All in-person programs will have enhanced essential eligibility requirements in order to comply with stateand federal COVID-19 regulations. Along with being mandated requirements, these measures are necessary to keep all participants and staffmembers safe. We understand these measures may be difficult for some of our participants. We understand that our initial in-person offerings will only serve a percentage of all who want to return toprograms. We currently have limited facility space available to us in which to hold programs. We fully intend to expandofferings as facility space becomes more readily available. We intend to ease restrictions as governmental regulations change based on state progress through theGovernor’s plan.

WDSRA is excited to offer a graduated return to in-person programming. This becomes possible as Illinoisprogresses into Phase 4 of the Restore Illinois Plan.

This guide serves as our starting point. Because the safety of participants and staff is our number oneresponsibility:

Please read through the eligibility guidelines carefully. This will be your guide to knowing whether or not yourson/daughter is able to participate. Should you have any questions, please call Ted Adatto at 630-681-0962.

We are thrilled to be able to start the process of returning to in-person programs. We will continue to worktoward safely serving all who wish to return to programs. Remember, we will continue to offer virtual optionsfor those not ready to make the jump back.

Pick-up/drop off procedures - Prior to the start of the

program you will receive an email with details of pick-up

and drop off expectations. This will include

parents/guardians remaining in the car while staff

welcome the participant.

Increased hand washing - Participants and staff will be

asked to wash or sanitize their hands upon entry to the

program room or area and also throughout the day at

transitions or specific intervals.

Face Coverings - Both WDSRA staff and participants will

be required to wear face coverings when 6 ft of distance

is not possible and/or during indoor programming.

Equipment/Supply Use -To reduce the sharing of items,

WDSRA may request that participants bring their own

equipment/supplies or provide items for individual use

only.

Food- WDSRA will reduce food preparation and service

during programs at the present time. When

eating during a program is necessary, participants are

asked to bring food in disposable containers or WDSRA

will purchase food from a restaurant.

Cleaning and Sanitation - WDSRA’s member park

districts have instituted enhanced cleaning procedures at

their facilities. WDSRA will be following these guidelines

as well. High touch areas will be cleaned prior to the start

of each program.

I M P O R T A N T I N F O R M A T I O N

For this session of in-person programming WDSRA will be

utilizing Lottery Registration. In this process all

registrations will be assigned once WDSRA "spins" the

lottery, which randomly assigns enrollments to the

programs you have registered for. The lottery will be spun

for Residents first followed by Non-Residents. If the

program have met the maximum number of participants,

those who were not enrolled in the program will be added to

a wait list.

Resident and Non Resident Procedures

WDSRA residents/non-residents can register online, by

mailing/emailing your registration form to WDSRA at

[email protected]. Once WDSRA has spun the lottery

you will receive an email confirmation with the programs

you are enrolled in. If you are placed on a wait list and

WDSRA can accommodate you safely, you will be enrolled in

the program.

Resident and Non Resident Registration Dates

allow participants/families who qualify for

Participants must be able to independently provide self-care such as bathroom needs, handwashing, eating, personal

hygiene, and dressing.

Participants must be able to maintain physical distance expectations with verbal reminders.

Participants will need to be able to refrain from physically aggressive behaviors such as biting, spitting, scratching,

hitting, kicking etc. or behaviors that require direct or close proximity intervention from staff.

Participants must be able to independently put on and wear a face covering when directed for the duration of the

program or when needed.

Participants must be able to stay in safe proximity to the group and refrain from elopement or wandering.

Participants must be able to respond to verbal prompts and lack the need for hand-over hand instruction.

Participants must be able to be responsible for their personal items with verbal prompts and reminders.

If a participant is unable to meet the following expectations, he/she may be unable to participate at this time in accordance

with current federal, state, and local mandates and guidelines. We expect guidelines to become less restrictive as Illinois

passes through each Restore Phase. Staff will consistently evaluate participant progress in meeting these criteria on a

weekly basis and communicate any concerns with parents.

PARTICIPANT ELIGIBILTY

Has the participant experienced a fever of 100.4 or greater in the last 24 hours?

