This course will review the pathogenesis, clinical presentation, diagnosis and treatment of common and uncommon oral lesions/diseases. Learning Objectives: At the conclusion of this course, participants will:

Understand the importance of a comprehensive medical and dental history as well as recognize the clinical signs and symp-toms of oral pathology.

Recognize the etiology and consequences of common diseases.

Assess clinical signs and symptoms in order to formulate a differential diagnosis.

Determine when and if a referral is recommended for a definitive diagnosis.

Identify and describe some of the more common oral lesions.

Understand the various diagnostic procedures and tools available to the clinician.

Recognize and have a basic understanding of new and evolving technologies in prevention and diagnosis of oral lesions/

diseases.

6:00 - 9:00 PM C2 “ MANAGEMENT OF THE MEDICALLY COMPROMISED PATIENT IN THE DENTAL OFFICE” (3 CEUS)

Presented by Martin J. Dunn, DMD

In 1910, the average life expectancy in the United States was 40 years of age; today it is almost 80. Currently, there are 5 million people over the age of 85, with the projection that by 2050, this number will reach 17 million. This increase is due to medical advances such as new surgical techniques, drug therapy, chemotherapy, and organ transplants that allow patients who previously were confined to their home or unable to work to live fairly normal lives and seek dental treatment. Many of these potentially at-risk patients have chronic disorders that are controlled but not cured; however, they need dental care and must be carefully managed in the dental office. This session will review the issues and complica-tions associated with medically compromised patients in the dental setting and provide guidance for proper management, focusing particularly on new concepts and recent guidelines. Learning Objectives: At the conclusion of this course, participants will:

Understand the new guidelines regarding hypertension parameters and determine when a patient should abstain from dental treatment.

Identify current prophylactic antibiotic coverage in cardiovascular disease and total joint replacement.

Analyze appropriate use of vasoconstrictors in local anesthetics for patients with cardiovascular disease and hyperthyroidism.

Determine if and when to discontinue anticoagulants in blood-producing procedures.

Evaluate the effect of new oral anticoagulants in treatment of atrial fibrillation for the prevention of blood clots and stroke on dental patients.

Explain the significance and use of the HbA1C test in diabetic patients.

Identify proper procedures for management of patients on steroids and patients on dialysis.

Martin J. Dunn, DMD received his dental doctorate from Tufts University School of Dental Medicine, with post-graduate residency in Oral & Maxillofacial Surgery at Boston City Hospital/Medical Center and Tufts University – School of Graduate Dentistry and also continuing education worldwide in Louisiana, Texas, France, Denmark and Switzerland. He taught Head and Neck Anatomy for 35 years at the Forsyth School of Dental Hygiene. Currently he teaches a course on Management of the Medically Compromised Patient at Forsyth. Dr. Dunn has authored several textbooks including “The Anatomical Basis of Dental Hygiene”. He is a Diplomat on the American Board of Oral & Maxillofacial Surgery as well as a Fellow of the American Association of Oral & Maxillofacial Surgeons and the American College of Oral & Maxillofacial Surgeons. Dr. Dunn has presented in 133 cities throughout 30 states and in Canada.

Renay Ivens, DDS serves as the dental supervisor at the Vermont Technical College, Department of Dental Hy-

giene where she has been treating patients as well as co-teaching Nutrition, Oral Pathology, and Local Anesthesia to the

students. She graduated with an Associates degree in Dental Hygiene from Ferris State University in Big Rapids, MI in

1980 and continued her education at Michigan State University to pursue and advance her career in dentistry. She re-

ceived her Bachelor of Science and DDS degree from University of San Francisco in 1995 and completed a 2 year post-

graduate dental residency program (AEGD )in 1997, with interests in oral stomatology. Dr. Ivens maintained a private

family practice in Galt, California for 8 years immediately after graduating from dental school and prior to moving to Ver-

mont. She worked as a dental associate with numerous general practitioners over the past 9 years in Vermont as well as volunteered her

time at the Vermont Technical College Department of Dental Hygiene where she and her colleagues implemented a dental rotation to ad-

dress the dental needs of the patients and expose students to the practice of dentistry. Dr. Ivens also is a Fellow in the field of Biological

