Irritable bowel syndrome, or IBS, is the most common gastrointestinal disorder in the world. Fortunately, it’s also very treatable.

“IBS is characterized by a set of criteria called ROME,” explains THIPA President and Gastroenterologist Norman Panitch, M.D. “It’s named for Rome, Italy where a group of doctors met and came up with criteria to identify patients with certain gastrointestinal complaints.”

Do you have these symptoms of IBS?

If you have IBS you will experience a change in bowel habits.

“A change in bowel habits can include diarrhea or constipation that’s associated with severe abdominal bloating, gas and pain,” says Dr. Panitch. “This bloating and pain is usually worse during the day and can last for over three months a year.”

However, it’s important to know that a dramatic change in bowel habits can also be a sign of illness more serious than IBS, especially when you’re over 50.

“There are certain red flags that don’t occur in IBS,” says Dr. Panitch. “These include abdominal pain that awakens you from sleep, loss of appetite and weight

April is Irritable Bowel Awareness Month

Page 2: April is Irritable Soothing An Irritable Bowel · 2014. 4. 24. · Irritable bowel syndrome, or IBS, is the most common gastrointestinal disorder in the world. Fortunately, it’s

Osteoarthritis - often called degenerative arthritis is from wear, tear and overuse of joints.

Rheumatoid arthritis - is the most common type of inflammatory arthritis. It happens when the body’s immune system does not work properly.

Psoriatic arthritis - is inflammation of the skin and joints. Scaling skin gives way to swelling of the fingers and toes.

Gout - is caused by crystals that collect in the joints and cause pain and inflammation.

Lupus - causes painful, swollen joints among other problems. It happens when the body’s immune system does not work properly.

Unfortunately, you can’t diagnose joint pain on your own. If you’re suffering from any pain, talk to your primary care physician.

When your knees ache or your hips throb you can’t get to your medicine cabinet fast enough. But before you pop another over-the-counter or prescription pain pill, you need to ask yourself one important question…

How is this pain affecting my life?

“Sometimes pain medication or cortisone shots is all patients need for pain relief,” says Orthopedist Charles Turek, M.D. “But when patients see me and they’re limping or having trouble getting in and out of a chair then we take a closer look.”

Who Needs Joint Replacement

Dr. Turek looks at x-rays of the affected joint and assesses a patient’s functional impairment. That’s your ability to complete what physical therapists call activities of daily living.

“We need to have a history and a physical examination to give us an idea of how a joint is functioning,” says Dr. Turek. “Some people have minimal arthritis which doesn’t look too bad on x-ray, yet they have a lot of trouble getting around. Other people with terrible looking x-rays get around well.”

Dr. Turek treats some patients with knee or hip arthritis for years using nothing more than pain medication and injections before he recommends joint replacement.

Surgery and Recovery

Before joint replacement surgery a medical clearance is required from each of a patient’s doctors. Patients and their family members also take a class where a physical therapist explains what happens to the patient in the hospital.

“Speed of recovery really depends on the person’s motivation and their physical strength and coordination,” says Dr. Turek. “Once the patient is over the acute discomfort we send them to a rehab unit where they’ll get physical therapy twice a day. Rehab may last three days for younger patients or ten days for people in their 80’s or 90’s. We want the patient to feel that they are stable on a walker, crutches or whatever they’re using for the remainder of their recovery.”

The average patient may use a walker at home for two to four weeks before transitioning to a cane for another two to four weeks, according to Dr. Turek. Within three months most patients no longer need any assistant devices.

“Patients should not be afraid of this surgery or feel that they’re too old for it. There is no too old for this procedure,” says Dr. Turek. “Hip or knee replacement can make a very significant difference in seniors’ lives and their ability to function and care for themselves.”

What is an Annual Wellness Visit? As part of your Medicare Benefits, Medicare covers an Annual Wellness Visit to your primary care physician to improve the medical care you’re receiving. This is a new benefit that went into affect in 2010.

So it’s like an annual check up then?Actually, it couldn’t be more different than an Annual Physical Exam. First of all, there’s no copay. Secondly, its goal is to help your doctor better understand your medical history as well as your potential risk factors for illness. The Annual Wellness Visit is in addition to your Annual Physical Exam to keep you healthy.

