+What is the ASQ-3?■ ASQ stands for Ages and Stages Questionnaire

■ It is a reliable screening tool designed to be completed by parents or teachers, at any point, for a child between one month and five-and-a-half years old (for NAZ purposes, we will only complete the ASQ for scholars ages 1 month through three years [47 months] old). It helps determine if scholars are meeting developmental milestones at the pace of other children their age.

■ The ASQ provides information on a child’s general functioning in these areas

■ Communication

■ Gross Motor

■ Fine motor

■ Problem solving

■ Personal-Social skills

+Filling out the ASQ

Step One:

● Determine which questionnaire to use by calculating the Scholar’s age at administration.

● You can utilize the helpful “age calculator” on http://agesandstages.com/free-resources/asq-calculator/

+Filling out the ASQ

Step two:

● The ASQ has 30 questions that are divided into 5 areas.

● Explain each domain of the questionnaire before filling out :

○ Communication focuses on language skills; what the child can say and what they can understand.

○ Gross Motor focuses on large muscle movement and coordination such as crawling, walking, or throwing a ball.

○ Fine Motor focuses on small muscle movement and coordination such as holding a rattle or cutting with scissors.

○ Problem Solving focuses on how children play with toys○ Personal-Social focuses on the child’s interactions with toys

and other children.

+Filling out the ASQ

Step three:

● Have parents/caregivers complete the questionnaire (whomever completes it must have at least 15 contact hours per week with the scholar). It typically takes 10-15 minutes to fill out (per child), and 2-3 minutes for staff to score.

● If appropriate, read each question to the caregiver and rephrase questions when necessary.

● Encourage parents to try a skill with their scholars if they are unsure of the answer.

● Ensure that parents fill out the open-ended questions at the very end of the questionnaire.

+Scoring the ASQTo score a questionnaire, the parents’/caregivers’ responses – yes, sometimes, and not yet- are converted to points - 10, 5, and 0, respectively- and are totaled for each

developmental area.

+Scoring the ASQ

Adjusted score:If a question is left unanswered, you must calculate the adjusted score. You can do this by dividing the total domain score by the number of items answered in that area:

total area score ÷ number of answered items = average total score for that area

The number should be between 1-10. Add the adjusted score for that item to the scores for other items in that domain. This will give you the total adjusted score for the domain.

If two items are omitted, the the adjusted score is added twice to the other item scores.

+Scoring the ASQ

There is a score chart on the last page of each ASQ, where the total score of each domain is entered:

+ Scoring the ASQ■ Scholars whose scores fall within the white zone of the bar graph are

considered to be “on track” developmentally. NAZ calls this the “green” performance status.”

■ Scholars whose scores fall within the gray monitoring zone in any developmental area may require specialized activities and/or repeat screening (though these scores do NOT indicate a need for further assessment yet). NAZ calls this the “yellow” performance status.

■ Scholars whose scores fall within the black zone of the bar graph in ANY developmental area should be recommended for further, in-depth assessment. NAZ calls this the “red” performance status.

■ In addition to writing in the total score and filling in the corresponding circles, write the performance status next to each domain:

+What is the ASQ:SE?

■ ASQ:SE stands for Ages and Stages Questionnaires: Social-Emotional.

■ It is a reliable tool used to screen developmental progress in scholars ages three months to five-and-a-half years old in the social-emotional domain (for NAZ purposes, we will only complete the ASQ:SE for scholars ages three months through three years [47 months old]). It is designed to identify behaviors in young scholars that require more in-depth assessment.

● Determine which questionnaire to use by calculating the scholar’s age at administration.

Note: the age instance may be different from the ASQ-3

+Filling out the ASQ:SE

Step Two:

● The ASQ:SE has 22-36 questions that cover 7 areas of Social-Emotional development.

● Parents/caregivers will complete the questionnaire. It typically takes 10-15 minutes to fill out (per child), and 2-3 minutes to score.

● Give parents the independence to fill out this assessment. Only provide support when necessary. This gives parents the comfortability to answer questions honestly.

● Ensure that parents fill out the open-ended questions at the very end of the questionnaire.

+Scoring the ASQ:SEStep three:

Use the following formula to convert parents’/caregivers’ responses to

points:

● Each Z response = 0 points

● Each V response = 5 points

● Each X response = 10 points

● Each checked concern = 5 additional points

○ If a parent checks an item as a concern, inquire about the exact

concern and write it on the questionnaire.

Add up the points on each page and note the total in the designated space on the bottom of each page:

Adjusted score:

You must calculate the adjusted score if items on the questionnaire were skipped:If more than two items were skipped on the 6-8 month questionnaires, do not score the questionnaire.

If more than three items were skipped on the 24-60 month questionnaires, do not score the questionnaire.

In all other cases, you’ll need to provide an adjusted score. Do this by calculating the average score for the items that were answered, then assigning the skipped item(s) that average score.

+ Scoring the ASQ:SETransfer the total points on each page to the score chart on the final page of the ASQ:SE.

- A score below the cutoff is given a performance status of green. - A score above the cutoff is given a performance status of red.

Write the performance status next to the table as shown below:
