MESSAGE FROM THE CHAIR

Please take a moment to leaf through this annual report, as it will give you an overview of some of the work we have been doing this past year. I would like to take this opportunity to highlight a few of the working groups that are really picking up steam. First and foremost is the A GP for Me initiative that has successfully engaged more than 1300 Burnaby citizens, physicians, and MOAs. Through surveys and engagement meetings, the A GP for Me working group (which includes physicians; community partners such as the school board, Burnaby Mental Health, and Fraser Health; and many more) has identified key areas of need in Burnaby. Our dedicated working group is now facing the challenging job of narrowing these down to key areas of focus for our initial funding proposal. Once the proposal accepted by the General Practice Services Committee (GPSC), implementation can begin!

One of our other successes during this past year, and a priority in the year to come, is public health education. We have begun a series of health education talks that have been extremely well received by “full-house” crowds. We plan to continue to focus on educating the Burnaby community through speaking engagements and a video series. The more the community understands about their health, the more they can partner with us to keep themselves and their families healthy.

The Global Family Care Clinic (GFCC), located at Kingsway and Edmonds, is a collaboration between the Burnaby Division, Fraser Health, and the GPSC that provides medical care for new immigrants and young families who often have complex health care needs, language barriers, and inadequate health literacy. It is another example of our success in identifying a need in our community, formulating a plan, and collaborating with partners to help those members of our community who are underserviced. New immigrant young families without a family doctor can be referred to the GFCC for initial health care until they can be transitioned to a family physician.

Our active and exciting IT working group is leading the charge towards enhanced EMR (electronic medical record) development, and working with the Pathways working group to bring Burnaby physicians a comprehensive database of available specialists and community services which will be your “go-to”, up-to-date, functional, user-friendly resource for all your referral needs.

This is just a small list of the many successes we are achieving together. Thanks to all of you for your time, input, energy, and hard work. Together we are re-building Burnaby as a healthy, happy, wonderful place to live and work.

As our fiscal year comes to a close, I hope you will join me as we celebrate the successes of our Burnaby Division of Family Practice. It has been a fulfilling year for me, as current Chair of the Division, and I thank you for allowing me and my fellow board members to help lead the Division to make a difference for Burnaby. The Division’s successes are actually YOUR successes. Thanks to your participation and commitment we have grown from a group of physicians who, for the most part, had lost their idealism and grown disconnected, discontented, and frustrated with the health care system, to a community of 165 physicians who are reconnecting with each other, with our specialist colleagues and community partners, and also rediscovering and remembering why we became family physicians in the first place.

MESSAGE FROM THE PHYSICIAN LEAD

I chose to be a family doctor because I love stories.

As family physicians we are privy to our patients’ stories — the history of their experiences and the joys and challenges of their relationships — and we are entrusted to become a part of these stories as we, understanding each person’s personal values, guide them in making the most appropriate choices.

At a recent strategic planning session, I invited our board members to share their stories. Why did they choose medicine? Why did they choose family practice? Why did they choose to work in Burnaby, and why did they join the Division of Family Practice?

We all need to remember why we are here and how we chose to do what we do.

Medicine chose me when, in grade six, I was hospitalized for inflammatory arthritis and was moved by the nurses and doctors who cared for me as a person and not just the disease. I wanted to help others to cope with challenging and uncertain circumstances and see and treat them as whole people.

I chose to work in my community because this is my hometown. I went to school here, played in most of the parks and spent countless hours in our libraries. To work here was to give back to the community that has given me so much more.

I chose to lead the Burnaby Division of Family Practice because our organization’s purpose resonated with my personal calling. When I talk to patients and my children about finding their calling – what they were meant to do with their lives — I identify their calling as the intersection of their talents, passions, and values with the needs of the world. What do you love to do and how can you use your talents in the most meaningful way to meet the needs before you?

Our mission is to engage, support, and mobilize family physicians in co-creating a network that will support the well-being of all members of our community. At the heart of our mission – at the core of all that we do – is the well-being of every patient.

As family physicians we care about our patients, but unlike other physicians, we share the unique perspective of seeing the whole person. We understand the resources of our community, and through new partnerships our organization has developed with others also interested in improving the health of our community we can help provide the care each patient needs.

The A GP for Me initiative asks each of us to expand our circle of care. As family physicians, we naturally make the patients we personally treat our primary concern. We are now asking ourselves to think beyond our own practices, and ask, “Who are our patients?”

The answer is, “Everyone is our patient” just as everyone is our neighbour, our brother or sister. We are all patients or future patients.

As a group we are engaged in the care of everyone in our community, especially those who do not yet have a family physician and are not receiving the care they need.

Our organization is involved in multiple initiatives that will ultimately enhance access to primary care, the health of our community, and the patient-doctor relationship. You as a member of the Burnaby Division can shape the story of primary care and ensure the health of our community.

MESSAGE FROM THE EXECUTIVE LEAD

The main priority over the last ten months has been the assessment and planning phase of the A GP for Me initiative, led by Paul Gallant, and the contribution of many members, their MOAs, and various stakeholders. Significant work has been done and should be completed by the end of the summer.

New initiatives for this coming year include the implementation of the A GP for Me project, the roll-out of Pathways (which will be available to all our members and their MOAs in the fall), and the expansion of the public education initiative, which aims to improve health literacy in the community.

In 2013 Leona Cullen joined us as a project coordinator and I would also like to acknowledge Sue Davis, our Physician Engagement Lead, who replaced Tanis Evans.

Thanks to all who have given their time to serve on working groups and committees. With everyone’s continued involvement and support I look forward to expanding the breadth and scope of the Division.

And finally, a special thanks to the founding members of the board of directors who represent such a hard-working and committed group of Burnaby family practitioners. You have much to be proud of in providing a solid foundation on which to build in the coming years.

Since joining the Division I have seen steady growth in our membership from an eight-member board to 165 physicians, two NPs, and one resident, and I have enjoyed meeting many of you at our events and meetings which embody the vision of our organization: Patients and Physicians Achieving Health and Happiness.

GLOBAL FAMILY CARE CLINIC

In November of 2013 we opened the Global Family Care Clinic in partnership with Fraser Health. The clinic provides services for immigrants and refugees who do not currently have a family physician and who have been living in Canada for less than three years. The clinic provides holistic primary health care to newcomers, including pregnant women, new mothers, children, and families. The clinic also has the ability to offer interpreters who speak over 150 languages, in person or by telephone.

Patients receive education about their own health and how B.C.’s health care system works. This includes information about making and keeping appointments, healthy eating and exercise, and taking medication if needed. Kimberley Reid, a nurse practitioner with Fraser Health, is funded through the Ministry of Health’s Nurse Practitioners for British Columbia (NP4BC) program. Kimberley works at the clinic with consulting family physicians from the Burnaby Division to ensure patients with complex health concerns receive the care they need.

The Global Family Care Clinic is a transitional place where individuals and families are seen for 18-24 months while they are settling into their communities and learning about the health care system. When they are ready, they will be offered help finding a family doctor in their community for continuing care. People are seen by appointment only, rather than on a walk-in basis.
