Heart failure is one of the most important public

health problems in cardiovascular medicine.

There are 4.7 only in Americans with heart

failure and 400,000 new cases each year with

250,000 deaths and 75,000 strokes per year

attributable to heart failure.

Cardiac Ejection Fraction (EF) is a good index of

left ventricular dysfunction and low EF

(<=35%) is a risk factor for stroke in patients

with cardiac failure.

Conclusions• Among patients with reduced LVEF who

were in sinus rhythm, there was no significant overall difference in the primary outcome between treatment with warfarin and treatment with aspirin.

• A reduced risk of ischemic stroke with warfarin was offset by an increased risk of major hemorrhage.

• The choice between warfarin and aspirin should be individualized.

Pharmacological Treatment for

Stage C HFrEF (cont.)

The selection of an anticoagulant agent (warfarin, dabigatran,

apixaban, or rivaroxaban) for permanent/persistent/paroxysmal

AF should be individualized on the basis of risk factors, cost,

tolerability, patient preference, potential for drug interactions,

and other clinical characteristics, including time in the

international normalized rate therapeutic ration if the patient has

been taking warfarin.

Chronic anticoagulation is reasonable for patients with chronic

HF who have permanent/persistent/paroxysmal AF but are

without an additional risk factor for cardioembolic stroke (in the

absence of contraindications to anticoagulation).

I IIaIIbIII

[email protected]

Pharmacological Treatment for

Stage C HFrEF (cont.)

Anticoagulation is not recommended in patients

with chronic HFrEF without AF, a prior

thromboembolic event, or a cardioembolic source.

Statins are not beneficial as adjunctive therapy when prescribed

solely for the diagnosis of HF in the absence of other indications

for their use.

Omega-3 polyunsaturated fatty acid (PUFA) supplementation is

reasonable to use as adjunctive therapy in patients with NYHA

class II-IV symptoms and HFrEF or HFpEF, unless

contraindicated, to reduce mortality and cardiovascular

hospitalizations.

● It is reasonable for patients with STEMI who do not undergo reperfusion therapy to be treated with anticoagulant therapy (non-UFH regimen) for the duration of the index hospitalization, up to 8 days (Level of Evidence: B). Patients With Nonrheumatic Atrial Fibrillation (AF)

For patients with AF, including those with paroxysmal AF, who

are at low risk of stroke (eg, CHADS2 score = 0), we suggest no

therapy rather than antithrombotic therapy (Grade 2B). For

patients who do choose antithrombotic therapy, we suggest

aspirin (75 mg to 325 mg once daily) rather than oral

anticoagulation (Grade 2B) or combination therapy with aspirin

and clopidogrel (Grade 2B).

Remarks: Patients who place an exceptionally high value on stroke reduction and

a low value on avoiding bleeding and the burden associated with antithrombotic

therapy are likely to choose antithrombotic therapy rather than no antithrombotic

recommend oral anticoagulation rather than no therapy (Grade

1B). We suggest oral anticoagulation rather than aspirin (75 mg

to 325 mg once daily) (Grade 2B) or combination therapy with

aspirin and clopidogrel (Grade 2B). For patients who are

unsuitable for or choose not to take an oral anticoagulant (for

reasons other than concerns about major bleeding), we suggest

combination therapy with aspirin and clopidogrel rather than

aspirin (75 mg to 325 mg once daily) (Grade 2B).

Remarks: Patients who place an exceptionally high value on stroke

reduction and a low value on avoiding bleeding and the burden

Some patient subgroups receiving

rivaroxaban experienced increases in

bleeding compared with those receiving

warfarin. These included patients aged

≥ 75 years, and those with prior MI or PAD.

It is important that these factors are

considered when selecting the optimal

therapy for individual patients, but not at the

cost of offsetting important reductions in

other adverse clinical outcomes.

¿ QUIEN NO?

nuevos anticoagulantes ofrecen mejor eficacia,

fibrilación auricular no valvular

La evidencia disponible es insuficiente para recomendar

alguno de ellos en particular sobre otro, aunque algunas

características del paciente, cumplimiento y

tolerabilidad a la terapia, así como el costo, pueden ser

importantes consideraciones al momento de elegir el

nuevo anticoagulante
