INTRODUCTION, address of the Chairman

The highlight of 2007 was undoubtedly the succesfull “Dwoika” three week visit in the period October/November. In this exchange visit we had two enthousiastic junior medical Specialists in ENT and in Gynaecology over to the Netherlands. We were fortunate to have Scheper Hospital Management prepared to offer their facilities for the two attendants. Healthy Lifestyle (HLS) and Psychiatry Working Groups completed their projects after having invested considerable effort and time in their projects over the last years. In November/December 2007 a special DEMMA Delegation, together with Scheper Hospital CEO, paid a visit to Mytishchy, in order to establish a bond between Emmen and Mytishchy Hospitals at Management level. We welcomed a new young DEMMA member who will support us in our future activities. As mentioned in earlier reports and in various minutes, the climate of Sponsoring has further deteriorated in The Netherlands.Demma as a consequence is digging deeply into her financial reserves. It is therefore of vital importance for DEMMA to make herself more known to the outside world and to potential Donors.

MEETINGS, PROGRAMMES and EXCHANGES in 2007:

Demma Foundation have met four times in 2007, namely on January 15th, April 2nd , August 6th and on 12th November 2007 mainly in Scheperziekenhuis in Emmen. Three Exchange Programmes were run in 2007:

DEMMA SPRING 2007 VISIT Treatment and Care of

persons addicted to Drugs and Alcohol in The Netherlands 21-27 April 2007

This first exchange programme in 2007 focussed on Drugs and Alcohol addiction treatment and was an activity of the Psychiatry Working Group. Earlier, in the fall of 2006 a delegation of DEMMA had visited various Psychiatric and Addiction treatment units in Mytishchy and in Moscow. Visitors in 2007 were Dr.Vladislav Kalyagin, head of the Alcohol and Drugs Unit in Mytishchy Hospital, Dr.Valentina Korshikova, former head of Psychiatry Department, Mytishchy Hospital; now working for the Moscow Region and the third guest was Dr Alexander Donhenko, deputy of Dr Kalyagin. The programme enhanced a visit to D&A Clinic, Vondellaan in Groningen, to Duurzaam Verblijf, a rehab unit just open in Beilen. Furthermore Jellinek Clinic in Amsterdam and GGZ in Assen were visited. The exchange visit was very succesfull and it was for the participants a very busy period as well. Fortunately the weather was very favourable, which enabled the WG to show our Russian vistors Friesland and Amsterdam on “their best”

In this traditional DEMMA exchange programme, two clinical specialists from Mytishchy came over to Drenthe to familiarise themselves with the working environment of their reciproke Dutch DEMMA colleagues. Visitors this time were Dr.Elena Sarancha, ENT specialist – daughter of Dr.Ludmilla Sarancha, also ENT specialist who has participated in earlier programmes of DEMMA Foundation in the nineties. The second visitor was Dr.Wladimir Davidyuk, a gynaocologist. In the three weeks the Russian colleagues were based in Scheper Ziekenhuis in Emmen, that kindly offered to host both doctors in their facilities. Other than facilitating in familiarizing themselves in Scheper Hospital ENT and Gynaecology Departments, the programme included visits to an Audician Center, Childrens Audio screening Centre, UMCG Teaching Hospital in Groningen. But there were also visits to a Musical, Groninger Museum, and traditional Friesland and Amsterdam tours. DEMMA Foundation are greatfull to Scheper Ziekehuis Management for their subsantial contribution in this very succesfull exchange programme.

Elabuga, Tatarstan visit 22-27 August 2007

An invitation was received from Valery Yanin -Secretary of the DEMMA-M organisation- and of Mrs. Olga Motyakova, Head of Doema Administration at the Kremlin in Moscow, to attend the Elabuga, Tatarstan Millenium Celebrations. An International Symposium was to be held and speakers from abroad were invited to contribute in presentations on International Cooperation at the level of cities and communities. It was an excellent opportunity to explain to the audience the national organization in the Netherlands “Vereniging van Nederlandse Gemeenten” VNG and on her experiences with various citybonds and city links. A large national study was recently completed in The Netherlands and it contained many lessons and parallels very much in line with experiences within DEMMA over the last 14 years of her exchanges with Mytishchy. Guus de Groot and Kees Tuinman were the attendees. A sponsorship was received from DEMMA. The occasion of the visit to Tatarstan was very unique . Both visitors were received with warm hospitality. The festivities were for the population at large with a great number of Officials and Dignitiaries. The Sessions were opened by the Minister of Economic affairs of the Russian Federation. Hosts were the President of Tatarstan with the Mayor of Elabuga. The international visitors and local VIP’s were housed on a large Wolga passenger liner, moored alongside Elabuga jetty, wich gave the event an additional maritime touch. Guus de Groot, Kees Tuinman

As agreed in 2006 with DEMMA-M during the 2006 Spring visit, it was deemed appropriate to have an exchange visit with Mytishchy at a Management Level.

