These normal audiometric threshold values represent the mean lowest level at which a group of young adults can detect a selected sound. speech discrimination (understanding of speech).Speech audiometry measures hearing sensitivity forspoken material. In addition, spoken material can beused to test for speech discrimination.

Audiometric booths provide a consistent and con-trolled acoustic environment and keep backgroundnoise at an acceptably low level to provide a calibratedacoustic test area for clinical audiometry or research.

Principles of operationAudiometers

In the normal ear, sound waves travel through airin the outer ear and cause bones in the middle ear tovibrate. The inner ear converts these mechanical vi-brations to hydraulic energy and ultimately to animpulse that is transmitted along the auditory nerveto the brain, where it is perceived as sound.

When a patient has a suspected hearing loss, aphysician may order an audiometric analysis; screen-ing and diagnostic testing are then directed by theaudiologist. The conventional procedure for determin-ing hearing threshold begins with familiarizing thepatient with the stimulus. The test proceeds accordingto a protocol recommended by the American Speech-Language-Hearing Association; the protocol requiresa fixed number of positive responses to stimuli (puretones or speech) presented in sequences of increasingintensity.

The weakest sound heard at a selected frequency isthe hearing level in decibels (dB HL) for that particularfrequency. This is a relative value; the intensity refer-ence is 0 dB HL, or audiometric 0, which correspondsto the average threshold response (for a normal inten-sity range of -10 to +25 dB HL) of a group of 18- to25-year-olds with no otologic pathology. The sensitiv-ity of the normal ear varies with frequency; therefore,0 dB HL represents different levels of sound pressureat different frequencies. (Minus dB HL readings indi-cate that hearing sensitivity is greater for that particu-lar frequency than for the average value.)

Pure-tone audiometers typically generate a series offrequencies between 125 and 8,000 Hz, depending onthe type of test and audiometer. The American Na-tional Standards Institute (ANSI) specifies three cate-gories of audiometers — wide, limited, and narrowrange — based on the range of frequencies, soundpressure levels, and testing capabilities available.Only wide-range audiometers can be used for bothpure-tone and speech testing.

In a pure-tone audiometer, single-frequency tonesare produced by an audio oscillator. An attenuator dial

controls the intensity changes within a specified range,typically from 0 to 90 dB HL. Signals can be deliveredin a continuous or pulsed (interrupted) mode; pulsedtones appear to be more useful when testing childrenor patients with tinnitus (ringing in the ear). Micro-processor-based units give the examiner the option ofdelivering the signal manually or automatically, usinga programmed presentation sequence. An interrupterswitch is used to insert or stop a signal. The examinerdirects the signal to earphones or to a bone-conductionvibrator. The patient may use hand signals or a hand-switch to respond to a tone that has been heard. Bothair- and bone-conduction pure-tone testing must beperformed to determine whether hearing loss is con-ductive, sensorineural, or both.

Pure tones are directed to earphones to test air-conduction hearing, which relies on the action of theouter, middle, and inner ear and neural pathways. Theelectrical signal from the audiometer is transformedinto vibrations within an earphone’s diaphragm thatgenerate a sound of a frequency and amplitude (dB)proportional to the electrical signal. The examiner cantest the left ear, right ear, or both. The thresholdsearch protocol is performed for a fixed series of fre-quencies in each ear. Results are recorded in a graphicand/or tabular format called an audiogram (see Fig. 1).

Bone-conduction hearing relies solely on the actionof the inner ear and neural pathways. Pure tones aredirected to a bone-conduction vibrator placed on theforehead or the mastoid process behind the ear. Theelectrical signal from the audiometer creates vibra-tions within a plastic shell in the vibrator. As thevibrations are transferred to the skull, the inner ear isstimulated directly as a result of bone distortion andindirectly as a result of inputs received from outer- andmiddle-ear vibrations. Some instruments record air-and bone-conduction data on the same graph or table,while others show the data on separate audiograms.

The procedure for bone-conduction testing is basi-cally the same as that for air-conduction testing. Ide-ally, both should be performed in an audiometricbooth; however, when this kind of room is unavailable,testing should be performed in quiet surroundingswith minimal background noise. Some instrumentsfeature a two-way microphone built into the earphoneheadset to permit communication between the patientand the examiner when the patient is isolated in anaudiometric booth.

In conductive hearing loss, air-conduction thresh-olds are less sensitive (higher) than bone-conductionthresholds. In sensorineural hearing loss, both air- andbone-conduction thresholds are equal and elevated(hearing is below normal). When hearing loss is

“mixed” (both conductive and sensorineural), air- andbone-conduction thresholds are again elevated; how-ever, air-conduction thresholds are less sensitive thanbone-conduction thresholds. Bone-conduction testingis sometimes used in lieu of air-conduction testing foryoung children, who cannot provide reliable pure-tonethresholds.

