State of Illinois Capital Development Board Request for Board Approval of Single Bid CDB Project No. 039-150-191 Repair/Resurface Roadways and Sidewalks Illinois State Fairgrounds Springfield, Sangamon County, Illinois Subject: Single Bid Award Project History: The scope of work for this project provides for rebuilding and resurfacing the roadways and sidewalks at the Illinois State Fairgrounds. Current conditions on both the roadways and sidewalks have deteriorated to such an extent that repairs are necessary to prevent vehicle damage and to remove tripping hazards from the walkways. The scope of work for the project also includes an ADA assessment of the fairgrounds as it relates to this scope of work to ensure compliance with the Illinois Accessibility Code. Requested Action: This project bid on February 17, 2021 and a single bid was received that was significantly over the estimate. CDB made the decision to rebid this project on August 4, 2021 and a single bid was once again received for the general trade. Three plan rooms and seven contractors held drawings for the project. State of Illinois Capital Development Board Request for Board Approval of Single Bid CDB Project No. 120-007-018 Replace Cooling Tower & Refrigeration Equipment/Upgrade Temperature Control Systems Western Illinois Correctional Center Mt Sterling, Brown County, Illinois Subject: Single Bid Award Project History: The Dietary Warehouse is a 50,000 square foot, 2-story building established in 1987. The scope of work for this project provides for the installation of larger capacity refrigeration equipment and a larger capacity cooling tower, if necessary, at the Dietary Warehouse. The scope of work also provides for replacement of the temperature control systems at Dorm 1, Dorm 2, Dorm 3, Dorm 4, and the Maintenance Building. Requested Action: This project bid as a single prime project. Bids were received on August 4, 2021, and a single bid was received. Seven plan rooms and twenty-two contractors held drawings for the project. Of the contractors that held drawings, seven (7) were mechanical contractors, two (2) were controls contractors, seven (7) were electrical contractors, four (4) were general contractors, and two (2) were paving contractors. Plans were also sent to the head of the Carpenter’s Union, the head of UA Local 137 Union for Plumbers, Steamfitters, and HVACR Contractors. The Architect/Engineer made several phone calls and sent emails to each of these companies in an attempt to gain interest in the project. However, the project received only one bid from Henson Robinson Company. There does not appear to be any concerns with the scope of work or the bid documents in how they were presented. In discussions with Illinois Union Leaders, there appears to be a lack of manpower and market uncertainty due to the effects of COVID 19. Leaders have seen this issue repeated across industries. It is our professional opinion that exhaustive efforts have been made to secure bids for this project. The project was bid as a single prime bidder. Bids were based upon Contract Documents dated May 4, 2021. The project was sent to seven (7) Mechanical Contractors, two (2) Controls Contractors, seven (7) Electrical Contractors, four (4) General Contractors, two (2) Paving Contractors, the head of the Carpenter’s Union, the head of UA Local 137 Union for Plumbers, Steamfitters, and HVACR Contractors, as well as seven (7) Plan Rooms.

Ross & Baruzzini, Inc. made several phone calls and sent emails to all of these companies to attempt to gain interest in the project. The project received only one bid from Henson Robinson Company. Only one bidder, Henson Robinson Company, attended the Pre-Bid Meeting.

There does not appear to be any concerns with the scope of work or the bid documents in how they were presented. In discussions with Illinois Union Leaders, there appears to be a lack of manpower and market uncertainty due to the effects of COVID 19. Leaders have seen this issue repeated across industries. The scope of work at the Assembly Hall provides for roofing removal and replacement of all roof areas. All existing roof systems are to be removed down to vapor retarder membrane covering the structural roof decks. New insulation and fully adhered EPDM roof systems shall be installed per the drawings.

The scope of work for the Old FTC Building provides for roofing removal and replacement of all roof areas. All existing roof systems are to be removed down to vapor retarder membrane covering the structural roof decks. New insulation and fully adhered EPDM roof system and a laminate fiberglass shingle system shall be installed per the drawings.

The scope of work for the Mendel Building provides for roofing removal and replacement of all roof areas. All existing roof systems are to be removed down to the structural roof decks. New insulation and fully adhered EPDM roof systems shall be installed per the drawings.

Description of RFP Change: When the contractor removed shingles on the Old FTC building (BR026), they discovered that the existing shingles were installed directly over gypsum sheathing with no plywood decking. Certainteed, who is the manufacturer of the replacement roofing materials, does not allow for the installation of shingles over gypsum roof deck and to do so would invalidate the warranty. The original design documents did not indicate the presence of gypsum on the sloped roof decks. The contractor will need to install new plywood sheathing on top of the existing gypsum deck before placing shingles.

