Assessment name: Critical Reflection
Your task is: Throughout the semester you will be undertaking 5 virtual field visits to various facilities to provide you with an understanding of the breadth of the health information management profession. Following each visit, you will be required to develop a journal entry that will offer your reflections on what you learnt about the facility and health information management. There are two parts to the site visit journal.Part A - site visit questions: Specific questions will be provided prior to the visit on topics related to that particular facility. You will be required to provide answers to those questions – which can be obtained during the visit.Part B – reflection - You will also then be required to provide a brief reflection of additional learnings from the visit.
Your task:
1. Prior to each visit, go to the Learning Resources tab and access the documents for that visit. Look for the document titled journal questions.
2. Download the document and have it available during the visit – taking notes during the visit.
3. Following the visit write up your responses to the questions (Part A) as well as your reflections (Part B) on the visit.
4. Once you are happy with your responses, post the entry to Blackboard.

Length and formatting : For each site visit journal Part A should be no more than 100 words and Part B should be no more than 400 words. Please ensure you clearly label the sections Part A and Part B. Please do not merge Part A and Part B. Use the template provided in Attachment A of this document to complete each journal.
Formatting the journalsHere is some information about how to present your report. Failure to follow these guidelines will result in reduced grades.
Microsoft Word format 12 point Times New Roman or 11 point Arial 1.5 - double-spacing page margins must be ‘normal’ or at least 2.54cm on each side and top and
bottom a title page which must include the following:
o your name and student numbero the unit name and unit coordinator’s nameo the title of your assignment (e.g. Assessment 1: Princess Alexandra

your name and student number must in the header page numbers must be inserted in the bottom right corner of each page Naming your files: As the documents will be loaded to Blackboard, please

Your task is: This is assessment is in two parts the first is to develop a report and the second is to present the findings of your report to the class in Week 13.

Part B:PresentationThe task in week 13 will be to provide a 5 minute pre-recorded voice over presentation that will be submitted online via Blackboard. Your presentation will cover the three points of defining and describing, challenges and role of HIMs.

Maximum word counts are provided for each of the sections as well. Ensure you adhere to these word count requirements as penalties will be applied for exceeding the length or being excessively short.
