As always the preparation for the annual conference gathers momentum around this time. Registration is now open for the 28th International Conference in Hong Kong from 14th-17th September 2011. A record number of abstracts were received for this event, 1076 in all and importantly to note they represent 57 countries. All abstracts were reviewed by a group of international reviewers, to whom I particularly express my gratitude to, for their diligence and input. Authors have been contacted regarding the status of their submission.

Continuing the focus on members of the ISQua Board

Sir John Oldham, a primary care physician, is an internationally recognised expert on large system change and quality improvement. This is based on a career long commitment to driving change in organisations and systems to achieve what he has called since the 1990s ”the quality trinity” improved clinical outcomes, improved patient experience, and maximising the use of resources both human and financial.

He has done so taking examples and methods from other industries.

This led him to design and operate the NHS Primary Care Collaborative in 2000. This remains the largest improvement programme in the world covering 32 million patients in 40 months and delivering improvement in access to GPs and substantial reductions in mortality to patients with CHD. John Oldham also created the concept of the award winning Healthy Communities Collaborative with residents of deprived areas as the improvement team members. They created a substantive reduction (32%) in falls in older people. He has also worked in the field of Education, raising performance of underperforming pupils in secondary schools. He was invited by the Australian Government to design and train a team operate a Primary Care collaborative across the whole of Australia, and similarly Saskatchewan in Canada.

He is a keynote speaker at many international conferences on quality and safety. More recently, he was also a member of the National Advisory Group for the NHS Darzi review. He has written a couple of books and numerous articles on quality improvement and large system change, and has been a visiting professor at Vanderbilt University and elsewhere. He is now National Clinical Lead for Quality and Productivity at the Department of Health and a member of the National Quality Board, setting the strategic direction for quality and safety in the NHS.

He has been an ISQua board member for three years and is currently honorary Treasurer. He says of ISQua “There has never been a more pivotal moment for the topics of quality and safety in health care, nor a more relevant time for ISQua. Safety and quality are concepts that cross boundaries, as are solutions to the problems we share. Poor quality not only costs patients, but costs our health care systems. It is our moral duty as professionals to narrow variance and eliminate error and ISQua is an essential catalyst to helping us achieve those goals in our everyday practice. The future is exciting.”

Duncan Inverarity is a Senior Partner in the Dublin law firm ByrneWallace. His area of expertise is Health Law, Employment Law and Administrative Law.

Mr Inverarity recently joined the ISQua Board having a long history of advising the health sector both in his native Australia and adopted Ireland. He has been involved in a number of high profile cases over patient care issues in both jurisdictions which impact significantly on the management and care of patients and medical ethic issues.

“I am delighted to have recently joined the Board of ISQua. I have been impressed with what the organisation has achieved over the years. The current initiatives will improve dramatically the access to and transfer of knowledge of critical information on health care matters. It will make vital information readily accessible to all and will provide a forum for discussion on issues central to quality and safe

health care management. I am particularly impressed with the initiatives being taken by ISQua in lower income countries. Clearly, this is an area where health care must improve and ISQua is committed to achieving this.”

In his day job Duncan advises both public and private sector employers and regularly appears in Employment Law Fora, including the Rights Commissioner Service, the Equality Tribunal, the Labour Court, the Employment Appeals Tribunal, the Circuit Court, the High Court and Supreme Court. He also advises employers on strategy in the conduct of investigations, and the public sector on administrative law matters. He has regularly appeared in the High Court in relation to Judicial Review proceedings.

Under the new ISQua Memorandum and Articles of Association, approved at the Annual General Meeting in October 2010, Duncan was appointed to the ISQua Board.

HTAi and ISQua: a new partnership

HTAi and ISQua have important areas of overlapping interest – the appropriate use of health technology plays an important role in health system quality – and the MOU sets the stage for the two societies to collaborate in various ways. The most visible of these is participation at our respective annual international conferences.

HTAi will be holding a satellite meeting at ISQua’s meeting in Hong Kong in 2011. This will be an excellent opportunity for dialogue between the HTA and quality communities, and further details will be available on the conference website in the coming weeks.

What is HTA?HTA is a field of scientific research to inform clinical and policy decision-making about the introduction, use, diffusion, and innovation of health technologies. Health technologies include pharmaceuticals, devices, diagnostics, treatments, and other clinical, public health, and organizational interventions.

HTA is a multidisciplinary field that addresses the health impacts of technology considering its specific healthcare context as well as available alternatives. Contextual factors addressed by HTA include economic, organizational, social, and ethical impacts. The scope and methods of HTA may be adapted to respond to the policy needs of a particular health system. In health systems throughout the world, HTA plays an essential role in supporting decision-making about access to technology, its diffusion, and innovation.

In October of last year, ISQua signed a Memorandum of Understanding with Health Technology Assessment International (HTAi, www.htai.org), the global society for all those producing or using Health Technology Assessment (HTA).

HTA and related methods are frequently an input to the development of clinical practice guidelines and standards of care.

More broadly, HTA acts as a bridge between research, clinical practice, and policy, helping to establish the policy context for improvements in the appropriate use of technology and, as an ideal result, health system performance.

