A 70-year-old paent had basal cell carcinoma on right cheek diagnosed by biopsy and managed in 2009 by large resecon of the tumor with marginal deep limit. An adjuvant radiotherapy was indicated; but the paent refused any addional treatment. Three years later, the paent returned with deep-seated and superficial disease in the same region with submandibular lymphadenopathy. Aſter posive biopsy and negave lymphadenectomy, the paent sll refusing radiotherapy and was lost to follow-up. Two years later, the paent returned with a huge ulcerated and bleeding mass of the right cheek infiltrang oral cavity, floor of the orbit and obstrucng both nasal cavity (Figure 1). Sixty six Gy in 33 fracons were prescribed using RapidArc radiotherapy. The second image shows the immediate complete response to irradiaon therapy (Figure 2). Unfortunately, the lesion recurred a year later and actually the paent is candidate to reirradiaon protocol.