Does the participant have a cough, sore throat or difficulty breathing?

Has the participant experienced muscle aches, headache or chills?

Has the participant lost sense of smell or taste?

Has the participant tested positive or been exposed to someone who tested positive for COVID within the past 14

days?

Prior to admittance to the program area parents/guardians/participants must complete a brief health screening

questionnaire. A paper form with screening questions, name and date must be completed for each participant prior to

drop off.

Following is a list of questions included on the health screening questionnaire form:

If the answer to any of the questions is “yes”, the participant will be asked to leave and not return until cleared by a

medical professional.

Sport SkillsBelow is a listing of athletic programs being offered. We are excited to get back in action with programs to refine our

skills. After registration, participants will receive a detailed welcome email including important information you need to

be aware of. To help keep everyone safe, if you have any equipment at home such as a basketball or football we

New Participant Intake WDSRA requires all new participants to complete anintake with the Outreach Supervisor prior to attendingtheir first program. The intake allows the agency to get toknow the participant and their individual needs better.This additional information provides WDSRA staff theopportunity to properly staff programs and provide anenvironment that will be successful and enjoyable to theparticipant. If you are new to the programs and have notdone an intake please contact Brianne Carroll at 630-681-0962. For safety reasons, an individual withoutcurrent annual information and intake forms on file willnot be permitted to participate in WDSRA programs.

Americans with Disabilities Act (ADA) Grievance ProcedureThe Western DuPage Special Recreation Association hasestablished an internal grievance system for prompt andequitable resolution of grievances alleging any action bythe Western DuPage Special Recreation Association thatwould be prohibited by the ADA or the applicable Title IIRules. Equal AccessNo participant shall be denied equal access to programs,activities, services or benefits, or be limited in the exerciseof any right, privilege, advantage or opportunity on the basisof race, sex, creed, national origin or disability.

Say CheeseWDSRA staff occasionally takes photographs or video ofparticipants for promoting/advertising our programs,services, events, activities, and facilities in our brochures,website or social media, and other promotional avenues. Byregistering for, participating in or attending WDSRAprograms, events, or other activities, the participant (or parent/guardian of a minor participant) irrevocablyagrees to the use and distribution by WDSRA of his or herimage (or of his minor child/ward) in photographs, videorecordings, and any other electronic reproductions of suchprograms, events and activities for any purpose withoutinspection or approval and without compensation, rights toroyalties or any other consideration now and in the future.

WDSRA Weather Cancellation PolicyIn the event of severe weather, WDSRA may decide tocancel programs in the best interest of our participants. Thefollowing guidelines will be used to determine cancellations:

Severe Summer Weather Outdoor programs or indoor programs without airconditioning: temperature above 95 degrees, a heat indexabove 100 degrees or a heat advisory issues by the NationalWeather Service.

Other Weather All programs (including indoor): tornadowarning is in effect in DuPage or surrounding county.WDSRA will use discretion for all other weather warningsand watches.

Canceling Programs 1. WDSRA staff will look at all circumstances beginningtwo hours prior to the starting time of the program orfirst pick-up to determine if the program should becanceled. 2. If the program is canceled, program staff will post thecancellation on the WDSRA Rainout Line.3. Missed programs due to weather will be issued arefund.

Refund Policy• Refunds will not be issued for missed classes.• Cancellations for weekly programs a refund will be issued. • A house credit will be issued for refunds $10 or less.• Any variance to the refund procedures will be included in the program description.• All refunds will be issued in check form.

Late Pick-Up PolicyIn order to be fair to our participants and programleaders, WDSRA has enacted a late pick-up policy andwe would appreciate your cooperation in this matter.Many of our programs have more than one pick-uplocation and if one person is late, it may disturb theschedule of others who are being picked up at anotherlocation. In addition, many of the WDSRA programleaders have responsibilities after everyone is picked up. We request that you double-check the pick-up time andbe prompt.

Dietary RestrictionsDue to the amount and various types of dietary needs,WDSRA will only provide an alternative option for mealswhen the program is preparing/making their own food, Ifthe program is attending a special event or eating at arestaurant in the community, the family must look aheadat the menu and relay the participant’s order to the headinstructor upon arrival.You may be asked to send or make appropriateaccommodations for your participant on occasion.

