We would like to thank you for your purchase of the “Maryland’s Eastern Shore” Blue Crab license plates. Proceeds from your purchase directly benefit the Bryan Brothers “Building Dreams for Youth” Foundation, Inc. and support a brighter future for children of the Mid-Shore. Please find attached the Official MVA Organizational License Plate Form and note the following instructions:

1. Please fill out the form completely. All of the information can be found on your vehicle registration card. (Note: Line two will be signed by Foundation.)

2. Enclose a one-time fee of $50. Make your check out to “Bryan Brothers Foundation, Inc.” $25 of the fee is a tax-deductible donation and $25 goes to the MVA to produce the plates as noted by the circled item on form.

3. You must have current & permanent Maryland plates to apply. You do not have to wait until your renewal and there is no recurring fee. When you get your “Crab” plates by mail, simply return your plates to MVA.

4. Please mail your application and check for $50.00 to the address below. Do not return application to MVA directly as it will not get processed.
