PROPOSED RECOMMENDATION That the Board of Health for Peterborough Public Health consider the following recommendations for leadership positions for the Board of Health for 2020:

Board of Health Chairperson – Mayor Andy Mitchell

Board of Health Vice-Chairperson – Ms. Kerri Davies BACKGROUND Given that the Board follows Robert's Rules of Order for governing the calling of and proceedings of meetings (as per By-Law #3), elections must occur via nomination. Dr. Salvaterra will call for nominations for the position of Chairperson of the Board of Health for 2020. The elected Chairperson will call for nominations for the position of Vice-Chairperson of the Board of Health for 2020. In October, staff reached out to members to assess interest in leadership and committee appointments. The Governance Committee reviewed responses at their meeting on November 19, 2019, and has proposed the recommendation noted above.

PROPOSED RECOMMENDATION That the Board of Health for Peterborough Public support the renewal of provincial appointments for Catherine Praamsma, Kerri Davies and Greg Connolley. BACKGROUND The Governance Committee met last on November 19, 2019. At that meeting, the Committee requested that the above-noted recommendation come forward to the Board for consideration. Peterborough Public Health Board membership currently consists of five (5) provincial appointments. Term expiry dates for those appointments are as follows:

Catherine Praamsma (April 26, 2020)

Michael Williams (April 26, 2020)

Kerri Davies (October 21, 2020)

Greg Connolley (November 18, 2020)

Andy Sharpe (March 3, 2021)

As per Board Policy 2-261, the incumbents with terms expiring in 2020 were contacted to determine their interest in renewal. Catherine Praamsma, Kerri Davies and Greg Connolley expressed interest in renewing their terms. Alida Gorizzan, Executive Assistant, contacted the Ministry’s Agency Liaison and Public Appointments Unit to confirm the maximum number of years an appointee can serve given that two members have already served six years each. The Ministry confirmed that appointees can serve a maximum of ten years, and noted that they would be eligible for reappointment. The Ministry was also asked about renewal timelines given there have been some boards of health with vacancies that remain unfilled. They responded that “it usually takes about four weeks to get all the approvals for reappointments. When we receive the reappointment packages, we need to obtain Ministry’s direction on the reappointment. After we receive it, the approval process starts.” Staff have been advised that some local public health agencies have experienced longer delays in filling these appointments.

TITLE: Establishment of Honourarium for 2020

PROPOSED RECOMMENDATION That the Board of Health for the Peterborough Public Health approve a 1.7% increase in honourarium for its members, representing a total amount of $154.16. BACKGROUND The Board has an established policy and procedure for establishing honorariums for meeting attendance (policy 2-150) and for review of remuneration (2-153). Based on these policies, the Governance Committee is to request Board approval for changes to honorarium amounts at the first Board Meeting of the year and that the request be based on wage increases for unionized staff or the Ontario Consumer Price Index (CPI), whichever is less. The Governance Committee met last on November 19, 2019 to address this matter. In 2019, the Board approved a 1.0 % increase in honorarium paid retroactive to January 1, 2018 and a 1.5% increase effective January 1, 2019. According to Statistics Canada, the CPI Inflation rate for 2019 (available to September 2019) was 1.7%. Staff wage increases were/are 1.75% for all three unions in 2020. Therefore, the lower amount of increase at 1.7% is recommended. The current honorarium rate is $151.58 per meeting. Based on the above-noted increases, the 2020 rate would be $154.16.

Peterborough Public Health is offering support to our local communities to assist with strengthening “Municipal Alcohol Policies” to ensure the safety and health of our residents. In case this is relevant for your First Nation community, we would like to reach out and extend our support in updating any polices regarding alcohol and special occasion permits (SOP). Current proposed changes that have an impact at the local level include:

Changes to SOPs through the Alcohol and Gaming Commission of Ontario (AGCO) loosening regulations on hours of sale, eliminating barriers around licensed areas for consumption, and ending the regulation on serving food at events.

New SOPs for Tailgating Events allowing patrons to bring their own alcohol to consume in a confined area for exclusive sporting events including professional sports, semi-professional sports, and post-secondary events.

Allowing communities to designate public spaces for the consumption of alcohol. This includes parks, beaches, and other community owned property.

