This letter forwards the Inservice Inspection (ISI) Ninety-Day report on ASME Class 1and 2 components examined during the Eleventh refueling outage (2R1 1) at BeaverValley Power Station (BVPS) Unit 2. The ASME Boiler and Pressure Vessel Code,Section Xl, Article IWA-6230 requires that an inservice inspection report be submittedto the enforcement and regulatory authority having jurisdiction at the plant site withinninety days after completion of an inservice inspection. The recent inservice inspectionterm at BVPS Unit 2 ended on April 27, 2005.

The Class I and 2 examinations are part of Interval 2, Period 2 of the Unit 2 ISIschedule, and were performed in accordance with Chapter XI, Section IWA-6230 of theASME Boiler and Pressure Vessel Code, 1989 Edition.

OUTAGE SUMMARY

During the Eleventh Refueling Outage (2R1 1) at the Beaver Valley Power Station,Unit 2, Inservice Inspection (ISI) examinations were performed on Class 1 and 2. This was thethird outage in the second period of the second ten year interval. Also included in this report andthe counts below are examinations performed prior to 2Rl I during plant operation. Theexaminations were based on ASME Section XI, 1989 Edition.

ASME XI Class 1 and 2 Credited Exams (See Appendix I)

1. Thirty-one (31) Class 1 exams were performed and are divided as follows:

a. Pipe WeldsUltransonic Exam - 1Visual Exams - 2

b. Reactor Vessel Interior SurfacesVisual Exam - 1

c. Steam Generator Inner Radius SectionVisual Exams - 2

d. Reactor Coolant PumpUltasonic Exams (bolting) - 24Visual Exam (flange) - 1

2. Thirty-four (34) Class 2 exams were performed and are divided as follows:

a. Welds (vessels and piping)Ultrasonic Exams - 9Penetrant Exams - 2Visual Exams - 23

Examinations were performed by FirstEnergy Nuclear Operating Company (FENOC) andcontracted NDE Technicians. Appendix I compiles the examinations that have been creditedtoward fulfilling the Ten Year Plan requirements.

Pressure Testing

The Class 1 piping System Leakage Test was performed prior to plant start-up. All Class 1bolted connections subject to the examination requirements of IWA-5242, "InsulatedComponents" and Code Case N-533-1, "Alternate requirements for VT-2 Visual Examination ofClass 1, 2 and 3 Insulated Pressure Retaining Bolted Connections" were examined during 2R1 1.Also, Class 2 and 3 system functional and system inservice tests were performed on varioussystems to fulfill the current 40-month pressure testing requirement.

Deficiency Resolution

There were no ISI non-destructive examination deficiencies in 2R1 1.

Steam Generator Tube Examination

One hundred percent of the in-service tubes were examined in the three steam generators.Results of the examinations are submitted to regulatory authorities in accordance with TechnicalSpecification requirements.

NIS-2 Forms

Included as Appendix H are the NIS-2 Forms associated with repairs and replacements. CodeCase N-416, Alternative Pressure Test Requirement for Welded Repairs or Installation ofReplacement Items by Welding, Class 1, 2 and 3, was used. The revision of Record at BVPS forCode Case N-416 is currently Revision 2. However, some of the attached NIS-2 Forms had beeninitiated and/or completed prior to implementation of Revision 2. The NIS-2 specifies whenN-416 was implemented and the specific revision used.
