Reach the 3 Million Find, Treat, Cure TB” BASIC CONTENTS OF RNTCP WEBSITE OF THE DIRECTORATE OF HEALTH SERVICES, ODISHA About us The National Tuberculosis Control Programme (NTP) was implemented in Orissa in the year 1964 with the objective to provide free and domiciliary treatment to the patients detected through passive case finding method. The NTP did not achieve the desired success as a result the programme was revised with the DOTS (Directly Observed Treatment Short Course Chemotherapy) strategy and implemented in our State in the year 1997 with the objective to detect 70% of infectious sputum positive TB cases and cure at least 85% of them. The programme is implemented as per the Technical, Operational and Financial guidelines of Revised National TB Control Programme (RNTCP). The RNTCP was assisted by Danida through Government of India from 1996 to 2005. The Global Fund to fight AIDS, TB and Malaria (GFATM) provides financial and technical support to implement the programme in the State from the year 2006 to 2010 through Government of India. The Rolling Continuation Channel (RCC) project of GFATM has been extended till the year 2017. The Government of India releases necessary funds to the Director of Health Services, Orissa through the Orissa State Health & Family Welfare Society on the basis of the Statement of Expenditure, Audit report and Annual Action Plan. Govt. of India also provides 100% requirement of anti TB drugs for treatment under RNTCP. The Lepra-India, an international NGO, is also providing support under “Axshaya” project to IEC-BCC activities in 12 districts of Orissa (Angul, Bhubaneswar, Cuttack, Gajapati, Ganjam, Koraput, Kandhamal, Malkangiri, Mayurbhanj, Puri, Sambalpur and Sundargarh) The Catholic Bishop Conference of India (CBCI) and the Indian Medical Association (IMA) are also involved in RNTCP of the State. About us

Programmes / Schemes

Present Status

Since inception of RNTCP in 1997 till 2014, total 6,03,103 TB cases have been detected and treated. Total 4,71,687 cases have been cured & successfully completed treatment and 28,062 cases reported died. Out of the said reported death cases all deaths may not be due to TB. It includes death due to other causes also. In the year 2014, the annualized new sputum positive case detection rate of Orissa was 55% against the norm of 70% and the success rate 86% as against the norm of 85%.

The programme is implemented though 31 implementing units, 159 TB Units and 549 Microscopy centers.

48,993 DOT Centres are functioning under the programme to provide DOTS to the registered TB patients.

The programme has the following major activities. o DOTS for CAT-I & CAT-II patients o PMDT (Programmatic Management of Drug Resistant TB) for CAT-IV & CAT-V

patients (for MDR & XDR TB patients respectively) o Intensified TB-HIV activities to ensure 100% testing for HIV of all diagnosed

TB cases & treatment of co-infected patients. o Entry of data in NIKSHAY portal of RNTCP of all diagnosed TB patients. o Notification of all diagnosed and treated TB patients is mandatory for all

Laboratories, Private Practitioner and Hospitals. o Medical college involvement – Presently 6 out of 7 including AIIMS

Bhubaneswar are involved in the program for diagnosis and treatment of TB patients in the Medical colleges

The Anti TB Demonstration & Training Centre (ATD&TC) Cuttack is functioning as a training center of RNTCP and conducts External Quality Assessment (EQA) to maintain good quality sputum microscopy.

Intermediate Reference laboratory (IRL) at Anti TB Demonstration & Training Centre, Cuttack accredited on 10th Aug’ 2009 conducting solid culture, Liquid Culture for Culture & DST of M Tb & Line Probe Assay (LPA).

It is matter of pride for the State that Regional Medical Research Centre, Bhubaneswar has been declared as a new National Reference laboratory for the Eastern and North Eastern region of India. It has been accredited for culture and Drug Susceptibility Test for solid culture, supporting for follow up cultures from 7 districts (Koraput, Nabarangpur, Kalahandi, Rayagada, Gajapati, Kandhamal & Malkangiri). NRL RMRC Bhubaneswar is also supporting Diagnostic samples of MDR suspects from districts of Nabarangapur, Kalahandi, Rayagada, Gajapati, Malakngiri .

PMDT (Programmatic Management of Drug Resistant TB Cases) rolled out in the entire state. Till 3Q14, out of the sputum samples of 5499 Drug Resistant TB (DR-TB) suspects examined, 700 patients have been diagnosed as Drug Registrant TB and out of them 613 patients (88%) have been initiated on treatment since the year 2009 till 2014. In addition 21 nos. of XDR TB patients have been detected under RNTCP. All drugs and diagnostics are provided free of cost.

Diagnosed DR-TB patients can now be admitted in the existing DR TB wards at SCB, VSS & MKCG Medical College.

TB and HIV collaborative services have been strengthened in all districts with the state progressively increasing the proportion of TB patients tested for HIV. The TB HIV Intensive Package is implemented in all the districts since 2010 in collaboration with the OSACS. During the year 2014 out of total 45777 registered TB patients, 31330 (68.4%) are examined and 628 found TB-HIV co-infected.

To further improve TB surveillance in both Private and Public sectors in India, NIKSHAY – a real time, online notification, recording and reporting system has been implemented in the state with most of the districts entering data for all the patients registered in 2013. Till date 72,541 patients from Govt. Sector have been entered in the Nikshay portal. Block level MIS-Coordinators along with STS play an important role to ensure 100% entry in this portal.

TB Notification which has been made mandatory as per GoI notification has been initiated and reporting of TB patients from private Sector started. Our partner Indian Medical Association (IMA), Odisha is fully supporting our efforts. Till date (4Q14) 1111 TB patients had been notified and registered from Private sector in NIKSHAY.

The State Task Force (STF) mechanism for the involvement of all Medical Colleges in

RNTCP is operational, headed by DMET as STF Chairperson. As such three Govt

Medical Colleges of our State and SUM Medical College & Hospital Bhubaneswar

have received one LED Microscope each for diagnosis of TB cases.

The State has organized National Task Force (NTF) Workshop in the month of March 2014 and Zonal Task Force (ZTF) meeting on 15-16 January 2015 in Bhubaneswar for active involvement of Medical colleges in RNTCP.

State has optimally rolled out Tribal action plan in 11 notified tribal districts catering to 22.47% of tribal population of the State.

School awareness programmes, Patient Provider interaction meeting, PRI

Sensitization, Street Play & Folk Dance are conducted at district level to create

awareness for early diagnosis and treatment of TB.

In addition Swasthya Kantha in all villages is being utilized to disseminate TB

messages to the community

The programme is also supported by Partners like Axshaya GF-TB Project in 12

districts, Indian Medical Association and Catholic Bishop Conference of India (CBCI)

and Red Cross India.

bacillus by Robert Koch, a German Scientist, in the year 1882. This discovery led to inventions of appropriate medications to fight TB.

Indian Medical Association (IMA) has made TB notification mandatory and published the said notification in a separate Resolution.

The 4th DR TB Centre has been established at DHH Koraput to diagnose DR TB using

CBNAAT (Cartridge Based Nucleic Acid Amplification Test) technique. Archive Section
