Welcome
“An Implant’s Design Dictates its Clinical Capabilities”
The Bicon System was designed not as a research project to study osseointegration, but rather as a means to restore dentition. Bicon’s unique 1.5˚ locking taper implant to abutment connection follows sound bioengineering principles and provides for 360° of universal abutment positioning. Bicon’s implant to abutment connection also has been definitively proven to be bacterially sealed. The implant’s elegant plateaued design, provides for cortical-like bone with central vascular systems around the implant. This cortical-like bone not only grows faster, but also
provides for functionally different capabilities than the appositional bone around non-plateaued implants. The implant’s sloping shoulder provides sufficient space for the interproximal papillae, which are crucial for gingivally aesthetic restorations. Since its introduction in 1985, the Bicon design has benefited from a sensible biological width, which is only now—some 20 years later—being promoted as switching platforms. For these reasons, Bicon clinicians and their patients do not experience the frustrations and limitations inherent in other implant systems.
800-88-BICON B
SHORT IMPLANTSSince 1985 » Bicon Short Implants maximize implant placement possibilities and minimize the need for grafting procedures. With Bicon, longer implant lengths are not necessarily better. For many clinical situations, shorter implants offer a better solution.
EXTRA-ORAL CEMENTATION & THE IAC™Since 1985 » With the elimination of screws, Bicon’s restorative procedures are conventional, requiring only standard impression techniques and allowing for intra-oral or extra-oral cementation techniques. Because of Bicon’s 360˚ of universal abutment positioning, Bicon introduced the revolutionary Integrated Abutment Crown™ (IAC), a screwless and cementless restoration. The IAC affords the dentist a guaranteed aesthetic subgingival crown margin for every restoration, with no extra effort or expense for the dentist or technician.
LOW-SPEED DRILLINGSince 1985 » Low-speed drilling at 50 RPM without irrigation allows a clinician to harvest the patient’s own bone with titanium reamers for autogenous grafting. Slow drilling is forgiving and is unique to Bicon. Additionally, it greatly extends the longevity of the titanium reamers, thus reducing a clinician’s costs.
1mm 10µm 1.5˚ LOCKING TAPERSince 1985 » Bicon’s 1.5 degree locking taper connection provides a proven bacterial seal at the implant to abutment interface, with a microgap of less than 0.5 microns. Bicon’s bacterial seal avoids the microbial leakage issues that can result in inflammation of the soft tissue around an implant, which could lead to not only bone loss around the implant but also to the loss of the implant itself.Image courtesy of Ziedonis Skobe, PhD, Forsyth Institute and Harvard University, Boston, MA and Thomas G.H. PLATEAU DESIGNSince 1985 » The plateau or fin design offers at least 30% more surface area than a screw implant of the same dimensions and allows for the callus formation of mature haversian bone between the fins of the implant. This cortical-like bone forms at a faster rate of 10-50 microns per day in comparison to the appositional bone around non-plateaued implants, which forms at a slower rate of 1-3 microns per day.

PLATFORM SWITCHINGSince 1985 » Bicon features complete interchangeability of abutment diameters, providing sensiblebiological width — now discussed as platform switching. Bicon’s harmonious biological designprovides for impressive bone maintenance.

RESTORATIVE FLEXIBILITYSince 1985 » Bicon offers a complete selection of abutments: temporary, healing, stealth shouldered, non-shouldered, fixed-detachable, Brevis™ overdenture, and Locator® overdenture — providing for exceptional restorative flexibility and platform switching. All Bicon abutments are completely interchangeable, and all benefit from the unique 360˚ of universal positioning provided by Bicon’s locking taper connection. Once clinicians appreciate what 360˚ of abutment positioning can do for their clinical practice, implant dentistry will never again be the same for them.
SLOPING SHOULDERSince 1985 » Bicon’s sloping shoulder affords more flexibility at the time of implant placement and provides for impressive bone maintenance. It also provides more room for bone over the implant, which provides support for the interdental papillae, enabling aesthetic gingival contours to be easily and consistently achieved.

