Instruction:-

1. Bank Draft from any Nationalised Bank should be drawn in favour of Canteen Stores Department

Public Fund Account (Main) payable at ______________________(concerned Depot).

2. All SSC?EC officers must bring their original service record/discharge papers in support of having

rendered complete five years service in the Armed forces.

3. Original copy of PPO is required to be submitted while booking four wheeler by retired officers.4. Hony Officers to bring their pension book, discharge book and identity card at the time of booking

of four wheelers in CSD Depot.

5. Xerox copy of PAN is required to be attached with indent form.

6. For retired officers, indent form duly countersigned by their concerned Zila Sainik Welfare

Officer/Station HQs will be accepted.

7. Cutting erasures and whitener used in indent form will not be accepted.

8. No AFD indent form with the printed name of dealers will be accepted, however a separate

availability certificate from dealer can be accompanied with indent form.

9. Widows of service officers must produce an evidence that they are not remarried.10. No interest will be claimed for delay in delivery of vehicle by dealer.

It is certified that information/particulars given by the above applicant are verified and found

correct.
