Heart disease is one of the leading killers for residents in Louisiana; however, our community can count on the resources at Terrebonne General Medical Center (TGMC) for the best cardiac care and physicians at Cardiovascular Institute of the South (CIS). “TGMC is committed to providing the most updated technology and continues to offer the best care and services to our cardiac patients,” said Phyllis Peoples, President and CEO. “With so many people in our community affected by heart disease, we have to lead the effort in this crusade to treat this disease,” she added. TGMC is in the process of improving all three cardiac catheterization suites to Toshiba Infinix Dpi Digital Catheterization Lab systems. With thistechnology, TGMC will continue to provide a multitude of benefits to those receiving care in its highly

acclaimed cardiovascular catheterization lab. Increased safety and better patient outcomes are a few of the benefits patients can expect with the improvements. According to Craig Walker, MD, interventional cardiologist at TGMC, and founder, president and medical director of CIS, this equipment not only reduces patient exposure to radiation by half, but also decreases the amount of dye used which can cause a variety of adverse effects. Additionally, studies can now be performed on a patient weighing up to 450 pounds. Most importantly, the equipment provides better image quality for diagnosis and treatment. “Enhanced imaging means we are able to ‘see’ our patient cases better,” said Dr. Walker. “The more we can see, the better decisions we can make, which results in best outcomes.” The state-of-the-art equipment offers digital flat plate technology, which is the very latest in cardiac catheterization (cath) labs. The cath lab system is less cumber- some and has a smaller footprint which allows more patient access during procedures. The system makes imaging of small details very visible. High definition flat panel detectors provide contrast and dynamic resolution allowing for easy visualization of intricate details helping to improve patient care during pro- cedures.

“Imaging technology has improved vastly, not only does this equipment emit lower radiation doses, but images are processed more rapidly and efficiently to provide a quicker diagnosis,” explained Peter Fail, MD, interventional cardiologist at TGMC and CIS. “This is especially important for use in our Evalve Mitral Clip clinical research project in which we are one of 37 centers in the world who are participants.” The Toshiba technology features advanced imaging processing technology to automatically optimize imaging during demanding interventional procedures and provides uniform images. “This system facilitates excellent outcomes while reducing x-ray exposure, thereby improving patient and physician’s safety,” said Richard Abben, MD, interven-tional cardiologist at TGMC and CIS. “This concept is particularly important when applied to intricate device implantation and catheter ablation procedures,” he added. TGMC’s cath lab is equipped to provide a wide variety of services and procedures, and has been recognized by both The Wall Street Journal and the Healthcare Data Source as providing safe and effective services. TGMC was the only hospital in Louisiana and one of 32 hospitals in the United States to receive the Premier Award for Quality. Treatments performed in the Cardiac Catheterization lab include balloon and laser angioplasty, stent place-ment, corroded artery repair and the latest technolo-gies in the transmyocardial revascularization technique performed with the Eclipse Laser System. Also, treatment for peripheral arterial disease (PAD) with a concentration on limb salvage and amputation prevention techniques, Critical Limb Ischemia (CLI), and deep vein thrombosis (DVT) procedures are performed. In addition, the TGMC Cath Lab and CIS physicians

participate in many clinical research trials such as the Evalve Mitral Clip clinical research study. Recently, CIS hosted the largest peripheral arterial disease (PAD) medical conference and 4th largest cardiovascular conference in the nation in New Orleans. Live cases from the TGMC Cardiac Catherization Lab were performed by Dr. Walker, Dr. Fail and Dr. Abben, and broadcasted live via satellite to the conference where world-renowned physicians narrated and discussed the cases. The TGMC cardiac team has been recognized nationally for its innovation in the field of cardiology. We are working hard to bring the best care possible to the people of Southeast Louisiana. Through the efforts of this dedicated group of cardiologists, specially trained cath Lab health care professionals, critical care nurses, telemetry unit nurses and other allied health professionals, TGMC is on the forefront of advanced cardiac care. Baker, whose own background ironically includes being a Hollywood trainer. “The Smartrainer counts reps for you, gives you performance scores, whistles at you when you’re taking too long between sets and even sends you emails when you don’t show up for a while. It offers programming from the top minds in the fitness industry at a level of precision and accuracy impos-sible to emulate by any human, no matter how skilled or educated.” And, if a member’s doctor has advised against performing certain exercises, the Smartrainer will even pull the exercise out of their program if the member elects to skip it more than twice. The most common diagnosis that we treat at Adaptive Prosthetics and Orthotics is foot drop. Foot drop is really a symptom and is secondary to a number of other diagnoses including stroke, multiple sclerosis, cerebral palsy, ALS, spinal cord injury, traumatic brain injury, and peripheral nerve injury. The peripheral nerve injury can be as innocent as crossing one’s legs for long periods of time. Not treating foot drop can result in a poor gait at best and at worst further injury by falls. Foot drop is typically treated with some type of Ankle Foot Orthosis (AFO) or commonly called a brace. Most people have the picture of a brace in their mind of metal bars attached to a shoe similar to the brace seen in the movie Forest Gump. There seems to be a stigma with wearing metal braces that probably started with the polio epidemic. The stigma of a metal brace keeps many people from seeking proper treatment. Although the double upright metal AFO still has its clinical application, there are many more options in AFO design today. The Orthotic industry has been using thermoplastics for many years to fabricate AFO’s. Plastics offer a more intimate fit than metal braces, give the orthotist more control over alignment of the lower limb, and allow for wearing different shoes. Plastics are also very modifiable when there is a problem with fit. Carbon graphite AFO’s are very popular today because they are light, strong, and fit into shoes very easily. Most carbon AFO’s are not easily modified but newer composites are coming onto the market that can be modified. Sometimes AFO designs are reinvent-ed like the Arizona AFO that combines the thermoplas-tic with a leather ankle gauntlet. The latest treatment for foot drop and by far the most revolutionary, is the WalkAide device. The WalkAide is not actually a brace but a new therapy classified as a Myo-Orthotics. The WalkAide uses functional electrical

stimulation (FES) to correctfoot drop. FES is defined as a method of applying low level electrical current directly to the motor nerves of the body to restore function. The WalkAide uses two electrodes attached to the surface of the skin to stimulate the peroneal nerve subsequently activating the tibialis anterior muscle that is responsible for lifting the foot. The WalkAide has many advantages over conventional bracing and other FES systems. The WalkAide uniquely uses a tilt sensor in the device to sense different phases of gait. Therefore, nothing is placed in the shoe as with bracing or other FES systems. The benefits of the WalkAide are greater sensory feedback, increased strength and endurance, maintained range of motion, reduced muscle atrophy, reduced tone and more. In order to be fit for a WalkAide or any other brace, a prescription from a medical doctor is needed. At Adaptive we have a demo unit that can be fit and programmed to the user before purchasing at no charge. Not all foot drop patients are candidates for the WalkAide. Only upper motor neuron and high spinal cord injury diagnoses benefit from the WalkAide because an intact nerve is needed for the device to work properly.