The success of all dental implants is highly susceptible to surgical technique which must be meticulously performed. Careful patient selection and final restorative prostheses appropriate to individual patient needs and remaining anatomy are essential. Selection of individual or multiple implants of appropriate size and configuration for the existing anatomy is also crucial if success is to be expected.
For the above reasons, Bicon strongly recommends that all dentists attend one or more endosseous dental implant courses or symposia covering the use of the Bicon dental implant system before attempting to incorporate implants into their dental practices. Because Bicon should not and cannot control factors within the scope of services performed by the dental practitioner including patient selection, surgical technique and restorative techniques, Bicon assumes no responsibility beyond replacement of product for failures or other unfavorable reactions or results to or from any of its buyers, any dental practitioners or patients arising out of or through use of a Bicon implant. Bicon implants are manufactured from surgical grade titanium alloy (Ti 6Al 4V) and are supplied as hydroxylapatite coated (HA), or Integra-Ti™, and are considered by biomaterial researchers to be highly biocompatible in numerous animal and human studies which have been conducted over a period of many years. However, as with all such coatings, the possibility of chipped surfaces resulting from insertion of the implant into the bone is always a possibility. Some implants may have some minor chips present in the HA coating when received. These surface imperfections are not thought to affect the overall function of the HA coated implant. Furthermore, while many authorities claim that HA coatings are non-resorbable, the
material has not been used for a long enough period of time to establish the validity of this claim. Bicon implants and titanium abutments are exceptionally resistant to bending and fracture. It is not possible, however, to guarantee that fracture or bending of an implant or abutment cannot or will not occur from traumatic injury or excessive occlusal loads. For the foregoing reasons, except as specifically stated herein, Bicon makes no warranties extending beyond the representation contained in this warranty or beyond the description contained in any invoice. This warranty is in lieu of and excludes all other warranties, expressed or implied including merchantability and fitness for a particular purpose. Bicon sells and warrants to buyers who are licensed dental practitioners and who purchase implants and abutments for purposes of their use and resale as part of their services which they perform for patients. Bicon makes no written warranty to any consumer, patient or end user and authorizes no person to make any such written warranty on its behalf. The remedies of any buyer from Bicon (and its liabilities) are limited solely and exclusively to replacement or repair of a defective Bicon implant or abutment when that product has been demonstrated to Bicon authorities within six (6) months of delivery to the buyer to have been defective at the time of shipment and is returned to Bicon for inspection, replacement, or repair. Bicon is not liable to any person for any incidental, consequential or other damages of any kind whatsoever, whether any claim or potential claim is based upon theories of contract, negligence, or tort.

➤ Implant Selection Considerations ■ All implants are packaged with a healing plug.
■ The choice of implant length and width depends upon available bone and expected occlusal loads.
■ The Bicon implant is designed, for an aesthetic emergence profile, to be placed below the crest of the alveolar ridge to the extent that the implant is 5.0mm below the facial crest of the gingiva.
■ The implant may be placed at or slightly above the alveolar crest, if an aesthetic emergence profile is not a concern.
■ In general, choose the widest but not necessarily the longest implant the bone will allow.
■ Place one implant per tooth, posteriorly to the canine, if practical.
■ Use HA coated implants for poor bone quality or grafted bone cases.
■ See reference pages 85-86 for depth indicators on the 2.0mm pilot drill and the latch reamers.

