• The indication is given principally in the case of flexible juvenile pes valgus with open growth plate and growth potency of approximately two years. In addition the indication can be given for additive subtalar alignment in adults.

• DieIndikationwirdüberwiegendbeimflexiblenkindlichen Knick-Senkfuß mit noch zwei Jahren Wachstumspotential gestellt. Sie kann aber auch bei der additiven Rückfußaufrichtung beim erwachsenen Plattfuß angezeigt sein.

• Está indicado principalmente en los casos de pie plano valgo juvenil flexible con perspectivas de crecimiento de aproximadamente dos años. Además se puede utilizar en pie plano en adultos.

STEP 1 / 1 SCHRITT / 1° PASO

Incision

• The 1.5 cm long incision is made in the soft spot in the lateral area of the AS. Widen the approach to the AS with a small clamp.

• Der Hautschnitt erfolgt auf 1,5 cm Länge im soft spot, lateral im Bereich des AS. Mit einem Klemmchen wird der Zugang zum AS geweitet.

• La incisión cutánea - con una longitud de 1.5 cm - se realiza en la zona lateral del pie, y del AS. Se amplia el acceso al AS con una pinza pequeña.

Introduction of guide wire

• The guide wire is introduced from subtalar direction and checked with the imageintensifierafterwards.

• Der Führungsdraht wird subtalar eingebracht und anschließend unter dem Bildwandler kontrolliert.

• Whilecheckingwiththeimageintensifier,manual subtalar mobilization can be realized until the talus is repositioned in the correct axis. The distraction is definedaccordingtothesizeofthelastdilatator.

• Manuell kann dann eine subtalare Mobilisation erfolgen, bis unter Bildwandlerkontrolle eine achsgerechte Reposition des Talus erreicht ist. Entsprechend der Größe des letzten Weitungsstabes wird die notwendige Distraktion bestimmt.

• La movilización subtalar se realiza de manera manual,verificandoconelintensificadordeimágenes, hasta que se logre una posición correcta del astrágalo. La distracción necesaria será marcado por el tamaño del último dilatador.

Soft Tissue Preparation – AS

• The approach is prepared with the 8 mm Bouchier-Olive. Afterwards the AS is widened with dilatators in steps of 1 mm.

Weichteilpräparation – AS

• Mit einer 8 mm Bouchier-Olive wird der Zugang vorbereitet. Nach der Vorbereitung wird der AS in 1 mm Schritten mit den Weitungsstäben aufgeweitet.

Preparación De Las Partes Blandas – AS

• Se prepara el acceso con un dilatador inicial punta oliva de 8 mm. Después amplíe el AS con los dilatadores, aumento de un milímetro a otro.

Introduction of Screw

• The required vario screw is clamped into the impactor and introduced over the guide wire from subtalar direction. The screw is available in two sizes. The small screw allows widening from 10 to 13 mm, the larger screw from 13 to 16 mm.

Einbringen der Schraube

• Die Vario-Schraube der gewünschten Größe wird in das Setzinstrumentarium eingespannt und über den Führungsdraht subtalar eingedreht. Es stehen 2 verschiedene Schraubengrößen zur Verfügung, mit denen eine Aufweitung von 10 bis 13 mm oder von 13 bis 16 mm möglich ist.

• El tornillo vario subtalar del tamaño deseado se coloca en el impactor y se inserta a través de la aguja guía. Están disponibles dos tamaños de tornillos: el tornillo más pequeño permite abrir entre 10 hasta 13 mm, y con el grande entre 13 hasta 16 mm.

Spreading of the Screw

• In order to place the screw in craniocaudal direction,theflatsidesofthehandleoftheimpactor need to be positioned in parallel with the sole of the foot. Then the screw is spread up to the definedwidth,whichcanbereadontheimpactor.

• DasSetzinstrumentariummussmitdenflachenGriffseiten parallel zur Fußsohle stehen, damit die Schraube in cranio-caudaler Richtung zum Liegen kommt. Anschließend wird die Schraube bis auf die festgelegte Weite aufgespreizt. Die festgelegte Weite läßt sich auf dem Setzinstrumentarium gut ablesen.

• Los bordes planos del mango del impactor deben estar en paralelo con la planta del pie, para que el tornillo esté en dirección craneocaudal. Después se abre el tornillo hasta llegar el ancho requerido, lo cual se puede determinar en el impactor.

• Afterwards the impactor and the guide wire areremovedandfinallyreduction and mobility are checked.

• Setzinstrumentarium und Führungsdraht werden entfernt und zum Abschluss wird die Reposition und Beweglichkeit überprüft.

• Porfin,retireelimpactorylaagujaguíay además compruebe la reducción y la movilidad.

Optional Lengthening of the Tendon

• In this case the Achilles tendon does not display relevant shortening. For this reason lengthening is not necessary.

Optionale Sehenverlängerung

• Die Achillessehne zeigt keine relevante Verkürzung. Eine Verlängerung ist deshalb nicht erforderlich.

Alargamiento del Tendón Opcional

• En este caso, el tendón de Aquiles no muestra un acortamiento relevante. Por eso no es necesario efectuar un alargamiento.