There is therefore great potential value in increasing dialogue between HTA and quality professionals, and in collaboration between HTAi and ISQua. Such collaboration might lend a distinct policy and innovation perspective to the field of quality, and might likewise work to enhance further the relevance of HTA for quality assurance and for improving patterns of care in health systems.

HTAi much looks forward to a long a fruitful relationship with ISQua, and hopes to welcome you to Rio in June!

What is HTAi?HTAi is the global scientific and professional society for all those who produce, use, or encounter HTA. With over 1200 members from 59 countries and six continents, HTAi is a thriving global network.

HTAi’s activities are diverse, and include a major annual conference, a senior-level Policy Forum, an official academic journal (the International Journal of Technology Assessment in Health Care), seven active Interest Sub-Groups that share resources and information, and various online resources (including an HTA Glossary, www.htaglossary.net). HTAi is committed to international collaboration, and has signed MOUs with ISQua, the World Health Organization, and the International Network of Agencies for Health Technology Assessment (INAHTA).

Hong Kong 2011 Educational Site Visits – Friday, 16th September 2011

Visit 1: Traditional Chinese Medicine in a New Era

School of Chinese Medicine, Hong Kong Baptist University, 7 Baptist University Road, Kowloon Tong, Kowloon

The School of Chinese Medicine (SCM) of Hong Kong Baptist University (HKBU) introduced the first Bachelor of Chinese Medicine and Bachelor of Science in Biomedical Science (Hons) Programme in 1998, and the Bachelor of Pharmacy (Hons) in Chinese Medicine Programme in 2001. Over the years SCM has built a comprehensive curriculum in Chinese medicine and pharmacy, from undergraduate degree programmes to postdoctoral research fellowships. SCM strives to promote modernisation and internationalisation of Chinese medicine in teaching, research, clinical service and technology. It established the Centre for Cancer and Inflammation Research in 2009 and the Hong Kong Chinese Medicine Authentication Centre in 2010. SCM also operates 11 Chinese medicine clinics to provide quality Chinese medicine services and conduct clinical research and teaching. The nine-storey purpose-built SCM building houses state-of-the-art teaching and research facilities.

Overview of Visit

Participants will visit a modernised Chinese medicine clinic equipped with computerized management system for patient record, pharmacy and herbs storage, and a newly established research centre for cancer, inflammatory conditions and novel therapeutic strategies that combine Chinese medicine and modern technology. The visit will also include a short tour to a museum of Chinese medicine.

Visit 2: Healthcare Opportunities with Advanced Technology

Established in 1922, Hong Kong Sanatorium & Hospital (HKSH) is one of the leading private hospitals in Hong Kong. With over 400 hospital beds and more than 20 specialist centres, HKSH takes pride in its wide application of advanced technologies, such as Dual Source CT, MRI, PET-CT, robotic surgery and Tomo Therapy, in patient care. HKSH is also among the first hospitals in Hong Kong to implement e-health records enabling better patient management and public-private partnership. Overview of Visit

The visit will include a hospital tour showing participants how HKSH applies the advanced health technologies to ensure quality patient care and efficient hospital operation. The tour will cover different hospital services such as Intensive Care Unit, Maternity and Paediatric Departments, and application of its key IT systems.

Visit 3: Smart Design for Efficiency

Prince of Wales Hospital, 30-32 Ngan Shing Street, Shatin

Established in 1984, the Prince of Wales Hospital is a 1400-bed acute regional hospital serving a population of 1.3 million. It is the teaching hospital of the Medical Faculty of the Chinese University of Hong Kong, one of the two local universities that provide doctors’ training. The hospital employs advanced information technology to increase work efficiency, achieve quality patient-centred care and enhance communication between departments and among 9000 colleagues of 7 hospitals in the district. In the autumn of 2010, the hospital saw its capacity increased by one-third with the addition of a new 14-storey multi-purpose block with a range of new facilities. The new design, equipment set-up, practices and workflow of the new hospital wards and units help to enhance efficiency and quality of health care.

Overview of Visit

The visit will focus on two concepts, “Smart Design for Efficiency” and “Simple IT, Total Solution”. The tour will cover central nurse call system and nursing station, negative pressure infection control rooms in each ward, filmless stations, flexible gender ward set-up, etc. There will also be demonstration of key IT systems- a comprehensive “Clinical Management System” with its array of clinical functions and interface with other systems and a cluster wide interactive intranet platform to enhance communication.

Evolution of the Quality Agenda

The evolution of the quality agenda has shifted from a perspective of self-regulation towards an external, politically controlled regulation. This may lead to a more systematic, leadership oriented and patient centred approach. However the risk of this shift could be a lack of ownership or even alienation of the professional community.

My focus on quality and patient safety during the past 20 years is mainly related to the publicly financed healthcare system of Denmark, a small country in the Northern part of Europe. However, it is apparent that the development and challenges in health care are identical across the industrialized countries. Different approaches and solutions have been chosen, and the international exchanges of experiences as they take place within the ISQua framework are therefore important.