➤ Impression Components and Technique Considerations
■ Making an Implant Level Impression: The use of an Impression Reamer (see page 62) on a guide pin may be necessary to widen the site for the insertion of the impression post. The impression post must be tapped into the well of the implant to ensure that it is fully seated. Once the impression post is seated, an impression is made. After recording the impression, remove the impression post from the implant (the impression post should not come out with the impression). Assemble the impression post and the implant analog, and then seat the unit into the impression. Before pouring impression in dental stone, pour a soft tissue model.
■ The use of a metal or plastic implant analog is at the discretion of the technician. If it is anticipated that the implant analog will be used in conjunction with any soldering techniques, a metal implant analog must be used.
■ When using a metal implant analog, the titanium abutment must be tapped into the analog to ensure proper axial positioning.

➤ Temporary Abutment Considerations
■ Temporary abutments are made of surgical grade titanium alloy Ti6Al4V.
■ Temporary abutments are designed to form a gingival sulcus and to preserve papillae during the one stage surgical procedure or after a two stage uncovering procedure.
■ Choose an abutment height that is sufficient to laterally support the interproximal papillae without its being too tall such that it would receive undesirable forces from the tongue.
■ The actual diameter of the temporary abutment accounts for the final abutment’s diameter with spacing for an emergence cuff. The height is measured from the top of the implant to the top of the temporary abutment.
■ Actual diameters of the temporary abutments are 4.0, 5.5, 6.5, and 7.2mm for the 3.5, 4.0, 5.0, and 6.5mm abutments respectively.
■ The abutment height is measured from the top of the implant to the top of the abutment when the abutment is fully seated.
■ Temporary abutments may be modified with a #1557 carbide bur or other carbide burs.

■ Healing abutments are polycarbonate and are indicated for cases in which an abutment is needed to preserve the papillae for 180 days or less.
■ Healing abutments may be modified to meet a desired clinical shape.
■ The indicated diameter of a healing abutment is measured at its widest diameter. Its height is measured from the occlusal end of the post to the highest occlusal portion of the healing abutment.
■ If a healing abutment post were to fracture, removal is facilitated with the use of a round carbide bur.

➤ Non-Shouldered Abutment Considerations
■ The abutment can be rotated 360° to reach a desired position or to achieve parallelism prior to being seated.
■ The 3.5mm abutments are recommended for mandibular incisors only; 4.0mm abutments are primarily used for maxillary laterals and bicuspids; 5.0mm abutments are more universal in their use; 6.5mm and 7.5mm abutments are ideally suited for molars.
■ The diameter of each Emergence Cuff (see page 22) corresponds to the diameter of a non-shouldered abutment.
■ Do not make an impression with the emergence cuff.
■ Choose the widest abutment and sleeve (see page 21) that will support the interproximal papillae without encroaching upon them.

➤ Non-Shouldered Prosthetic Components Considerations
■ The impression sleeves and transfer dies are color coded according to the corresponding diameter of the non-shouldered abutment: 3.5mm = blue, 4.0mm = red, 5.0mm = yellow, 6.5mm = green, and 7.5mm = purple.
■ The sleeves may be used for impression making, temporization, corrective copings, and/or as waxing sleeves.
■ The temporization sleeves may be used in lieu of the emergence cuffs.
■ Always confirm the complete seating of snap-on acrylic sleeves with either the non-shouldered abutment or colored abutment transfer die.
■ It is paramount that the proper abutment transfer die height be chosen for pouring of a stone model, since all transfer dies of the same color will snap into an impression sleeve of that color.
■ The diameter and height of the transfer dies are sized to match the diameter and height of a corresponding non-shouldered abutment.
■ Although possible, avoid making an impression using the tooth colored temporization sleeves, since they are not color coded with their corresponding transfer dies and they are more retentive.
■ Sleeves may be easily modified to suit a particular clinical need.
■ Current versions of the non-shouldered abutment has a groove approximately 3.0mm above its height of contour providing retention for the new snap-on impression and temporization sleeves.
■ The snap-on impression and temporization sleeves will not necessarily fit precisely on a non-shouldered abutment without the groove.

