Leave longer on lesser curvature Stabilize the PTFE cuff to the aorta using gentle vascular pickups. Unscrew the green cap counterclockwise until released. Pull back the cap and connecting ePTFE suture to release and expand the braided stent. Once the stent is fully expanded and released from the delivery mechanism, the delivery system can be removed. Take caution to ensure the tip of the delivery system is free from the distal end of the stent. Secure the anastomotic site and seal the false lumen by running a continuous suture layer circumferentially around the transected aorta, attaching the inner AMDS felt cuff through the aorta and to the outer PTFE felt. Ensure no folding of the inner felt Perform the polyester graft to aorta-AMDS complex anastomosis with a continuous suture line. Ensure each suture bite incorporates the conventional polyester graft and the PTFE felt-aorta-AMDS PTFE cuff complex creating a meticulously tight seal between the PTFE felts and the dissected aorta. Pre-soak entire AMDS device and delivery system in saline Introduce AMDS into the true lumen through the open distal aorta until the PTFE cuff is in the same plane as the transected aorta. Retract the protective sheath to fully expose the PTFE cuff Secure the PTFE cuff to the aorta by placing uninterrupted sutures starting at 6, and following with 12, 3, and 9 o’clock positions. Use of a PTFE felt strip external to the aorta is necessary to avoid excessive tension or tearing.