Participants were Peter Windt, CEO of Leveste, Emmen; Edzard Braam, GM and surgeon of Braam Clinic in Assen and DEMMA Member; Jan Burgggraaf, Vice-Chairman of Demma and Kees Tuinman, Secretary.

During a compact programme, Mytishchy Hospital was visited, both new and old facilities, A Haematology Centre; “Andreevskiye Bolnici “( a privatetely run medical centre), and FIALKA Institute. There was ample opportunity to exchange experiences between Managers at both ends. Host was Gennady Proskurnicov, Director of Mytishchy Hospital. In spite of the Historical, Political and Cultural varieties between The Netherlands and Russia it was good

to see that Managers too –as Doctors do- have very similar issues and problems. An extra thank you was for Valery Yanin and his family who kept the vistors busy and acted as superb hosts and guides.

DEMMA PROJECTS WORKING GROUP REPORTS Healthy lifestyle Working Group:

The working group Healthy lifestyle supports Demma-M since 1999 in her introduction of sexual education at secondary schools in Mytishchy. These activities were funded in the NSPOH-project of the Ministry of Foreign Affairs of the Netherlands. This project initially was closed off on 10 June 2005, but the DEMMA working group managed to continue to get finances from the DEMMA Foundation during the term of the project, through which it was possible to continue working with Demma-M The balance of the progress made was discussed at a meeting of Demma in Diever, at 25 June 2004. In that meeting it was agreed that all knowledge and didactics required for the process were made available now to Russia, and in Russian language. DEMMA Healthy Lifestyle WG have succesfully contributed in the introduction. Further implementation and continuation of these activities would be at the discretion of the local communities and schools. In Diever it was agreed that a co-ordinator of the project in Mytishschy was to be appointed, who would focus on the introduction and maintenance of sexual education at schools in Russia. This co-ordinator would receive payment from DEMMA. As such in February 2007, Demma-M contracted Dr.Natalia Zublova as co-ordinator Healthy Lifestyle. Her area of expertise is Clinical Research. Dr.Zublova organised various meetings for Teachers and Physicians and gave lectures on sexual education subjects in schools and and in youth-centres in the Moscow Region. Additionally, the Working Group has promoted the purchase of education materials such as brochures and flyers for Mytishschy Schools and has donated an additionally 1500 euro’s to enable the purchase of 300 books on Family Planning, written by Sanderijn van der Doef and the translation into in Russian.

Psychiatry Working Group /Alcohol and drugs

In November 2006 a Regional Symposium on the treatment of Alcohol and Drugs addicted patients was held in Mytishchy , Moscow Region. Presentations were made by Russian Specialists and by invited DEMMA members Wim Elting and Kees Tuinman. In the various discussions during the Symposium a keen interest was shown in the Dutch model of addiction care. Consequently a reciproke invitation was made to interested Russian colleagues to come and visit The Netherlands/Drenthe in 2007. The invitation was positively received. As a result the DEMMA Spring visit in April of 2007 took place and was very succesfull. It is of vital importance, in the set up of exchange programmes, to be aware of the running issues, interests and questions availing at “ the other end”. The Psychiatry Working Group will continue her activities as soon as there is more insight in above mentioned issues and questions in Mytishchy and in Drenthe.

General Practitioner (GP) project

Since last year, considerable changes have taken place in Mytishchy following the introduction of General Practitioners in the Health Services. In Russia one is used at having direct acces to all (State) Medical Specialists in Policlinics of most General Hospitals. The introduction of a “ Generalist” such as the GP, was initially met with reservations. Dr.Semen Zuev who earlier had visited and worked in the GP practice of Dr.Coen Holzhauer is now a very busy GP in Mytishchy. It appears that in Russia there is a need for different types of GP’s in rural or for the more urban areas. There are indications for a further need to organise exchanges of GP’s at both ends, in which DEMMA is ready to contribute.

Occupational Health project

During a number of visits to Mytishchy, often simultaneously with other ongoing projects, options were explored to establish exchanges with Russian Occupational Health Physicians. In Russia however, the demarcation between Public Health and Occupational Health is less

outspoken as in The Netherlands. Elements of Medical Insurance, Medical retirement, applications for and recognition of Occupational

Illnesses are interwoven in this area of Health Care. Epimemiology, Clinical research and even diagnostics may take place in Russian Occupational Health Centres. Above Institutes, of which the Erismann Institute in Mytishchi is leading in Russia, are only remotely involved with training of (Occupational) Physicians. Erismann Institute Staff were twice visited over the years.The Phenomenon of independently appointed Physicians within factories, Offices or in large enterprises are also less common in Russia compared to e.g. in The Netherlands. Demma will maintain an interest in young physicians, working in Private enterprises in the area of

Preventative Health and facilitate in the arrangements of exchange programmes in the future.