➤ Stealth Shouldered Abutment Considerations
■ Although the stealth shouldered abutment is designed for all-ceramic restorations, it may be used in conjunction with metal castings.
■ Use the shoulder depth gauge to determine an appropriate abutment shoulder height, relative to the gingival tissues. The first mark on the shoulder depth gauge corresponds to a 2.0mm shoulder height of a 5.0mm stealth shouldered abutment. The second mark corresponds to a 4.0mm shoulder height, etc. If the soft tissue falls in between two marks, select the shorter of the two abutment heights for an aesthetic result.
■ The diameter of an abutment is dictated by the interproximal papillae. The abutment should support the papillae without encroaching upon them.
■ The stealth acrylic sleeve may be used for making an impression, for creating a temporary crown, for recording an occlusal registration, and also as a waxing sleeve or a corrective coping.
■ When using the one-piece sleeve as an impression transfer, it is essential to confirm that an internal flat of the sleeve has been aligned with an external flat on the abutment.
■ Prior to the intraorally cementing of a crown on a shouldered abutment, a radiograph must be taken to ensure that the crown is fully seated.
■ The aluminium oxide sleeve must be ground wet either using a sintered diamond disk or a diamond wheel on a model trimmer.
■ Only aluminous porcelains may be used in conjunction with the aluminum oxide sleeve.* (n.b., metal-ceramic type porcelains and pressable ceramics are NOT compatible with Bicon’s aluminum oxide sleeve.)
■ A 3.5mm stealth shouldered abutment crown MUST be extra-orally cemented with DiamondLink™ by DRM or another comparable cement.

➤ Locator® Abutment Considerations
■ The Locator® Attachment features a denture component with a skirt that easily locates the mating implant abutment.
■ The self-aligning ability of the attachment aids the patient in positioning their prosthesis in similar manner as a guide plane created by a milled bar.
■ The implant retained overdenture can be properly seated without damage to the attachment components. This is especially important for a patient lacking anatomical structures necessary to orient their denture due to a severely resorbed mandibular ridge.

Abutment Measurement GuideThe design of the Bicon abutment system is such that the hemispherical base of the abutment does not sit flush with the neck of the implant. By design, there is a radiolucency below the abutment post when the abutment is fully seated. Please note that the post of any abutment may not be modified. Altering the abutment post in any manner or
using a cementing medium will affect the frictional fit of the locking taper resulting in a potentially non-retentive abutment. The following diagrams depict the final seating of each abutment style as well as the method for measuring each abutment.

➤ SynthoGraft™ is a synthetic, biocompatible and resorbable granulate ceramic made of pure phase Beta-Tricalcium Phosphate Ca3(PO4)2 which is used as a matrix for oral and maxillofacial bone regeneration or augmentation.
➤ While several varieties of βTCP are now commercially available, they are not all equal in bone regenerating capability. These differences can profoundly affect the rate and quality of bone regeneration, as well as the rate of resorption of the graft material as it is being replaced by the patients’ own bone during the healing process.
➤ SynthoGraft’s™ unique structure provides increased stability and its micro-porosity and nano-porosity allow for rapid vascularization and subsequent resorption. SynthoGraft™ achieves the greatest efficacy when mixed with the patient’s own blood. Other additives, including autogenous bone particles, are not needed or recommended.

➤ Handpiece Considerations
■ The 400:1 handpiece has sufficient torque to cut through bone at 50 RPM.
■ The 18:1 handpiece is designed to be used with the pilot drill at ~1,100 RPM with external irrigation.
■ Irrigation is not necessary at speeds of 50 RPM or lower.
■ Failure to observe speed limitation may result in burned or overheated bone which could result in substantial bone necrosis.
■ The 20,000 RPM Air Motor attaches to conventional four hole dental tubing and is an alternative to the electric drill unit.
■ Air pressure of at least 80 PSI is recommended to drive the air motor with sufficient torque in dense bone.
■ Proper maintenance of handpieces is crucial for long term success of handpieces.
■ Do not exceed 132°C (270°F) when sterilizing.
■ In order to prevent discoloration and/or damage to the plating of the handpieces from chemicals that are not sufficiently cleaned from other instruments, do not autoclave the handpieces with other instruments.
■ Always check the handpiece for any abnormal vibration, heating, noise, or sluggish operation. If any abnormality is noticed, cease the use of the handpiece.