Creating alcohol policies and by-laws allow communities the opportunity to mitigate the potential harms and costs associated with increased accessibility and consumption of alcohol. Increased accessibility and liberalization of alcohol consumption is a well-documented risk factor for problematic use leading to criminal behavior, violence, and chronic disease outcomes. This has the potential to impact costly services including maintenance, emergency services, and public health. In 2014, the total cost attributed to alcohol use in Canada was estimated at $14.6 billion. This includes the cost for healthcare services, criminal justice, loss of productivity, and direct costs.1 Increased access and acceptability of alcohol use will lead to increased harms for residents and therefore a higher burden of cost for communities. Local Councils play an important role in mitigating harms associated with alcohol consumption for their residents. These changes have created the opportunity to develop policies in the best interest of the public’s health and safety concerning alcohol consumption. The following list outlines some local strategies that can minimize the risk of alcohol related harms to residents through alcohol policies:

This update is a tool to keep alPHa's members apprised of the latest news in public health including provincial announcements, legislation, alPHa activities, correspondence and events.

Update on Public Health Modernization

The formal consultation of the Ontario government's plan to modernize public health and emergency health services was launched on November 18, 2019. The Ministry of Health is

seeking input from a variety of public health stakeholders, in a range of formats over the coming months. Along with many others, alPHa will be making a formal submission to the

consultation.

To this end, we are inviting feedback from members to inform a collective response. This

response is intended to reflect the major themes and top priorities that are common throughout the local public health sector while acknowledging that different local circumstances

will generate different points of view. alPHa's submission will build on its Statement of

Principles that was released on November 15.

Members are being asked to review the province's Discussion Paper and the alPHa Statement of Principles and provide answers to the questions posed in the consultation discussion paper.

Health units have been given a template by alPHa to complete and have been invited to submit

one completed template/response (per health unit) to the association by January 3, 2020. Please note that this is not intended to replace health units' own responses to the consultation.

alPHa encourages members to avail themselves of the various opportunities for input to ensure that local and collective voices are heard during this consultation.

Upcoming Winter 2020 Symposium & Section Meetings: February 20 & 21

alPHa will be holding its Winter 2020 Symposium on February 20 and Section Meetings on

February 21 at the Central YMCA in downtown Toronto. The Winter Symposium will be a full-day event featuring a morning workshop on leadership, collaboration and change management

with speaker and facilitator Tim Arnold followed by an afternoon dialogue on Public Health Modernization. Section meetings for board of health members and medical officers of health

will be held the next morning until noon at the same location. Stay tuned for more program and registration details in the new year. In the meantime, members are advised to start

booking their hotel accommodations at nearby hotels (see flyer below).

DECISION HISTORY At its June 10, 2015 meeting, the Board of Health formally adopted the National Standard on Psychological Health and Safety for the organization and endorsed the creation of the Guarding Minds at Work Steering Committee to oversee the implementation of the standard for Peterborough Public Health. Earlier status updates were provided to the Board of Health on November 9, 2016, January 13, 2018, and March 13, 2019. BACKGROUND Psychological health and safety is about safeguarding the psychological health of employees. A psychologically healthy and safe workplace is one that promotes employees’ psychological well-being and actively works to prevent harm to employee psychological health due to negligent, reckless or intentional acts. Guarding Minds @ Work (GM@W) is an evidence-based, guided process developed by Canadian mental health experts to support the implementation of the National Standard on Psychological Health & Safety in the Workplace. According to GM@W, psychological health comprises our ability to think, feel and behave in a manner that enables us to perform effectively in our work environments, our personal lives, and in society at large. Psychological safety is different - it deals with the risk of injury to psychological well-being that an employee might experience. Improving the psychological safety of a work setting involves taking precautions to avert injury or danger to employee psychological health. It is important to note that psychological health problems occur on a spectrum, from mild psychological difficulties on

one end to severe psychological disorders on the other. The most common psychological health problems in the workplace are anxiety and depression. These conditions account for a large proportion of the negative impacts on employees and employers. Peterborough Public Health asked its employees to complete the GM@W survey in 2016 and again in 2018. This survey measures staff perceptions of workplace health according to 13 psychosocial factors. Both surveys generated strong response rates of 85% or higher. When comparing results from the 2018 survey to the 2016 benchmarks, each psychosocial factor improved and one remained the same. According to the GM@W rating system, all indicators ranked either as a “minimal concern” or a “relative strength”. These survey results are foundational to the work of the GM@W Steering Committee in identifying priority psychosocial factors that could be improved through targeted workplace initiatives. Engagement (PF10) and Protection of Physical Safety (PF13) rated as the two highest indicators, and Organizational Culture (PF2) and Civility & Respect (PF4) rated as the two lowest. These results of the top and lowest ranking psychosocial factors are the same as in 2016, however all improved. A breakdown comparing 2016 and 2018 results is provided below:

