To The Director General Agri. (Research) Ayub Agri. Research Fasialabad Through: Proper channel Subject: REQUEST FOR GRANT OF MEDICAL LEAVE (DUE TO ACCIDENT) FOR 14 DAYS W.E.F 11-05-2015. Respected Sir, It is submitted that: 1. I was placed on temporary duty for 03 months in the office of Assistant Soil Fertility Officer, Rawalpindi which was completed on 08.05.2015. 2. On 11.05.2015, I was going to Jhelum (SF Lab) to join my place of duty. I was travelling with my brother in his car. After Sohawa, at 06:45 am, we met a road accident and car fell down the road by rolling many times. The car was totally destroyed and both of us injured seriously. People present there, brought us to the Govt. Hospital at Sohawa. After getting first aid we were shifted to Rawalpindi. 3. The “Medical Certificate” issued / countersigned by authorized medical attendant (at Sohawa) with advice of complete bed rest for 15 days, is being attached (copy). 4. Your good self is therefore requested to kindly grant leave on medial ground for 15 days w.e.f.11-05-2015. I shall be highly obliged and thankful for favorable action.
