All about the dreaded 2WW in IVF

Life is not easy for IVF patients, with a number of complicated steps being part and parcel of theprocedure. A typical IVF protocol begins with ovarian stimulation, and is followed by egg retrieval, embryo culture and finally embryo transfer. Once the embryo transfer is done, the physical part of IVF treatment is deemed to be over, but it is not the end. In fact, now starts the emotionally toughest part of the journey, the dreaded 2ww that decides the fate of the treatment

2ww or two week wait is the period of time between the embryo transfer and confirmation of pregnancy, which happens to be the most nerve-racking phase for the patients. This phase is like a roller-coaster ride between hope and despair, as the patients contemplate whether all their efforts would pay and if they fail, what could have been the reason. Every single sensation, every single twinge in the body seems like a big event, with the patient taking it as a symptom of pregnancy (or a failed IVF for that matter). Numerous questions can give stressful days and sleepless nights,as the patient would want to go to any extent to ensure IVF success.

What physical changes can a woman experience during 2ww?

The 2ww can seem like eternity as stress runs high, with thewoman undergoing great deal of anxiety over the fate of IVF.At the same time, she may experience a lot of physical changes too, with symptoms that mimic an upcoming period. Cramping, abdominal bloating, breast tenderness, spotting and fatigue may be some of these, but there is no need to be alarmed by these. These symptoms do not imply a pregnancy or otherwise. On the other hand, medical intervention is needed in case of severe symptoms such as excessive bloating, lowerabdominal pain and shortness of breath, as these may indicate ovarian hyper stimulation

Can the patient go on with her normal daily routine during 2ww?

Patients are advised to take it easy during the first few daysafter embryo transfer, though there is no such evidence to prove that bed rest enhances the chances of success of IVF. The necessity of rest during 2ww is a subjective matter and thedoctor will recommend the same only if needed, otherwise the patient can continue with the normal day-to-day activities during this time period. A woman can also travel safely during 2ww, though it is advisable to be within proximity of medical assistance in case a need arises. However, it is a goodidea to refrain from strenuous physical activity as it can impair embryo implantation by causing uterine contractions.

Is it safe to have sex during 2ww?

The common belief is to practice abstinence from sex duringthe entire IVF cycle, including the 2ww because it may hamper embryo implantation or cause infection. However, thereis no scientific evidence to back this notion. In fact, latest studies have shown sexual intercourse actually supports embryo implantation and development. This issue is again a subjective one, as the doctor may ask a couple to refrain from sexual activity depending upon their individual circumstances.

Does stress during the 2ww reduce the chances of IVF success?

Stress has always been regarded as a culprit because of its negative impact on female fertility. Recent studies have however established that there is no need to be worried about it as stress levels have nothing to do with the success rates of IVF. Still, women need to stay positive during the 2ww and they can see support from fellow patients and counseling from the infertility clinic to tackle this issue positively.

Is medication required during 2ww?

Patients need extra care and support during 2ww so that all their efforts yield desirable results. Therapeutic support goes a long way in this regard, with progesterone and/or estrogen supplements prescribed as a part of the protocol. While progesteronesupplements keep the hormone levels steady to facilitate pregnancy, estrogen supplements are effective for thickening the lining of the uterus so that implantation can take place.

Is spotting during 2ww a bad sign?

Spotting or light bleeding during 2ww may be seen as an ominous sign, but it is relatively common to experience the same after embryo transfer, while a woman waits for confirmation of pregnancy. It can actually be implantation bleeding, which occurs when the embryo implant itself in the uterine wall(implantation bleeding is a common phenomena in unassisted pregnancy too). However, such an incident should not be ignored as it may be a sign of miscarriage too; therefore, it isadvisable to see the specialist if something like this happensduring 2ww.

How soon can a pregnancy test be taken after 2ww?

Pregnancy takes place when the embryo gets implanted in the uterine lining and it is indicated by an increased level of a hormone called hCG (human chorionic gonadotropin) in the blood stream. A blood test is used to confirm pregnancy, but an IVF patient should not lose patience and go for it too early. The reason isthat the results may be distorted because of the hCG trigger shot which is given as a part of the IVF protocol; a false positive cancome up because of the trigger hCG in the bloodstream. Tempting as it may sound; a woman should not go for a pregnancy testuntil before 10 days have gone after the trigger shot. The test should be done at the clinic only and not at home, after the 2ww following the embryo transfer has been completed.