Requested Action: We are requesting board approval of Proceed Order G-1 in the amount of $195,000.00 to allow the contractor to place new plywood sheathing materials over the existing gypsum deck.

Chicago, IL 60601 RE: CDB 321-055 Replace the Roof Elgin Mental Health Proceed and Change Order To whom it may concern: Per your request this letter is to document how the Proceed Order work relates to the design documents and the Change order and the reasoning associated with it. Additionally you have requested the need for the Proceed order such as time frame and urgency and how it relates to the critical path. The change to the design documents occurred when the contractor removed shingles on the BR026 - Old FTC building and discovered that the existing shingles were installed directly over gypsum sheathing with no plywood decking. The shingle manufacturer for the new shingles that are to be installed will not warrantee the shingles if they are not fastened to a minimum ½” CDX plywood sheathing substrate. Our drawings are being altered to show the addition of the new ½” CDX plywood over the existing gypsum board. This change created revisions on our sheets A1.B and A2.B which details the changes and installation (see attached). This was an existing condition that could not be determined due to no existing construction documents or as-built documents to determine this unusual construction method. Cores were performed on the EPDM areas of the roof but not in the shingled area as is a typical coring procedure. (Please see Nuclear Roof Moisture Survey performed by FTL Flood Testing Labooratiories, Inc. dated 9/24/2019). This additional work is to be accomplished in 28 days from the approved Proceed order and changes the critical path on this building 28 days. Additionally the contractor has assessed an additional cost of $186,022.07. This is an urgent and required change as it affects warranty, construction schedule and integrity of the work. The site is located on 60 acres in Park Forest and is surrounded by roads parking lots, sidewalks, and handicapped paths. The center is divided into three (3) residential units comprised of 13-14 homes each. Each of the 40 ranch-style homes has a kitchen, dining/living room area, utility room which contains a washer and dryer, two full and one-half bathrooms, and five bedrooms. The scope of work for this project provides for the reconstruction of roadway pavement, curbs and gutters, concrete sidewalks, asphalt pathways, and parking lots. The roadway will be replaced in kind by maintaining the existing roadway width, impervious area, and storm drainage pattern. This work is necessary because the existing roads are dilapidated and require replacement. The facility cannot be closed and will remain active throughout construction. As such, work will be coordinated with the facility to ensure there are no disruptions to service. This project received Authorization to Proceed on March 5, 2021. The Using Agency later requested another job (Project: 321-110-068) be completed at this site to replace watermain piping, fire hydrants, and service to each building because the current water service piping is failing. Work performed on the watermain project would cause the newly reconstructed roadways to be torn up. The project team made the decision to wait to perform the roadway reconstruction work until after the watermain project has been completed. However, patching will need to be completed on roadways and parking lots to enable the facility to use the roads throughout the winter. In the Spring of 2022, the project team will work to coordinate the sequence of the watermain project and the roadway project to ensure both projects are completed efficiently. _________________________________________________________________________________________________ Description of RFP Change: Due to project sequencing and extremely deteriorated roadways the using agency is requesting temporary patching and paving to existing roadways to provide a safe driving environment. facility that cannot be closed during the construction. Sequencing of the work

Project received NOIA 1/28/2021 and ATP was issued to Contractor 3/5/2021.

At the same site project to replace watermain piping (321-110-068) was requested

by UA at the later date with roadway project design already in works. This Project

is to replace site watermains, fire hydrants and service to each building. Existing

water service piping buried throughout the site is failing.

Project team decided to wait on award of watermain project in order not to tear up

new roadways.

321-110-068 water main project received NOIA 8/27/21 and we had a PLA mtg

9/7/21, Contractor is working on getting the required paperwork to Contracts. We

are waiting on ATP for watermain project.

Requested RFP/CO G-1 is for patching the roadways and parking lots to enable

facility to use the roads through the winter so springtime come we can coordinate

the sequence of watermain and roadway project together for most efficient repairs

and reconstruction of the roadways.

The scope of work provides for replacement of the pool roof drain covers, and also for scuppers to be re-flashed. The drains, scuppers, and thru-roof penetrations shall be extended as well.

3. Maintenance Building; the scope of work provides for the reroofing of the 6,625 square foot roof and new roof

covers.

4. Pavilion Building; the scope of work provides for the reroofing of the 800 square foot asphalt shingle roof with fascia and new drip flashing.