In January 2019, the GM@W Committee presented the results of the 2018 survey to staff as part of the All-Staff half day as a way to inform the discussion of next steps. There continued to be strong support for continuing the work of developing all employee’s conflict management skills, and this was the major focus of the committee’s work throughout 2019. In March 2019, the committee hired non-violent communication expert Marion Little from the John Howard Society to develop two half-day sessions designed to coach staff on leading a conflict management exercise that could be delivered throughout the organization. An invitation went out to all staff to participate in these voluntary training sessions on March 18 and March 25,

and 27 staff signed up. Of this group, 11 staff joined the team of conflict management exercise facilitators who were responsible for delivering the exercise to their teams. Marion Little helped refine the conflict management exercise so it covered the key elements of understand the needs and feelings of people in conflict, and how through curious questioning, listening, self-awareness and self-care one can navigate through charged emotions to mutually resolve the problem at hand. The exercise consists of a powerpoint slide deck and video depicting a difficult conversation between a manager and employee. These materials are then used by facilitators to lead group work activities where participants learn to apply the concepts and techniques. In 2019, this exercise was delivered by facilitators six times at team meetings reaching a total of 74 employees. According to follow-up evaluations, these sessions were well-received by participants and generated positive change as staff gained more confidence dealing with conflict situations, both personally and at work. Throughout the year the GM@W Committee organized six one-hour booster sessions with Marion Little and the facilitator team to address any issues that may have arisen during the exercises and offer more coaching on conflict management. This work will continue into 2020 with a system to reach all staff and include the training as part of new staff/student orientation. With the provincial government’s sudden announcement in April 2019 of significant changes to the public health system and possible job losses caused by funding downloads and health unit amalgamations, the GM@W Committee recognized the need to address the psychological effect of this uncertainty on staff and the work environment. They developed a series of “Therapeutic Thursdays” from June until December that brought staff together over lunch hours to enjoy fun activities together. These events included line dancing, mindfulness training, a YouTube video on handling uncertainty, board games, mixed martial arts to music, and gingerbread house decorating. These events were modestly attended and fostered mutual support among those who took part. More importantly, it reflected that PPH is an organization that cares about its employees. The GM@W Committee meets again in January 2020 under the new leadership of Patti Fitzgerald, Manager of Infectious Disease Programs, and Julie Ingram, Manager of Environmental Health. The GM@W 2020 Work Plan will include another staff survey (this happens every two years) and also consider ways of enhancing staff resilience in light of the restructuring of Ontario’s public health system. STRATEGIC DIRECTION This work achieved to date and future plans to create a safe and supportive workplace helps PPH serve as a role model in implementing the new National Standard on Psychological Health and Safety, as well as fulfill objectives as set out in the Strategic Plan under the Quality and Performance strategic direction.

- Discussion Paper - Modernization of Public Health (Survey Response from PPH) - Position Paper - Submission to Jim Pine, Special Advisor

BACKGROUND On November 18, 2019, the province announced the release of discussion papers on the modernization of the public health sector and emergency health services (EHS). Feedback on the papers can be submitted via an online survey by February 10, 2020. In addition, the province also announced it would be conducting in-person consultation sessions in late 2019 and into early 2020 across Ontario. The Board of Health’s Modernization Working Group met that day, and again on November 26, 2019 and recommended the following:

- send an invitation (jointly with the City and County) to Jim Pine, Special Advisor, to hold a consultation in Peterborough;

- consult with staff to prepare a response to the online survey; and, - prepare a position paper for submission to Jim Pine.

An invitation was sent shortly thereafter, and a session was later confirmed for December 19, 2019. On this date, the provincial team overseeing the modernization of public health and EHS came to Peterborough for face-to-face consultations. Special Advisor Jim Pine facilitated a full day of meetings, with public health discussions in the morning. Also present were Executive Lead Alison Blair, Chief Medical Officer of Health Dr. David Williams, and their staff.

The consultation consisted of a number of open-ended questions and a free flowing of comments from the participants, representing boards of health, First Nations, local public health agencies and municipalities in the Durham, Haliburton, City of Kawartha Lakes, Northumberland and Peterborough areas.

PPH board members participated actively and made many good points. Among them was Mayor Andy Mitchell’s key points about the need for modernization, the importance of focusing on our communities as our key clients, the use of evidence for decision-making, and

the importance of governance. He emphasized that boards of health needed to have local accountability, and that budgets had to be affordable. He hoped that the province would consider models that made the best of both provincial and local efforts.

Important takeaways from the day were that this current process is a “reset” and that it represents a true consultation, congruent with the wishes of our board. The timelines have been adjusted, with February 10th still a firm deadline for the survey, but acknowledgement that the consultation will stretch to the end of February, with further discussions, as required, into the spring. Internal feedback was gathered for the survey until December 20th, and the Executive team worked on drafting the survey response and position paper over the remaining weeks in 2019. The Modernization Working Group is scheduled to meet on Monday, January 6, 2020, to review both documents. These will be circulated to board members for review as soon as they are finalized as they will require board approval on January 8, 2020.