30,000 30,000 1,875 1,666 469 117 75 30
To determine bur or Reamer Speed, you must know the Motor Speed and Divide that number by the Gear Reduction of the contra-angle.
Motor Speed ÷ Gear Reduction = Reamer Speed (RPM)e.g. 20,000 ÷ 400:1 = 50 RPM
Handpiece Maintenance
Cleaning and Lubrication➤ Clean and lubricate the contra-angle handpiece after each use.
■ Attach the metal spray nozzle into the back of the handpiece and insert the pana spray into the metal spray nozzle. (Figure 1)
■ Spray for approximately 2 seconds
■ Disassemble the head from the handpiece using the supplied wrench. (Figure 2)
■ Spray into the head. (Figure 3)
■ Assemble the head to the handpiece sheath in the reverse order of disassembly. Make sure that the two keys at the union nut align with the slots in the sheath and tighten the union nut.
■ Wipe the assembled handpiece clean.
Figure 1 Figure 2 Figure 3
Sterilization➤ The 400:1 and 18:1 handpieces are autoclavable.
■ Wipe any debris off with an alcohol-soaked cloth.
■ Lubricate the handpiece using spray lubricant. Insert the handpiece into a sterilizing pouch and seal it.
■ Autoclave for 15 minutes at 132° C (270° F). Do not set the sterilizer temperature above 132° C (270° F).
■ To expel excess oil, operate the handpiece before using it intra-orally.

➤ Transitional Implant Considerations
Procedure:
■ Drill a 2.0mm osteotomy to appropriate depth with Bicon’s 2.0mmpilot drill with external irrigation.
■ Take care not to overheat the bone while drilling at ~1,100 RPM.
■ Pilot bur is marked at 6.0mm, 8.0mm, 11.0mm and 14.0mm.*
■ Be sure to drill to appropriate depth of intended implant to be placed or slightly beyond if the anatomy permits.
■ Make note of the quality of bone.
■ The Stealth and O-Ring implants are available in 8.0mm and 12.0mmlengths and either straight or angled.
■ Check angulation or trajectory of osteotomy with a paralleling pin.
■ Remove appropriate transitional implant from packaging and insert apical end of implant into osteotomy.
■ Assemble appropriate transitional implant driver to Bicon’s 400:1handpiece or a comparable slow speed latch-type handpiece.
■ Slowly screw implant into the prepared osteotomy.
■* Some pilot burs may have markings only at 8.0mm, 11.0mm, and 14.0mm. Check burs
before drilling. (See page 83)

Non-Shouldered Abutments and Prosthetic Components
Notes: Snap-on sleeves are only specific for abutment diameter. Abutment height is not a criterion for proper selection of snap-on sleeves.Transfer dies correspond to exact diameter and height of abutment placed.
Indirect Abutment Level Impression
Modified and unmodified color-coded impression sleeves are definitively seated on their
corresponding abutments.
Impression material is injected around the impression sleeves for the making of an abutment
level transfer impression.
Acrylic impression sleeves withdrawn in impression material prior to impression being sent to
the laboratory.
Soft tissue material being injected around impression sleeves and
abutment transfer dies.

Latch Reamer WarningThe reamers are used sequentially beginning with a 2.5mm diameter and ending with the diameter of the intended implant. The latch reamers have horizontal markings at 6.0, 8.0, 11.0 and 14.0mm, whereas older reamers did not have the 6.0mm mark. It is imperative that the depth indicators on the latch reamers are identified prior to surgery. No assumptions should be made about the height of the first marking on any latch reamer.
