Declaration of Trust

Part 1. Trust Name

This revocable living trust shall be known as the TRUST NAME.

Part 2. Declaration of TrustGRANTOR AND TRUSTEE, called the grantor, declares that he has transerred and delivered to

the trustee all his interest in the !ro!ert" described in Schedule A attached to this

Declaration o Trust. All o that !ro!ert" is called the #trust !ro!ert".# The trustee hereb"acknowledges recei!t o the trust !ro!ert" and agrees to hold the trust !ro!ert" in trust,

B. Amendment or Revocation by Other Person

guardian or other !erson shall not e%ercise it on behal o the grantor, unless the

grantor s!eciicall" grants a !ower to revoke or a$end this trust in a Durable &ower oAttorne".

(n this Declaration o Trust, the ter$ #trustee# includes successor trustees or alternate

successor trustees serving as trustee o this trust. The singular #trustee# also includesthe !lural.

D. uccessor Trustee

U!on the death or inca!acit" o GRANTOR AND TRUSTEE, the trustee o this trust and o an"

children)s subtrusts created b" it shall be SU**ESOR.

An" trustee in oice $a" resign at an" ti$e b" signing a notice o resignation. The

resignation shall be delivered to the !erson or institution who is either na$ed in this

Declaration o Trust, or a!!ointed b" the trustee under Section + o this &art, to ne%tserve as the trustee.

#. Po$er to Appoint uccessor Trustee

successor trustee is willing or able to serve as trustee, the last acting trustee $a"
