised) of the VDOT R&B Specifications and will be maintained with the other SWPPP documents for this land disturbance activity.

* 4. A description of all pollution control measures that will be implemented as a part of this construction activity to control pollutants in stormwater discharges are identified and described in the contractor supplied documents, the construction plan set (or other such documents) and applicable sections of the VDOT R&B Specifications and Standards, including but not limited to, Specifications 107.16 (as amended and/or revised), 303.03, 603, 604 and 606 and Standards 113.01 through 114.08. 5. Requirements for the prevention of the discharge of solid materials, including

building materials, garbage, and debris, to surface waters of the state, except as authorized by a Clean Water Act 404 permit, are contained in Sections 106.04, 107.02 and 107.16 (as amended and/or revised) of the VDOT R&B Specifications.

6. Requirements for compliance with applicable state or local waste disposal, sanitary sewer or septic system regulations are contained in Sections 106.04 and 107.18 of the VDOT R&B Specifications.

*7. A description of construction and waste materials expected to be stored onsite or at off-site support facilities and a description of controls to reduce pollutants

from these materials, including storage practices to minimize exposure of the materials to stormwater and practices for spill prevention and response, will be contained in documents supplied by the contractor and maintained with the

other SWPPP documents for this land disturbance activity.

* 8. A description of potential pollutant sources from off site support areas and a description of controls and measures that will be implemented at those sites to minimize pollutant discharges will be contained in documents supplied by the

contractor and maintained with the other SWPPP documents for this land disturbance activity.

*9. By completing and submitting the SWPPP Certification form LD-445E, the RLD, or his authorized representative, certifies that all contractor supplied documents, identified herein, will be reviewed, approved and included with the other SWPPP documents for this land disturbance activity prior to implementation of work in those areas identified by such information.

10. The name of the VDOT individual responsible for the inspection of the erosion

and sediment control measures is identified on the LD-445E form which will be maintained with the other SWPPP documents for this land disturbance activity (Note: Individual shall be certified through the DCR ESC Inspector Certification Program).

11. For those land disturbing activities requiring coverage under the VSMP

General Permit For The Discharge Of Stormwater From Construction Activities, the SWPPP shall be made available for review upon the request of the DCR, the EPA, local government officials or the operator of a municipal separate storm sewer system (MS4) receiving discharge from the construction site.

* 12. For those land disturbing activities requiring coverage under the VSMP General Permit For The Discharge Of Stormwater From Construction Activities, the VDOT RLD shall post, or have posted, a copy of the General Permit coverage letter and a copy of a completed LD-445A form, noting the name and contact information for the VDOT person responsible for the land disturbing activity and its SWPPP, outside the project’s construction office along with other Federal and State mandated information. Where there is no construction office (e.g., a maintenance activity), the permit coverage letter and the LD-445A form are to be maintained with the other SWPPP documents for the land disturbing activity.

13. For those land disturbing activities requiring coverage under the VSMP

General Permit For The Discharge Of Stormwater From Construction Activities on or after July 1, 2009 that have not been previously covered by a VSMP permit, the SWPPP shall be made available for review by the public upon request. Such reviews shall be at a time and publicly accessible location convenient to the VDOT and shall be scheduled during normal business hours and no less than once a month.

SECTION IV POST CONSTRUCTION STORMWATER MANAGEMENT 1. This land disturbance activity is exempt from the Stormwater Management

Regulations because it disturbs less than 1 acre or less than 2500 square feet in an area designated as a Chesapeake Bay Preservation Area. N/A

2. This activity is linear in nature and the following stormwater outfall locations

are exempt from the requirements of the Virginia Stormwater Management Law because all of the following conditions are met (Note: Documentation must demonstrate that all three conditions (a-c) have been met and such documentation shall be maintained in the project drainage file located at the site identified in Note 6 of this Section): 104+53 Left, 108+14 Right

a. the land disturbance created by this activity at each listed outfall is less than one acre and,

b. there are insignificant increases in peak flow rates at each listed outfall and,

c. there is no existing or anticipated flooding or erosion problems downstream of the discharge point of each listed outfall.

3. The following outfalls do not require a permanent SWM facility because, as a

result of this land disturbance activity, the amount of additional impervious area draining to the outfall is less than 16% of the total right of way and permanent easement area draining to the outfall: N/A

Type I mulch (Straw) to be used on newly seeded areas adjacent to all waterways, wetlands, swamps, or any area in which drainage flows toward areas under the jurisdiction of the environmental regulatory agencies. Type I mulch shall be applied to provide a minimum 90% coverage. Type I mulch shall be tacked with Fiber mulch at the rate of 750 lbs. per acre and/or mulch tackifer. Type II mulch (Fiber mulch) may be substituted for Type I mulch at the recommendation of the District Roadside Manager. Type II mulch shall be applied at a rate of 1500 lbs. (net dry weight) per acre to provide a minimum of 90 percent coverage, and shall be applied in a separate application. Erosion Control Mulch, as listed on the VDOT Approved Products List, shall be applied in accordance with the manufacturer’s recommendations. Erosion Control Mulch shall provide 100% coverage of all denuded areas. All topsoil is to be free of hard lumps, clods, rocks and foreign debris and is to be hand raked to tie into existing lawns. All seed must be in conformance with VDOT seed specifications for Grasses & Legumes and be provided at the project site in bags not opened and labeled for use on VDOT projects with a green tag certifying inspection by the Virginia Crop Improvement Association.

Non-Standard Flexible Pavement Planing (item code: 16515) Non-Standard Flexible Pavement Planing shall include all requirements and specifications found in Section 515 of the 2007 VDOT Road and Bridge Specifications. The item shall be used for payment for all flexible pavement planning on this contract from one inch to 12 inches or as directed by the engineer. Payment for this activity shall be by the square yard. The pay item shall pertain to all areas depicted in detail and/or station range on the typical sections and plan views provided or as directed by the engineer.