This project was bid in October of 2020 and received Authorization to Proceed in April 2021. In this time frame there have been market increases, stocking delays, and product pricing volatility that has impacted this project. Description of RFP Change: This change order is necessary to provide for material and labor price increases on roofing materials due to market fluctuations and stress to the supply chain and distribution. This is a multi-prime project to replace roofing on 4 buildings and to install pool dehumidification system. This project bid on 10/14/2020 and ATP was received on 4/21/21. During the first pay progress meeting on June 9, 2021, Sean Englemann of Joiner Sheet Metal and Roofing, the general contractor, mentioned delays in getting roofing materials and also alerted the project team to cost escalation information provided by the EPDM and TPO manufacturers. He mentioned that the earliest we would receive the material would be November. Per management direction, at this time, bid escalations were reviewed one at time and would need to wait for an escalation procedure to be put in place. Mr. Englemann stated that he could not give me a price until material shipped. A formal process was provided for this process in August and Joiner processed formal cost escalation when materials were shipped. Roofing materials were delivered at the end of September. The A/E has reviewed the change order. Manufacturer letters indicating the percentage increases are provided as well as specific cost increases for roofing materials, sheet metal, and misc. items as specified. However, it is increasingly apparent that the global supply chain that supports our economy and had to rapidly adjust to declining demand last year, is now struggling to maintain pace with the recovery. This natural supply /demand imbalance has been compounded by backlogs in ports around the world and the February Texas freeze. Articles this week in business publications have highlighted the severe impact on Texas’ petrochemical complex, the largest in the world, that the recent snow and ice has caused, resulting in shutdowns for businesses as diverse as manufacturers of autos, tents, and semiconductor chips. The combined effect of these issues means that the supply of many raw materials that Versico uses in its products has been, or will be, disrupted. Consequently, it is likely that there will be some challenges in meeting your needs this season and that you may experience extended lead times for some items. Versico’s supply chain, production planning, and customer service teams will do everything in their power to minimize this impact and to provide clear and timely communications about your orders.

Along with these supply challenges, the cost of all raw materials has risen with unprecedented speed and severity. It is therefore necessary that Versico reminds you of our recent price increase announcement (attached) that must be enforced as we seek to mitigate the impact of this global supply chain challenge. Further price increases remain a possibility but for now, while supply chains are experiencing this volatility, requests for longer term quotes will include escalation clauses as a hedge against further increases in raw material costs. In the event that costs stabilize, these escalations clauses will not be enforced.

Clearly 2021 is going to present us all with challenges, but we must not lose sight of the fact that they are an unfortunate component of a return to more normal times. Versico will do everything possible to ensure that it smooths out the bumps in the road to recovery and continues to deliver on the Versico Experience that you have come to expect.

RE: Joiner Roofing Projects

To Mr. Englemann,

I am aware you have several material orders with Versico that have delivery dates impacted

negatively due to the raw material supply currently affecting the roofing manufacturing industry.

As material supply tightened at the beginning of the year for the construction industry, not just

roofing, suppliers of raw materials raised prices on the purchasers of their products. In typical

years, these raw material supply increases would occur with adequate notification, allowing

manufacturers such as Versico to provide ample time to our customers of the coming price increase.

Unfortunately for 2021, which has been unlike any other year in terms of raw material supply and

cost in the roofing industry, this has not been the case. Roofing manufacturers have taken on

several increases for raw materials throughout the year, often with no pre-notification, which has

limited our ability to provide notification to the customer of the coming increase. Also, because

these increases have been so drastic, and because of the roofing product lead times caused by the

limited amount of raw material supply, we have had to pass these costs on to materials that were

quoted prior to the increase but are shipping after the increase.

Ultimately, it costs more to roof buildings now than it did six months ago, four months ago, two

months ago, or even two weeks ago (~the time of our last announced increase). The lack of raw

material supply, and the increased costs to construction have been documented throughout the year

by manufacturers, industry organizations, and others. I have attached Versico’s initial letter of

concern about these issues we issued in March of this year. I am also including links to other

industry publications noting the worsening raw material and cost issue throughout the year.

