Documenta�on of medical necessity provided by

the consumers Speech Language Pathologist must

include the following:

1. Documenta on of the medical diagnosis or treat-

ment of medical diagnoses related to the need for

the AAC device or so�ware by a licensed Speech-

Language pathologist.

2. Descrip on of the recommended device/

accessory/so�ware, how it will be used and incor-

porated in the consumers daily life.

3. Explana on of the criteria that will be used to

measure progress toward both short—and long

term goals using the device and so�ware.

4. Outline the training plan that will be used to max-

imize the use of the device and so�ware.

program that sup-

ports children

that are non-

verbal, have

speech delay, or

are unable to fully

express them-

selves by offering

communica on

technology support. This wonderful program gives fami-

lies the opportunity to obtain suppor ve technology to

help the child express themselves at home, in school,

and in everyday life. CPMA works with the consumer’s

licensed speech pathologist who will recommend what

technology device, suppor ve apps, and tools the con-

sumer needs to achieve speech goals and be-er quality

of life. By using technology support, the child is able to

be-er communicate, express themselves, and reduce

behaviors. Eligible consumers for this program are

based on MassHealth, fit into a low-income financial

bracket, have an up to date IEP (if a student), and are

under the treatment of a licensed speech pathologist.

Requirements when reques�ng an iPad device:

• Consumer must be on MassHealth.

• Consumer must be have a clinical diagnosis, and be

under the treatment of a Speech-Language

pathologist.

• Therapist to provide and AAC evalua on to CPMA.

• Therapist to recommend so�ware (apps) that will

benefit the child with speech therapy goals.
