A Railway System comprises four parts forming a whole:1. The equipment, 2. The People who operate and maintain The equipment3. The associated procedures, 4. The environment in which these three parts have to operate
and the interfaces between them.
The railway system is complex and its very complexity means that a constant eye has to be kept on the consistency and proper integration of its different parts throughout the design and development process and, more generally, throughout the whole of their life cycle.

The increase of defects, such as quality of geometry are two elements to be able to master the first years after commissioning, under penalty of being quickly overwhelmed
In the case of a non-monitoring default, all means of monitoring and maintenance are monopolised for corrective actions disorderly at the expense of preventive maintenance scheduled.
