APPOINTMENTS – CLINIC SCHEDULING PURPOSE: To provide clinic appointment scheduling information. To assure that patients are given appointments on the correct days and at the correct times. POLICY: Individual clinic appointments are attached. OPEN CLINICS: 1. Schedule the appointment in the computer; give the patient an appointment letter with the
clinic date and time.
2. Inpatient Discharge Templates are available for inpatient units to utilize when booking established clinic patients that are being discharged from the inpatient areas. Refer to the attached schedules for clinic specific instructions. The yellow clinic Appointment Chart Request Form shall no longer be utilized for inpatient discharge clinic appointments. If additional assistance is required when scheduling a patient, please contact the specific clinic area. If unable to book appointments in the computer prior to patient discharge, please contact the clinic manager on the next working day. The inpatient
unit shall be responsible for contacting the patient with the follow-up appointment.
CLOSED CLINICS: 3. For patients needing an appointment to a Closed Clinic, refer to the attached for clinic-
specific instructions.
4. Please include patient’s current home address and phone number on all consults.
5. All tests such as x-rays, ultrasounds, echo, mammograms, etc., shall be scheduled by the area from which the order originated.
6. Please call the appropriate clinic for additional information or assistance.

Appointments are scheduled per clinic staff only as directed by physician and clinic space availability. Appointments are only to be booked by Infusion Area personnel. New Patient Scheduling Instructions:
Physicians wishing to have patients treated in the Infusion/Recovery Area should submit clinic referral via Epic. These are reviewed by Case Management and approved by the Clinic Manager. Physician will need to provide orders for the treatment and be available by phone to handle problems that arise since there is no medical staffing present in the area.
Follow-up Patient Scheduling Instructions:
Follow-up appointments are scheduled and given to patient as they are leaving from their current appointment. Physicians are notified of patients who have missed appointment and designate when the patient is to return.

All dialysis access requests are to be scheduled with Vascular Surgery. 8) All Clinics: If the patient’s condition warrants a sooner appointment than next available, please have the consulting physician contact the physician service in the clinic.