Each level is approximately 55,000 square feet in area. The scope of work for this project includes architectural, structural, plumbing, electrical and ventilating components. This change order relates to the structural scope of work. The structural scope included concrete repairs related to corrosion of embedded steel reinforcement at slab top and bottom surfaces, beams, and columns. The project provides for repairs and installation of embedded galvanic anodes within patches to mitigate corrosion in the near vicinities of patch areas, structural concrete repairs related to corrosion of post-tensioned reinforcement in slabs and beams, and repairs required in the evaluation of existing conditions. During Design CTL Group originally estimated quantities of post tensioning repair based on their evaluation from 2019. On 2/11/2021, Contracts bid officer reached out to me to ask for assistance in getting bid extensions for this project. CAD brought up an escalation request at this time. I made management aware of the request and conversation. I have included emails that pertained to the conversation surrounding the escalation. Per management direction, at this time, bid escalations were reviewed one at time and back up provided prior to an ATP. Direction on this process changed after the escalation discussion of this project. Included in the change order package is the initial conversation which centered around $60,000 in additional costs but delays in getting the contract awarded between February and July accrued additional costs due to market volatility. Description of RFP Change: This project was stopped on June 30, 2015 due to the State budget impasse. Sometime before the work was stopped, the general contractor had submitted RFI #019 due to field observations while preparing a section of parapet wall for rebuilding per the project scope. As reported in RFI #019 - Item #2, the outer wythe of brick immediately below the specified removal area appeared to be de-bonded below the concrete roof deck. Additionally, demolition for the approved in-place mock-up also exposed the spandrel beam below the deck which had visible rust and signs of oxidation that was being addressed so the work could proceed. Since the prolonged duration of the work stoppage, the masonry condition has further degraded due to exposure and moisture damage. It has deteriorated to the point at which there is concern that it no longer provides for a sound supporting base for the rebuilding of the building parapets as originally specified. _________________________________________________________________________________________________Requested Action: We are requesting board approval of Change Order G-08 in the amount of $244,237.12 to address the necessary corrections to the existing parapet walls. _________________________________________________________________________________________________

RFPCO G-08 Attachment #4 Description

At the time that the CSU Renovate Robinson Center project work stoppage order was issued, the General Contractor submitted RFI #019 as a result of field observations from preparing a section of parapet wall for rebuild per the project work scope. As reported in the RFI (Item #2) the outer wythe of brick immediately below the specified removal appeared to be de-bonded below the concrete roof deck. Additionally, demolition for the approved in-place mock-up also exposed the spandrel beam below the deck which had visible rust and signs of oxidation that was also being addressed for the work to proceed. Since the prolonged duration of the work stoppage the masonry condition has further degraded due to exposure and moisture damage to where it is a potential concern for providing a sound supporting base for the rebuilding of the building parapets as originally specified. This change order is being generated to address both concerns so as to allow for the masonry work to start back up and allow the project to proceed.

Outer Wythe De-bonded Brick

1. Remove an additional four (4) course of brick below the original specified level which was to the topof the concrete roof slab (Architectural Sheets A301, A302 & A304).

a. Removal of the additional brick courses should provide for a suitable base to support thenew parapet construction as well as allow for exposure of the top flange of the spandrelbeam and part of the web for observation of the spandrel beams existing condition as thework progresses.

2. As work progresses with demolition and rebuilding of the parapets, when the spandrel beam isexposed if surface rust is observed, then the following steps should be taken:

a. Fully expose top flange of the I-Beam of the area in question topside and underneath (fromthe outside).

b. Wire-brush the beam flange (top & bottom) to clean steel, removing rust residue oncompletion.

c. Brush or roller apply one coat of “Rust-Oleum 769 Damp Proof Primer (rust-inhibitingcoating) per Mfr. Instructions/guidelines to cleaned and prepped flange at top and bottom.Allow to dry overnight.

d. Re-build wall/parapet per specifications and appropriate masonry standards.

3. If more significant damage such as pitting, advanced oxidation and expansion of the spandrel beamis observed, then the beam should be exposed at the localized area and the Architect of recordshould be contacted to evaluate the condition and assess if further structural analysis would berequired. Based on past exposure and observations prior to the work stoppage order, it is believedthat this would likely be an occasional localized occurrence and not a general condition.

a. If it is determined that the structural integrity may be compromised, then a structuralengineer will be brought in to confirm structural integrity of the beam. If more significantrepairs are reported as necessary, an additional change order will be required forengineering calculations and the creation of repair details for contractor repairs to be cariedout.

The Veterans Home (W0700) is a 119,735 square foot, one-story building established in 1991.