In 1993 Denmark published the first National Strategy for Quality Improvement in Health Care. This strategy formed the basis of the first WHO strategy on this issue. As a junior medical officer in the National Board of Health, I was involved with the development of the Danish strategy. The basic principles were voluntary action, local ownership, shared responsibilities and the “Good Apples” theory. It was an objective to ensure high professional quality, minimal risk, continuity in care across sectors and high patient satisfaction.

Today, 20 years, later the goals are unchanged, but the principles and methods have been modified. The outcomes of these efforts, however, are unknown as an evaluation of the strategy and of the initiatives has never been carried out. Nonetheless it is my firm belief that the many years of quality efforts have made an impact and have reduced the possible negative effects endorsed by the increasing complexity and specialization in health care.

While I was regional Director of Quality in the healthcare organisation of Copenhagen, accreditation was introduced, and the hospitals in the region achieved accreditation by JCI in 2001.This lead to a political decision about the establishment of a national accreditation framework and Institute of Quality and Accreditation was established in 2005. Other important national milestones in this decennium are the Patient Safety Act in 2004 following a Danish pilot study on adverse events, the establishment of the National Indicator Project monitoring clinical performance, and finally the political decision of annual national surveys on patient’s experiences.

During this span of time there has been a shift in the basic principles on which quality improvement is based towards legislation, mandatory actions, transparency in data and leadership. Overall it has been in the form of a movement from self-regulation to external regulation.

Improvements still to comeMany joint initiatives have been taken in order to establish structure and processes. The actual quality challenges are to ensure better outcomes especially in areas such as cancer and chronic diseases. And although the Danish Act on patients’ rights is extremely progressive, health care is only slowly moving towards patient centeredness and the involvement of patient and/or relatives at the organizational level is only at the beginning.

Engaging the medical professional is crucial for bringing researched based knowledge into action In this respect the huge amount of unused clinical data contained in our different databases must be used for improving practice in order to closing the knowing-doing gap.

It is a fact that quality improvement in Denmark has attracted some highly qualified medical professionals, but also that the medical culture has still not caught up. Quality and patient safety therefore must be included in the medical curriculum and we need to ensure that the clinicians are onboard.

Internationally the evidence behind methods in quality and especially in patient safety is weak and there it is a joint responsibility to establish a more solid evidence base in the years to come.

Janne Lehmann Knudsen, MD, PhD, MHM

Dr Knudsen is a specialist in Public health and administrative medicine and is now the Director of Quality and Documentation in the Danish Cancer Society, and lecturer at the University of Copenhagen. She has previously been appointed to the Danish Hospital Institute and the National Board of Health.

For 12 years she was the director for quality and development in the Capital region of Denmark. Currently she is appointed as a leading surveyor and a member of the advisory board of the Danish Institute for Quality and Accreditation. She was responsible for the planning and the organisation of the ISQua conference in Copenhagen 2008. A textbook on Regulation of Quality in Health Care is among her latest publications.

International Accreditation Programme (IAP) Update

The IAP remains the only healthcare specific external evaluation body that “Accredits the Accreditors” and is now formally recognised by Ministries of Health in a number of countries. Over the last three years activity has increased by 116% and currently ISQua accreditation has been awarded to 19 organisations, 31 sets of standards and 7 surveyor training programmes, in 29 different countries. National accreditation schemes, in both health and social services continue to seek accreditation with ISQua. We recently welcomed the Central Board for Accreditation of Healthcare Institutions, Saudi Arabia and Healthcare Improvement Scotland into the programme.

None of this work would be possible without our surveyors, totalling 58 from 19 countries. The Accreditation Council recently approved Ahmad Al Khateib, Jordan, Ann Jacobson and Claudia Jorgenson, USA, as surveyors. Each of these surveyors came very highly recommended by their organisations and we are extremely grateful to them and their peers for volunteering their time and energy.

Like any quality improvement framework we are constantly evaluating it’s structures and processes. ISQua undertakes post survey evaluations, all information gathered from June 2009 to December 2010 has been analysed and will be used to inform improvements to the IAP. Some feedback relates to the standards and rating scale which will be taken into consideration with the standards are reviewed. We are now working on these results to make improvements where we can.

It is planned to commence the revision of the International Principles for Healthcare Standards, 3rd Edition and ISQua Accreditation Standards for Healthcare External Evaluation Bodies, 3rd Edition within the next two months. A subgroup of the Accreditation Council has been formed to oversee this project, chaired by Brian Johnston, Australia. It is anticipated to publish these revised standards at the beginning of 2012.

The ISQua Board now feel that a strategic review of the IAP is timely and to this end an Accreditation Strategic Development Group was established, chaired by Wendy Nicklin, Canada. Health Care Informed will conduct a portion of this review and may contact you, within the coming weeks. I would be very grateful if you could take the time to provide feedback to help us improve the current programme and maybe even expand into other areas.

The Accreditation Council met in The Netherlands in April for their regular mid-year meeting. Thanks to Helene Beaard and NIAZ for hosting this event. This meeting saw representatives from Australia, Canada, Denmark, Jordan, The Netherlands, France, United States, India, South Africa and Malaysia reviewing the IAP’s current activity and planning future directions.