Dentistry, is a member of the American Dental Association, and the Vermont State Dental Society, an honorary member of Sigma Phi Alpha-

Phi Chapter, and serves on the VTC Dental Hygiene Dept. Advisory Committee. She stays involved in numerous community services as

well, including Tooth Tutors and Special Olympics.

The American Dental Association estimates that there are 3,000 life threatening emergencies in dental offices every year. They further estimate that the average dentist will have one or more life threatening emergencies during his or her professional career and the number of these emer-gencies are on the rise. Most office emergencies are not life threatening but should be treated as such until proven otherwise. This 3 hour course will detail the preparation for medical emergencies including the necessary drugs, equipment and scheduled mock drills em-phasizing the duties of each team member in the management of the emergencies. The eleven most common medical emergencies will be de-tailed including prompt recognition of the signs and systems of these emergencies and an equally prompt treatment by the entire dental team. Learning Objectives: At the conclusion of this course, participants will:

Be aware that medical emergencies, some of which are life threatening, do occur with a degree of frequency in the dental office.

Know the necessary drugs, equipment and mock drills to prepare for these emergencies.

Identify the specific duties of each of the team members in treating medical emergencies.

Recognize the signs and symptoms of specific emergencies.

React quickly and confidently in the treatment of each medical emergency.

Know the precautions to undertake to prevent specific medical emergencies from occurring.

BASICS FOR IMPLEMENTING DIETARY ASSESSMENT AND COUNSELING IN THE DENTAL OFFICE” (3 CEUS)

This course will provide a refresher on nutrition assessment and counseling in the dental setting using the most recent nutrition guidelines and readily available online resources. There will also be an emphasis on the questions to ask during the medical history review and dental examina-tion to aid in identifying patients at risk for marginal or poor nutrition along with guidance on referral of patients in need of further nutrition counsel-ing. Learning Objectives:

At the conclusion of this course participants will:

Demonstrate basic nutrition assessment using the national nutrition guidelines

Identify patients at high risk for poor nutrition.

Implement basic nutrition assessment and counseling as it relates to general and oral health using online resources in the dental operatory.

Refer at-risk patients to a nutrition professional for complex nutritional considerations.

Linda Boyd, RDH, RD, EdD is Dean and Professor of the Forsyth School of Dental Hygiene at Massachusetts College of Pharmacy and Health Sciences. Dr. Boyd has served in leadership roles in predoctoral dental education in Oregon and graduate and undergraduate dental hygiene education in Georgia, Idaho, and Massachusetts. Prior to enter-ing academia she practiced in general and periodontal practice for over 20 years. Dr. Boyd completed a Master of Science in Nutrition Science and Policy at Tufts University and dietetic internship at New England Medical Center. She completed her doctorate in educational leadership and post-secondary education at Port-land State University.

Dr. Boyd has published widely in textbooks and peer-reviewed journals on a variety of education, nutrition, and oral health topics. She is currently co-editor on a nutrition and oral health textbook with Dr. Carole Palmer at Tufts University and contributing editor for the 12th edition of Dr. Esther Wilkins’ seminal text Clinical Practice of the Dental Hygienist. In addition she has presented to local, state and na-tional organizations on critical thinking, educational and systemic disease and oral health topics. She is active in several professional organizations and has served in leadership positions in the American Dental Hygienists’ Association, American Dental Education Associ-ation, and Academy of Nutrition and Dietetics. Most recently she served as content advisor for revision of the Academy of Nutrition and Dietetics’ position paper on Nutrition and Oral Health and co-authored a practice paper to accompany the position paper.
