Atlas of Psychiatric Pharmacotherapy .RONI SHILOH, DAVID NUTT and ABRAHAM WEIZMAN. Martin Dunitz Publishers, 2000. Pages: 248. £29.95/$49.95. Why does psychopharmacology engender such terror in examination candidates and others? Is it its huge and rapidly changing nature? Or is it the knowledge that merely by tweaking a receptor name here or ligand name there, scores of baf¯ing multiple choice questions may be generated? One reason is perhaps the persisting dif®culty in ®nding up-to-date material in one digestible format. Any book which attempts to tackle this Herculean task must therefore be wel- comed. This book is true to its claim to be an atlas by relying heavily on multi-coloured ¯ow charts, schematic diagrams and tables, and accompanying text is cast in a valuable supporting role. Some illus- trations are busy, but then the topic is complex. For practical purposes the book is divided into four principal sections. Section A is a review of mechan- isms of action of commonly prescribed drugs. Although otherwise thorough, anti-dementia treat- ments are relatively brie¯y overviewed in a subsec- tion of a single chapter on `miscellaneous drugs'. No mention is made of newer treatments such as riv- astigmine or galantamine. Section B reviews the effects of abused substances; Section C reviews detailed interactions between classes of drugs, includ- ing the notorious monoamine oxidase inhibitors. However, practising clinicians will ®nd the penulti- mate section on treatment strategies to be the most useful. There is a detailed review of pharmacological strategies in mood disorders, psychosis, anxiety and even personality disorders. The book is evidence based in that data is cate- gorised into `well established data', `partially esta- eblished data' and `not well established data'. However, psychogeriatricians will know that most published therapeutic trial literature is based on sub- jects under the age of 65. Moreover, the authors' gui- dance on treating patients over 65 is inconsistent; for example, references are made to lower therapeutic serum levels for lithium in some chapters, but not in others. Perhaps psychopharmacology would be ideally suited to electronic publishing media incor- porating updates as advances are made. The authors suggest that this book would be suita- ble for medical students and general practitioners. Yet it presumes a pre-existing depth of knowledge that one could not legitimately expect such readers to pos- sess. Although specialist psychogeriatricians may feel a little overlooked, the book will doubtless appeal to postgraduate psychiatry trainees and clinicians treat- ing younger patients with psychiatric disorders. It is attractively presented, and given the sheer amount of information it conveys is reasonably priced. S.
