JOB DESCRIPTION

This programme is a 365 day a year programme therefore you will be required to work bank holidays and weekends on a rota basis over a 35 hour working week.

Job purpose / Context:

To provide a high quality service to service users of the Diamorphine Assisted Treatment (DAT) programme in Sandwell

Manage a caseload of service users on the DAT programme, helping them in their recovery to address their substance misuse issues and achieve a healthier and safer lifestyle. This will include completing assessments, a range of interventions, harm reduction advice and ongoing psychosocial interventions with service users

Provide relevant interventions based upon appropriate assessment or need and provide all relevant services in line with agreed targets and objectives of the DAT programme

Provide advice, advocacy and support service users supported by the DAT programme

Support engagement with the outreach team to ensure we are offering a wraparound service to help service users achieve all of their goals

Provide reports and updates in a timely manner to the Multi agency operational group overseeing the programme

Where appropriate to work in close partnership with other agencies to ensure service users access a full range of services beneficial to improving their positive outcomes

Responsible to: Team Leader

1. Duties and Responsibilities:

Work directly with service users and potential service users delivering motivational interviewing interventions to support engagement with the DAT programme

Promote, encourage and support engagement with Mutual Aid and Recovery Services alongside the DAT programme interventions.

Work in partnership with Criminal Justice Agencies and the Police to provide appropriate interventions for service users in custody, on probation orders and on release from prison to ensure safe continuity of care between the DAT programme and custody

Undertake outreach and satellite provision to ensure the service covers the geographical area sufficiently where required.

Link closely with the outreach team to ensure appropriate levels of support are offered and followed up for DAT programme service users

Attend promotional events locally and nationally. Deliver information, advice and training events to Sandwell professionals. Attend appropriate internal and external meetings in relation to the DAT and

other business as required. Manage own workload, engage service users in the provision of a range of

interventions, assessment and care planning in line with the DAT programme. Contribute to the development of services and policies. Undertake assessments, offer harm reduction advice Participate fully in supervision with the senior practitioner and team leader.

Provide written and verbal reports as requested by the Borough Manager or Team Leader.

Comply with all Cranstoun policies and procedures. Carry out duties to which they are competent to perform as directed by the

Borough Manager or Team Leader.

2. Advice and Information

Raise awareness about substances, their use and effects including information on the DAT.

3. Assessment and Care Planning

Undertake assessment sessions with pre-identified service users, ascertaining and responding to their treatment, housing, accommodation, employment and personal support needs.

Undertake initial comprehensive assessments of service users’ needs & determine appropriate intervention required in line with the parameters of the DAT programme.

Plan and agree service responses which meet individual’s identified needs and circumstances.

Assist in the transfer of individuals between agencies and services ensuring consideration to DAT programme and clinical needs of the service user.

4. Support

Support individuals to access and use services and facilities. Support individuals who are substance users including those identified as

suitable for treatment as part of the DAT programme. Assist individuals to explore and plan for future employment, training and

education opportunities. Enable individuals to access housing and accommodation. Enable individuals to administer their financial affairs. Work with individuals to address their substance use using recognised

models. Help individuals address their substance use through an individualised

Recovery Plan.

5. Through care

Assist in the transfer of individuals between agencies and services taking into consideration the DAT programme involvement.

6. Monitoring

Provide information to support decision making. Ensure records are kept up to date and relevant via the digital database and

ensure any papers are scanned to the system.

Support the DAT team to collate data to enable effective monitoring of DAT programme progress in line with partner requirements

7. General – Professional Practice

Relate to and interact with individuals. Ability to facilitate Group Work. Promote peoples equality, diversity and rights. Promote choice, wellbeing and the protection of individuals. Assess and act upon immediate risk of danger to substance users. Develop your own knowledge and practice. Make use of supervision.

8. Operational Duties

Contribute to maintaining a place of safety for all service users & staff Maintain records as appropriate & in line with Cranstoun Policies and those of

the Borough Contribute to incident reporting and investigations Report to the Team Leader as & when required in respect of client work. Liaise with all referral agencies & workers as & when appropriate. Maintain monitoring procedures as per the Borough policies Provide information, advice and advocacy services to service users as

required Assist service users in relation to their aftercare and resettlement Write reports relating to service users progression to courts etc. as and when

required Support the work of the clinical team Attend meetings as required Undertake any additional duties as may be reasonably required by the Team

Leader or Borough Manager.

9. Other Duties

Observe Cranstoun:Sandwell information management strategy by;-

Carry out day to day tasks in accordance with stated policies, procedures and regulations to assist the Borough achieve its compliance obligations, including CQC.

Ensure the effective flow of information within the service, with managers and external parties by passing on and seeking information required, raising unresolved concerns and taking an active interest in Cranstoun:Sandwell internal communications.

Help Cranstoun to increase its performance and long term sustainability by optimising the management of IT, facilities and other physical resources that support the achievement of the Borough plan.

Project the desired image of Cranstoun:Sandwell by;-

Understanding and promoting Cranstoun values. Being a role model of the values through own example while ensuring they

are demonstrated by the staff in your team. Ensure adherence to confidentiality policy in all aspects of work

PERSON SPECIFICATIONQualifications

Essential Requirements

Experience of working with people who have complex health needs Experience of working with people who face barriers to engaging with

services and identifying ways to remove these obstacles/barriers Experience of direct client work with drug and alcohol users or a comparable

client group. Ability to Facilitate Groups Experience of using assessment tools Experience of one-to-one work An understanding of the needs of drug and alcohol users and knowledge of

harm minimisation. An understanding of the intentions of the Diamorphine Assisted Treatment

Programme The ability to work in a pressurised, multi-disciplinary environment Good written and verbal communication skills with a good level of IT skills. Effective listening skills Ability to work effectively and efficiently within a team and build effective

working relationships with other agencies. A non-judgemental attitude to drug and alcohol users An understanding of care planning Previous experience of recovery orientated care coordination including

reduction planning and detoxification’ Word processing skills including the ability to use an electronic data

management system

Desirable

Knowledge of the range of community drug and alcohol services and models of treatment

Knowledge of Dual Diagnosis Knowledge of brief intervention methods including Brief Solution Focussed

Therapy and Motivational Interviewing A qualification in substance misuse, social work, nursing and/or counselling

(Level 3 or equivalent)
