Application for

European Technical Assessment

The BBA will prepare a service agreement for preparation and submission of the technical file plus a contract for delivery of the European Technical Assessment on the basis of the information supplied by you in the attached application form. Part One of the form is provided to register your application with EOTA. It is important that your requirements are fully and precisely defined within this document as failure to do so could result in errors or omissions in the contract. If there is insufficient space for any of your entries on the form please insert extra pages. If you consider that any of the questions do not apply to your product, mark as ‘not applicable’. Please do not leave any sections blank. Information supplied by you will be treated in confidence and will not be disclosed other than as required to any third party that the BBA instructs or consults or in connection with your application under statute; or by English law. Where appropriate, reference should be made to the relevant EOTA document. Please contact your BBA Area Business Manager or our Customer Centre if you have any queries.

Steps to CE marking

• Step 4 – the designated EOTA members will produce a methodology* to deliver the ETA – programme of tests, calculations etc. – existing data may be considered *The European Assessment Document (EAD) Step 5 – draft methodology finalised with comments from Applicant (Manufacturer) / European Commission • Step 6 – work programme carried out according to EAD

• Step 7 – ETA drafted and circulated for comment (applicant / EOTA)

• Step 8 – Comments resolved – ETA Issued
