“What’s in a name?”...Quite a lot in fact.

Our decision to say goodbye to thename Trinity Home Hospice anddevelop our new identity as “HospiceToronto” came following a lengthyprocess this year that involved strate-gic planning, community consulta-tion, and lively discussions with ourboard, staff and volunteers. Weemerged from the process committedto our new name, reflecting thediversity and geography of the com-munity we serve; through our choiceof a mandala-based logo we wereable to reinforce the history and values of our Hospice. In Sanskrit theword mandala means circle. It is saidthat these archetypal templates, withno two alike, are created to remindus of the cycle of life and death. So ina way the Hospice has come full circleadopting the name Hospice Toronto;a derivative of our original nameincorporated in 1988 as “TrinityHospice Toronto”.

Changes this year began with ournew identity but certainly did not end there. We have demonstratedour commitment to facilitate accessto a full spectrum of palliative careservices for our clients while avoidingduplication (articulated in ourrenewed mission statement) throughthe enhancement of our existingprograms and services, as well asthrough an evolving collaborationwith our community partners.

Celebrating the unveiling of our new name andlogo at our annual “There’s No Place Like Home”event provided a wonderful opportunity toreflect upon this past year.

Internally, we have accomplished this through:• expanding our “Kit for Kids”

children’s program to includeExpressive Arts groups in particular,a group called “By My Side” forchildren with a life threatening illness and their families or for parents with a life threatening illnessand their children, recognizing theimportance of facilitating thisprocess for the family unit.

• a multi-year grant from the OntarioTrillium Foundation; we are devel-oping a fund raising infrastructureto support our growing funddevelopment activities. This grantwill enhance our ability to continueto diversify our revenue sources,reducing our dependence on government support. In addition tothis, it will enable us to redirect ourfront line resources to work towardour goal of tripling our volunteerrecruitment and training by theend of our 2005/06 fiscal year.

One of our most rewarding experiences this year was beingapproached by students from NorthToronto Collegiate to make ourHospice the subject of their submis-sion for a Toskan Foundation award.In meeting with these four grade tenstudents we were able to explain thephilosophy behind the provision ofhospice care and to give them insightinto the scope of the services we provide. The play developed by thestudents to demonstrate the “HospiceToronto Experience” later won anaward and a donation of $5,000 forthe Hospice, but most exciting for uswas the opportunity to provideawareness to students in high schoolabout “hospice”, a goal we have longwished to achieve. Currently, we areworking with the students to securesome funding for them to “take theirshow on the road” as an outreachproject for Toronto high schools.

The next few years will seeextraordinary changes in the healthcare system in Ontario. We havealready begun to see this in theestablishment of the new LocalHealth Initiatives Networks (LHIN’s).Hospice Toronto continues to be acommitted, active and responsive participant in this process.

The past year has brought significant changes to the Hospiceand we are excited about the futureopportunities for Hospice Toronto.However, the heart and soul of whatmade Trinity Home Hospice will carry forward and always remain apart of who we are.

In addition to this, enabling us toprovide broad access to services notdirectly delivered by our Hospice, wehave worked hard to develop collabo-rative relationships with a variety oflocal organizations:

Gilda’s Club: Now provides space andreferrals for our Expressive Artsprograms for children and adults whoare ill, their care providers, andindividuals who are recently bereaved.In addition to this, to supplement thelimited physical space at our Hospice,Gilda’s Club is also now providingspace for our Hospice’s ongoingvolunteer training activities.

Casey House: Recognizing the increas-ing need to support individuals livingwith HIV/AIDS who are stabilizedthrough medication or are in remis-sion, our Interim Support Program hasintroduced a pilot day program providing Expressive Arts Therapy in a group setting and hosted by CaseyHouse. This has enabled us to expandour outreach and service delivery toour clients living with AIDS.

The Society of Sharing: To compli-ment the in-home volunteer basedservices provided through HospiceToronto, we have worked toward formalizing a referral relationshipwith the Society of Sharing. Thisorganization works with volunteersto provide services which include grocery shopping, transportation tomedical appointments and friendlyvisiting, a perfect compliment to themore intensive, practical, “end oflife” care provided by our Hospicevolunteers. This has become a won-derful resource for our clients whoare in remission and a supplement toour Interim Support Program.

The Gitche M’Qua Centre for Healingand Dying: Following a pilot this yearto explore the potential for caregiverssupport, we are now working towardthe development of a “care for thecaregiver” plan for volunteers andprofessionals, in cooperation with thisorganization.

MISSION STATEMENT

We provide skilledcompassionate careto people with aterminal illness whochoose to die athome. Volunteersworking togetherwith the client’sfamily and friendsgive support thatenhances thequality of their lives.