Behavior CodeParticipants, staff, volunteers and parents are expected toexhibit appropriate behavior at all times. The followingguidelines have been developed to make WDSRA’sprograms safe and enjoyable for everyone. Additional rulesmay be developed for particular programs and athleticleagues as deemed necessary by staff. Staff, participants,volunteers and parents shall:

• show respect to all participants, staff, volunteers andparents • refrain from using foul language • refrain from causing bodily harm to other participants,staff, coaches or parents • demonstrate respect for equipment, supplies andfacilities • represent WDSRA in a manner that is consistent with thegoals, objectives and ethics of the association

Discipline A caring and positive discipline approach will be used. WDSRA reserves the right to dismiss aparticipant, staff, volunteer or parents if it is in the bestinterest of the association. Each situation will be evaluatedon its own merit.

Annual Information Form WDSRA requires all participants to complete an annualinformation form prior to participation. This form expiresevery year on December 31. The form provides us withvaluable information which enables us to offer programsthat are safe and appropriate for your needs. If you arenew to WDSRA programs or do not currently have anannual form on file, please contact our office and requestone. If the agency receives a registration form that doesnot have a current annual, we will send you one tocomplete and return.

Out of District Policies Families living outside a WDSRA member parkdistrict and belong to a neighboring SRA are welcometo register in WDSRA programs during the out ofdistrict registration time frame. This would apply toNEDSRA, FVSRA, NWSRA, and SEASPARresidents.

If you live within a community with a Park District thatdoes not belong to a Special Recreation Associationa 50% fee will be assessed per program.

If you live in a community without a Park District or inan unincorporated area that falls outside of the ParkDistrict boundaries, a 50% fee will be assessed perprogram. An example of this would be Wayneresidents and some parts of West Chicago.

Western DuPage Special Recreation Association Important Information

The Western DuPage Special Recreation Association (WDSRA) is committed to conducting its recreation programs andactivities in a safe manner and holds the safety of participants in high regard. WDSRA continually strives to reduce suchrisks, and insists that all participants follow safety rules and instructions that are designed to protect the participants’safety. However, participants and parents/guardians of minors registering for this program/activity must recognize thatthere is an inherent risk of injury when choosing to participate in recreational activities.

You are solely responsible for determining if you or your minor child/ward are physically fit and/or skilled for the activitiescontemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way orrecently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity.

Warning of RiskDespite careful and proper preparation, instruction, medical advice, conditioning and equipment, there is still a risk ofserious injury when participating in any recreational activity/program. Understandably, not all hazards and dangers can beforeseen. Participants must understand that certain risks, dangers and injuries due to acts of God, inclement weather,slipping, falling, equipment failure, failure in supervision, premises defects and all other circumstances inherent torecreational activities/programs exist. In this regard, it must be recognized that it is impossible for WDSRA to guaranteeabsolute safety.

Waiver and Release of All Claims and Assumption of RiskPlease read this form carefully and be aware that in signing up and participating in this program/activity, you will beexpressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which youor your minor child/ward might sustain as a result of participating in any and all activities connected with and associatedwith this program/activity (including transportation services, when provided).

I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and Ivoluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minorchild/ward or I may sustain as a result of said participation. I further agree to waive and relinquish all claims I or my minorchild/ward may have (or accrue to me or my child/ward) as a result of participating in this program/activity againstWDSRA, including its officials, agents, volunteers and employees (hereinafter collectively referred as WDSRA).

I do hereby fully release and forever discharge WDSRA from any and all claims for injuries, damages, or loss that my minorchild/ward or I may have or which may accrue to me or my minor child/ward and arising out of, connected with, or in anyway associated with this program/activity.

In the event of an emergency, I understand and authorize WDSRA staff and officials to secure from any licensed hospital,physician, and/or medical personnel any treatment deemed necessary for immediate care for myself or minor child/wardand agree that I will be responsible for payment of any and all medical services rendered.