The scope of work provides for painting walls and other building components, removing and replacing existing VCT floors, removing and replacing existing handrails and wall guards, removing and replacing or refinishing doors and casework, including nurse stations, providing new door hardware, remodeling congregate showers plus other miscellaneous remodeling. The work also includes replacing two central station air handling units and integrating the controls into the Building Automation System (BAS), retrofitting VAV terminals from pneumatic controls to new DDC controls that are tied into the BAS, and integrating all residential room temperature controls into the BAS as well. The scope will include replacing two gas-fired domestic hot water heaters, providing new point-of-use mixing valves, minor electrical work in support of other work, and other miscellaneous minor repair work.

The A/E will need to determine if any components of this project are eligible for a utility company or other energy grant/rebate and will be responsible for preparing and submitting the grant application if the project qualifies for the rebate.

This work may include, but is not limited to, architectural, civil, structural, mechanical, and electrical services as needed. The A/E will be responsible and must utilize or be a qualified environmental firm for preparing/submitting plans and reports in accordance with Section 669 of IDOT's Standard Specifications for Road and Bridge Construction. The A/E will also be responsible for monitoring all soil excavation, ensuring contractor compliance and measurement for payment in construction.

In the event that hazardous materials are encountered, they will be addressed outside of this contract.

The scope of work provides for replacing the existing shingle roof with a metal standing seam roof and replace accessories as needed. The lights shall be upgraded to LED lighting.

The Flora Salt Storage/Domar (D0734) is a 4,070 square foot, one-story building constructed in 1999. The scope of work provides for repairing or replacing the existing shingle roof on the salt dome and replacing any damaged sheeting and accessories as needed. The lights shall be upgraded to LED lighting.

This work may include, but is not limited to, roofing, structural and electrical.

The A/E will need to determine if any components of this project are eligible for an Illinois Energy Grant/Rebate Program and will be responsible for preparing and submitting the grant application in the project qualifies for the rebate.

The Morton Maintenance Bulk Storage Building (D0449) is a 512 square foot, one-story building constructed in 1990. The Wash Bay (D0485) is a 6,000 square foot, one-story building constructed in 2003. The scope of the work provides for enclosing approximately 2,500 square feet of space between two existing buildings (D0449 and D0485) and constructing a three-bay wash area, including a concrete floor with a water/oil separator, overhead doors, lighting, and HVAC equipment. The work also provides for enclosing the five bays of the Morton Maintenance Bulk Storage Building (D0449), including overhead doors, heating, electrical and lighting.

The Morton Maintenance Storage 1 Building (D0413) is a 9,600 square foot, 2-story building constructed in 1967. The scope of work provides for extending three bays by approximately 10-15 feet and adding an approximately 20'x14' low clearance, overhead door to each bay, upgrading the heating, ventilating, electrical systems, and lighting.

The Morton Maintenance Material Storage 2-Bay (IDOT308-0001) is a 740 square foot, one-story building constructed in 2012. The Morton Maintenance Material Storage 3-Bay (IDOT308-0002) is a 986 square foot, one- story building constructed in 2012. The scope of work provides for constructing a two-sided pole building approximately 1,500 square feet for cold storage, including electrical and lighting. The open-front structure will attach between the existing buildings (IDOT 308-0001 and IDOT 308-0002).

This work may include but is not limited to architectural, civil, structural, mechanical, and electrical services as needed. The A/E will be responsible and must utilize or be a qualified environmental firm for preparing/submitting plans and reports in accordance with Section 669 of IDOT's Standard Specifications for Road and Bridge Construction. The A/E will also be responsible for monitoring all soil excavation, ensuring contractor compliance and measurement for payment in construction.

Portions of the north and south sections of the campus are in a Zoned AE flood classification or floodway fringe, which will need to be considered as part of the design.

The scope of work provides for renovation of the fume hood exhaust system, including removing and replacing approximately 165 existing fume hood exhaust air valves, dampers, controller(s), approximately (80) VAV terminal units and removal of associated stand-alone pneumatic controls. The new replacement DDC control system will need to communicate with the existing University's central building automation software system. The work also includes installation of new sash sensors, fume hood displays on approximately 205 existing fume hood workstations and evaluation of all roof-mounted exhaust fans, existing ductwork, and heat recovery systems, with recommended upgrades, and/or replacement.

PROJECT SCOPE OF WORK:The Stores Building (BC108) is a 65,751 square foot, 3-story building established in 1964. The DixonCorrectional Center is a 96-building facility established in 1916.

The scope of work provides for engineering services to diagnose the cause of the Warehouse (GeneralStores) transformer failure, provide solutions to repair/replace the transformer, and to assist with design of atemporary generator installation to power the building until repairs are made.
