BRITISH COLUMBIA MINE SAFETY AWARDS Open Pit Mine and Quarry Awards Application Form Established in 1961 the Open Pit Mine and Quarry Awards recognize excellence in open pit and quarry safety. Award application deadline is February 1st. This year’s awards will be presented at either the Safety Awards Banquet or a combined Provincial Mine Rescue & First Aid Competition & Safety Awards Banquet We hope you will join us and others from industry as we celebrate your success in safety, the team effort of your employees and the success of those involved in mine rescue thoughout the province. Each safety award-winning mine will receive three (3) free awards banquet tickets, one for each of the two OHSC Co-chairs and one for the Mine Manager. The award winners will be finalized by April. Should a mine win an award, up to three (3) pre-purchased tickets will be refunded. We also invite and encourage your attendance at the Provincial Mine Rescue and First Aid Competition to support the teams while enhancing your knowledge about mine rescue . The competition is held the second Saturday in June from 8 a.m.–4 p.m. in a BC community. For banquet information, visit www.minesafetyawards.gov.bc.ca Eligibility and Rules All permitted mines in B.C. which have accumulated the required number of combined worker and contractor hours and have no fatalities in the competition year are eligible for the awards. At the Chief Inspector of Mines’ discretion awards will be given to those eligible open-pit mines and quarries which have the lowest injury-frequency rate in the competition period. Other factors that will be considered in adjudicating awards are lost-time accident frequency rates for the two most recent consecu- tive years and a comparison to the industry average.* *Industry average is determined by WorkSafeBC’s estimated injury rates. Injury rate is defined by WorkSafeBC as the number of short- term disability claims per 100 person years of WorkSafeBC covered employment. Open Pit Mine and Quarry Awards

These awards recognize the dedication of industry, unions and workers who are committed to maintaining a high level of health and safety at B.C. mines.

The awards are presented annually by the Chief Inspector of Mines to mines and quarries for their safety accomplishments in the previous calendar year.
