AIMS
• To prevent Adverse Drug Events (ADE’s) by implementing medication reconciliation
at admission in Burns Lake Hospital
• Goals:
– To increase the effectiveness of the medication admission reconciliation process over time
– To reduce the rate of Unintentional and Undocumented Intentional discrepancies at admission by 75% in 6 months