Why do I need to have two visits if I’m not sick, isn’t it a waste of time?On the contrary, your Annual Wellness Visit will save you time over the long run and help you feel stronger and healthier. That’s because the more your doctor knows about your health status the better he or she can treat any health problems that you’re suffering from today or prevent new health problems from occurring in the future.

What happens during my Annual Wellness Visit?Your primary care physician will ask you to fill out a questionnaire called a “Health Risk Assessment.” Your doctor will ask you about your medical and family history and also about any new doctors that you’re seeing or prescriptions you’re taking. Your doctor will assess your risk factors for illness and provide you with a personalized prevention plan for good health. This plan will include any preventive services or screenings that you may need over the next five to ten years.

Answers to Your Questions About Your Annual Wellness Visit

What won’t happen during my Annual Wellness Visit?Your primary care physician will not ask about or discuss any new health problems that have not been already diagnosed. Nor will he or she conduct any tests or labwork. If you need to talk about new health problems, schedule another appointment.

What does the Annual Wellness Visit cost? You pay nothing out of pocket for this visit. However, you may incur a copayment or deductible expense if additional services are performed or ordered by your doctor.

When should I schedule my Annual Wellness Visit? You should schedule your visit every year. Call your primary care physician and make your appointment today.

Schedule your Annual Wellness Visit with you Primary Care Physician today and receive a $10 Gift Card from THIPA!

If you fondly recall how family practices used to treat three or four generations of the same family, then you’ll want to meet THIPA’s Dr. Bahar Hannah Zadeh.

Dr. Zadeh has an old-fashioned family practice on Skypark Drive in Torrance where she treats patients of all ages. “My passion is really preventive care,” says Dr. Zadeh. “I really value prevention as opposed to waiting for things to go wrong.”

Dr. Zadeh’s number one recommendation to her patients over the age of 65 is exercise.

“Exercise and staying active has been shown to be very beneficial to longevity and good health,” says Dr. Zadeh. “If you’re worried about high blood pressure and cholesterol or diseases such as Alzheimer’s, heart disease and diabetes then staying active physically and mentally is your best medicine.”

Physical activity is important even if you’re already suffering from an age-related illness, says Dr. Zadeh.

“Some people are a lot more limited by their physical ailments. In those cases I usually recommend swimming or walking,” explains Dr. Zadeh. “These are very safe in terms of not putting to much pressure on aging joints but at the same time you get the cardiovascular benefits and benefits of strengthening the bones and improving the mental state.”

MEET THE DOCTOR

Besides exercise, stress reduction is another recommendation Dr. Zadeh makes to her patients.

“Many of my patients are surprised to discover what a difference reducing stress through yoga, Tai Chi or other techniques can make in managing most physical ailments,” says Dr. Zadeh.

Dr. Zadeh also encourages her patients to eat a healthy diet and visit her on a regular basis.

“Seniors in particular need to make sure they see their primary care physician regularly,” says Dr. Zadeh. “Bring a bag of your medications to every doctor’s appointment. Most

important, don’t assume that your doctor knows everything about your visits to other doctors or healthcare providers. You need to ask questions and stay involved in your healthcare.”

You can reach Dr. Zadeh’s office by calling 310-784-6300..

INGREDIENTS:• 1 medium head

cauliflower, broken into florets, about 2lb.

• 1 lb. ground turkey• 6 oz. mozzarella, low

fat, shredded • 1 cup bread crumb• 2 tbsp. oil (canola,

olive)• 8 oz. Sour cream• 8 oz. Milk• 0.5-1 tsp. black pepper• ½ cup flour• 1 tsp. nutmeg

DIRECTIONS:1. Steam cauliflower florets in a cup water to preserve most of its

benefits. Don’t overcook it, because it will soften more in the casserole.

2. While the cauliflower is cooking; pan fry the ground meat at low heat in two tbsp. oil, sprinkle with black pepper, stirring frequently to prevent clumping.

3. In a bowl place ½ cup flour, sour cream, milk, 2/3 of the cheese, some salt and nutmeg, mix together.

4. Strain the cauliflower and place one layer in the casserole dish, spread the ground meat over, and place the remaining cauliflower on the top.

5. Pour the cheese mix on top evenly, sprinkle the breadcrumb over that and finish with the remaining cheese. Bake it in the oven at 330-350 degrees uncovered, for about 40 minutes. Internal temperature should reach about 180 degrees.
