ELIGIBILITY GUIDELINES Demonstrated experience in schools Capacity to work PreK – Grade 12 Submit only one concise program model Provide evidence of Fed Tax ID/SS# Be aware of NPS fingerprinting,

insurance and contracting procedures

REQUIREMENTS Have a mailing address Have a phone or fax Have an email address Be aware that any potential contracts

that may exceed $36,000 will be required to submit paperwork to the PQS (Pre-Qualification Solicitation) at NPS

RESPONSIBILITIES Meet regularly together to share ideas Implement NPS arts evaluation tool Develop/Maintain accurate records Be flexible Be aware that participation provides no

guarantee of school requests
