     Required  Documentation:  Prospective   students   submitting   application   are   required   to   submit   copies   of   the   following   documents.    Applications  may  be  accepted  prior  to  their  receipt;  however  students  may  not  start  classes  without  submission  of  all  required  documents.

*  Be  aware  that  full  participation  in  the  program  is  a  requirement  for  satisfactory  progress  and  course  completion.    Any  condition  that  may  prevent  full  participation  must  be  fully  disclosed  prior  to  a  student’s  enrollment.  Illegal  use  of  drugs  or  alcohol,  or  drug  or  alcohol  abuse  during  school  is  prohibited.    The  use  of  drugs  or  alcohol  outside  of  school  hours  that  affects  a  student’s  performance  in  school,  is  also  prohibited.  It  is  at  the  State  of  Utah’s  discretion  to  allow  or  not  allow  those  with  a  felony  to  receive  their  license.  

   A  $50.00  non-­‐refundable  application  fee  is  due  at  the  time  of  application  for  enrollment.    Your  application  will  not  be  processed  until  this  fee  is  paid  in  full  unless  otherwise  approved  by  administration.  You  can  submit  your  application  and  fee  through  mail  or  fax.    Use  the  following  information  for  credit  card  information.
