Bristol Community Health Role Profile
Job Title: Community Navigator CoordinatorBand: Band 5Pay Bracket £21,909 – £28,462 per annum depending on experienceReports to: PPE Lead, Bristol Community HealthJob Purpose: Responsible for the day to day management of the Community
Navigator project, to deliver a high quality volunteer led service aimed at reducing isolation and loneliness in older people aged 50+ in the South, Central and East of Bristol
Recruit and supervise local Community Navigator Volunteers to support the activities of the project
Managing the expectations and performance of the volunteers
Improve access to voluntary and community sector organisations and support groups for older people in Bristol
Liaise and network with local agencies, groups, health and other care professionals, attending outreach activities in community settings to promote the service
Analyse service needs and gaps in provision and act on evidence or make recommendations for action.
Expand and develop the Migrant Health service across the city and raise its profile leading to improved service provision.
Promote cultural awareness and identify sensitive issues within healthcare and how different communities perceive these issues.
Work on equity of access to the service by all communities and segments of the population, promoting cultural awareness and identifying sensitive issues that the diverse range of older people in Bristol might face
To actively coordinate key meetings and maintain good communication with the project partners, who include Care Forum, Barton Hill Settlement and Southville Community Development Association
Key Responsibilities:
To be responsible for the recruitment, day to day line management, training and supervision of volunteers.
Instilling a sense of pride, passion and commitment in the
volunteers for the job they are doing
Responsible for effective monitoring and management of the community navigator project
Setting up and running a steering group which will have oversight of the project, and attending it on a regular basis
To be responsible for data collection, monitoring and production of reports in relation to the service to ensure productivity and efficiency across the service.
To design and implement training programmes for volunteers with the support of the training department.
To be aware of and identify the diverse range of older people across the South, Central and East of Bristol, and have a good awareness of the differences in these communities and the different methods of engagement appropriate depending on these differences
To work independently and autonomously and to support team members in all areas of their work
To involve local people and voluntary organisations in service changes and the development of the community navigator project
Lead on obtaining and using feedback from older people to shape and improve the service on an ongoing basis
Work on equity of access to the service by all communities and segments of the population
Establish and maintain links between the Bristol Community Health Community Navigator service and the service running in the North
Use a Common Measurement Framework to track the progress of individuals who are seeing a navigator
Demonstrate a commitment to learning throughout the project, and a willingness to adapt practice based on this learning
Manage, respond and prioritise queries , using initiative to prioritise responses with internal and external agencies
Manage, respond and support colleagues when dealing with difficulties arising in complex/sensitive cases/issues e.g. child protection and domestic abuse issues.
Use own initiative to prioritise workloads and that of the volunteers where necessary
Use and maintain efficiently filing systems, including paper and electronic documents.
Ensure that own work and that of others meets BCH standards.
Ability to travel to partner organisations and attend public forumsSkills Specification
Able to manage relationships and work as part of a team, promoting good team working relationships.
Able to demonstrate excellent communication skills with individuals and external organisations, using a variety of verbal, written and electronic tools.
Able to work both independently and collaboratively to set up and run a project using a ‘test and learn’ methodology
Experience Experience of recruiting, supervising and managing volunteers
Experience of working with a variety of partners to achieve a common goal
Experience of maintaining performance measurement frameworks and using these for reporting
Experience of establishing or being part of a steering group that is responsible for delivering a project
Experience of working with older people
Experience of facilitating training and development programmes for volunteers or other members of staff
Experience of working in a team and understanding of team dynamics.
Experience of using community engagement tools and techniques
Understanding of data protection, patient confidentiality and freedom of information policies as they relate to waiting list management
Able to use Microsoft Office word processing systems
Able to maintain own and office filing systems and access information in a timely manner
Able to analyse and monitor the effectiveness of systems and take action to improve performance where necessary.
Knowledge and qualifications
Good knowledge of the local communities in South, Central and East of Bristol, as well as the Voluntary and Community Sector organisations operating in these areas for older people.
Specialist knowledge in a relevant subject area, through either experience or a qualification to a degree level equivalent
Knowledge of the framework for accessing health and social care services in Bristol
Knowledge of the issues faced by older people in the Bristol area
Demonstrable knowledge of the Equality Act 2010.
An understanding of the implications of cultural differences for service delivery
Able to identify and report awareness of vulnerable adults and children working within a BCH framework.