Speech audiometry measures the sound levels atwhich speech is detected and individual words aredistinguished from one another. Spoken material isintroduced as an input signal to the audiometer froma microphone, phonograph, tape recorder, or compactdisc. The signal is intensified in an amplifier andmonitored by a volume units meter. Intensity range iscalibrated in dB HL and is controlled with an attenu-ator as in pure-tone audiometry. The examiner directsthe signal to earphones or through a second amplifierto loudspeakers. An ear-selector control may also beused to direct the signal to the left or right earphoneonly.

Most testing is done through air-conduction ear-phones. Sensitivity for speech is determined using aspeech reception threshold (SRT) test. Thresholdsearch protocols are similar to those used for pure-toneaudiometry. The second component of speechaudiometry is the speech discrimination test (SDT),

which assesses the patient’s ability to understandmonosyllabic words at suprathreshold (above thresh-old) intensities. Patients with severe inner ear diseasecannot distinguish between similar words, even atthese high sound levels.

When earphones are not practical — for example,when young children are being tested — free-fieldtesting is used. The patient is placed equidistantbetween two loudspeakers in a sound-isolation room.Warble tones (tones with specific modulation rates),narrow-band noise (tones of several frequencies ofequal intensity), or spoken material is used as theauditory signal.

The shape of audiometric curves can be very usefulin diagnosing certain types of hearing loss when othermethods of clinical and laboratory analysis are unsuc-cessful. For example, patients with hereditary domi-nant mid- and low-frequency sensorineural loss showno evidence of pathology aside from their charac-teristic audiograms (see Fig. 2). Audiometric testing ofsiblings provides confirmation of this diagnosis.

When the nontest ear is suspected of responding tothe test signal, it must be masked. One of three typesof masking is applied to the earphone correspondingto the nontest ear: white noise, pink noise, or narrow-band noise. White noise consists of a wide range of

frequencies with uniform loudness and is used to masktones or speech. Pink noise is similar to white noiseexcept that it consists of a higher proportion of lowerfrequencies and is used to mask speech alone. Narrow-band noise consists only of frequencies close to the onesbeing tested and is used to mask pure tones. Properapplication of masking is complex, especially when thehearing loss is mixed or if only one ear has a severesensorineural loss. Masking intensity is usually quan-tified in decibels sound pressure level (dB SPL).

Audiometric booths

The walls of audiometric booths consist of steelpanels on the outside, sound-absorbing perforatedsteel panels on the inside, and an incombustible acous-tic insulating material (e.g., fiberglass) in between.

Single-wall booths are constructed from a singlewall and a layer of sound-absorbing material and areused primarily for pure-tone air-conduction testing.Double-wall booths employ “booth inside a booth” con-struction in which the walls of the inner examinationbooth are separated from the outer panels by an airspace; this design further reduces outside noise inter-ference and is preferred for bone-conduction, sound-field, and speech testing. Single- or double-wallconstruction can be used in both single- and double-room configurations.

Single-room booths (see Fig. 3) isolate the subjectfrom the environment, but not the examiner. Mostaudiometry can be performed in a single-room booth;the exception is live-voice speech testing. Double-roombooths (see Fig. 4) consist of two connecting rooms (acontrol room and an exam room) that isolate the exam-iner and subject from the environment and also fromeach other. Either a single or double wall (dependingon the booth construction) separates the two rooms andincorporates a window consisting of several layers ofglass. Windows should provide visual contact betweenexaminer and subject but obscure the subject’s view ofany distracting activities in the control room. Win-dows are also present in the walls of a single-roombooth and may be present in the external walls of adouble-room booth to allow the inside of the booth tobe viewed from the outside.

Single-wall booths have one door, opening outward;double-wall booths have a second door, opening in-ward, which is mounted on the same frame. Somedouble-room configurations offer an optional single ordouble door between the control and test rooms. Vibra-tion isolators underneath the booth keep floor vibra-tions and sound transmission to a minimum. Aspecially designed forced-air system can minimize ven-tilation noise.

Portable booths are single-wall structures and moveon locking casters. They may also come with vibrationisolators and ventilation systems. Larger, nonportablebooths come in kits that include prewired wall, floor,and roof sections; wall and corner joiners; acousticfilling material; and a ventilation system — manyportable booths come completely assembled. Somesmaller portable booths are designed to be disassem-bled for carrying from one testing location to another.

Reported problemsReliable audiometric measurement depends on

equipment calibration, the test environment, patientcooperation, and examiner performance. All audiomet-ric equipment must be calibrated according to inter-national standards. However, calibration and testresults are invalid if the test is not performed in theproper environment. Background noise levels mustnot exceed those prescribed by organizations such asANSI, and visual distractions, ventilation, and tem-perature must be controlled. Finally, the room shouldbe set up so that the patient cannot see what theexaminer is doing with the audiometer while the testis being performed.

ECRI has received no reports of safety problemsunique to audiometric booths. Single-wall booths canbe constructed of optional fireproof panels and doors;the inner panels and doors of double-wall booths canalso be made of fireproof material.

Earphones should be in proper condition: padsshould be soft, pliable, and free of cracks. Cords shouldalso be checked to make sure that the left and rightearphones are connected to the correct earjacks. Ear-phones that are designed and calibrated for use withone audiometer should not be used with other audi-ometers because they may not reproduce the sameauditory signal sent by the audiometer.

Booths with fluorescent lighting must have thelight’s starter mounted on the outside of the examina-tion room because starters produce a hum that mayinterfere with testing. In addition, windows musthave moisture-absorbing materials between the panesto prevent fogging. If the booth has a step, a rampmust be provided for subjects in wheelchairs or onstretchers. Because doors must be solid and close withan acoustic seal, they are often very heavy.

Purchase considerationsSome audiometers feature software that assesses

the hearing level by comparing it to age-related valuesfrom the general population. Software is availablethat allows the user to design a format that can

accommodate a large amount of patient (and company)data or to customize test protocols to fit the individualpatient’s needs and capabilities. Some audiometersprovide a graphic output of results for threshold test-ing, and several units can store a limited number ofaudiograms in nonvolatile memory.

Multichannel audiometers are available that canexamine several people at once. Each channel of a

group audiometer is programmed to administer a com-plete series of tests to each individual; all channels arelinked to a central control channel through which theexaminer can set the test parameters and initiatetesting. Special software programs compare the indi-vidual’s hearing threshold to a normal range of values(adjusted for age and exposure to noise) — for exam-ple, from an employee’s occupational group. (Manycompanies require their employees to undergo regular

audiometric testing as part of a hearing-conservationprogram. Multichannel audiometers enable compa-nies to perform time- and cost-effective on-site testingof the workforce.)

An audiometric booth must be large enough to ac-commodate the examiner and the subject, as well asany equipment, attendants, or family members pre-sent during testing. In addition, booth selection shoulddepend on the amount of sound reduction needed in aspecific environment to meet standards for audiomet-ric testing. Where a high level of ambient noise exists,even double-wall booths may not meet the require-ments for background noise levels at the lower testingfrequencies (250 and 500 Hz) during testing withoutearphones.

Users should consider a booth’s percent of harmonicdistortion, memory size, calibration accuracy, and in-cluded standard functions. Various suppliers includeoptional parts such as noise-reduction headsets/insertphones, external power amplifiers, computer cables,microphones, software, memory cards, and servicemanuals. Check to see if the warranty includes ahotline service for any technical problems and/or userdifficulty.

Stage of developmentMicroprocessor-based units have automated most

test procedures and hearing-threshold calculations.Certain computerized units have validity checks withintheir operational programs that repeat the signal for agiven frequency and compare the two responses. PC(personal computer)-based audiometers that can beused with notebook, desktop, or networked systems arealso available. These systems may have features suchas graphical displays of results, warnings, and referrallevels; use of disk drives for storage; and connection toa printer to obtain additional records.

To ensure that the delivered frequency closely cor-responds to the displayed or selected frequency inpure-tone testing, several manufacturers use quartzcrystals in their audiometers to stabilize the circuitfrequency of sound transmitters; quartz control of sig-nal timing has also improved synchronization of tonepresentation and masking.

For instances in which standard tests are beingused in different ways (e.g., bone-conduction testing inspeech audiometry), calibration standards are stillbeing developed.

gather more specific information about the nature of ahearing problem. More sensitive and specialized testshave been created for these instruments to enhancediagnosis and monitor the results of rehabilitativetherapy.

Audiometric booths are a widely accepted compo-nent of the instrumentation used in clinical diagnosisof hearing disorders. With the increasing availabilityof smaller portable models, additional applicationsmay develop. There have been no recent changes inthe technology of these rooms, and none are foresee-able in the near future.

Type: Refers to wide-, limited-, or narrow-range audi-ometers as defined in the ANSI S3.6 standard.Wide-range audiometers provide the widest rangeof capabilities and test frequencies and are recom-mended for clinical diagnostic purposes (i.e., whereair- and bone-conduction testing, speech-threshold

testing, and masking are employed in a sound-iso-lation room). For pure-tone testing, when portabil-ity is important (e.g., testing adults in industry),limited-range audiometers are sufficient. Theseaudiometers produce tones of 500, 1,000, 2,000,3,000, 4,000, and 6,000 Hz, with an intensity rangefrom 10 to 70 dB HL. Narrow-range audiometerswith a more limited selection of frequencies andintensities are used only with pure-tone testing inscreening situations.

Chart B: Audiometric BoothsVibration isolators: Rails incorporated into floor con-

struction that provide uniform weight distributionthroughout the floor and thereby reduce vibrationswithin the room.