Volunteer Services has continued todevelop new training modules inresponse to the Hospice’s growth anddiversification. In the fall, our profes-sionally qualified ComplementaryTherapists will attend a training module designed to support theirwork with Hospice clients. Anothernew training and orientation moduleis being designed for the volunteersinterested in assisting the Hospicewith outreach, event planning andorganization.

In the Spring and Fall, currentCare Team volunteers who would liketo enhance their skills and work inour “Kid for Kids” program will begiven a further 21 hours of trainingto learn the skills necessary to supportgrieving children.

Volunteer ServicesThe Care Team training continues

and 50 compassionate, dedicated and eager new volunteers will havereceived 30 hours of training by theend of 2005. From April 2004 toMarch 2005, the last fiscal year, CareTeam volunteers donated 19,000hours of work. To quantify thesehours, such dedication would be valued at approximately $380,000.

When we total all our volunteeractivities; the Board, committees,administration volunteers, funddevelopment and all our direct careprograms, the number of hoursdonated by the volunteers is equiva-lent to 14 people working full time.

The Care Team program is our coreservice. Through the care teammodel we provide support to ourclients in their own homes, offeringcomfort, security and peace of mindto the person who is dying andrespite to their family and friends.

During the past year we haveseen a significant rise in our clientreferrals from several sources. Wecontinue our good connections withmany of the downtown hospitalsand clinics including PrincessMargaret Hospital, St Joseph’sHealth Center, the AmyotrophicLateral Sclerosis (ALS) clinic atSunnybrook & Women’s CollegeHealth Science Centre, and theHospice Palliative Care Network(HPCNet). A week taken at randomshowed fifty three clients with CareTeams, ten receiving Interim

Support, three in the BereavementSupport Program, and nine waitingto be assessed for a total of 75clients. The majority of our clients,86%, have cancer then HIV/AIDS,ALS, the end stages of MultipleSclerosis and cardiac disease accountfor the remainder.

The “A” team model of care,meeting with and scheduling familyand friends into a care team, contin-ues to be an integral part of HospiceToronto’s work, and during the pastyear several “A” teams, one as largeas 40 members, sustained clients formany months following them intohospital and home again as needed.

Hospice Toronto, volunteers and staff continue to strive forexcellence in the holistic approachto palliative care in all areas.

Operating Statistics

Fund DevelopmentFundraising is a challenge shared byvolunteers, staff, and Board membersof Hospice Toronto. Talking aboutend-of-life issues is never an easy task,yet we all need to have the courage toraise this topic in our discussions withour donors and supporters at fundrais-ing events and through direct appeal.We engage our donors and share withthem our enthusiasm for the fine community work that is carried out byour caring volunteers, as they interactwith individuals facing life’s most difficult challenge. We relate manystories to our donors to help themunderstand how much their financialsupport has made a difference to thelives of our clients and their families.

As Hospice Toronto has grown,funding from Government sources hasnot kept pace. We have been very successful in raising community awareness and soliciting funds from a broader donor base. In 2004 – 2005,

Hospice Toronto is proud to provide awide range of ComplementaryTherapies to our clients from Hospicevolunteers who are also certified andtrained complementary therapists in avariety of modalities. Our volunteersoffer the following treatments;Massage Therapy, Reflexology,Therapeutic Touch, Healing Touch,Reiki, Soundwork, Music Therapy andLegacy Work (recording a client’s biography through words and pictures).

Complementary Therapyof positive benefits from theComplementary Therapy treatmentsincluding relaxation, assistance withsleeping, and an increased overallsense of well-being.

The program is one of our fastestgrowing with more than 50% of ourclients benefiting from our profes-sional volunteers’ expertise. Quiteoften the complementary therapist isthe first introduction to the Hospicefor the client, obviously a successfulone as more and more requests arereceived for these treatments.

The Kit for Kids program was bornout of a desire to meet the needs ofchildren, either living with a lifethreatening illness or who have afamily member living with a lifethreatening illness. For the past sixyears Hospice Toronto has evolved itsexclusive training geared towards volunteers that wish to work withchildren. These volunteers have completed the 30 hours of volunteertraining and have then continuedwith a further 21 hours training towork with children. The program

Kit for Kids Programprovides a safe and non-judgementalplace for the child to explore andexpress their feelings. Play, which maysometimes be forgotten, is highlight-ed through shared activities and itcan bring a sense of normalcy to thechild’s life. We have also piloted anexpressive-arts-based anticipatorygrief and bereavement group for children this year that has been verywell received. Hospice Toronto continues to strive to come up withinnovate and creative ways to meetthe needs of children.

Hospice Torontocontinues tostrive to come upwith innovateand creative waysto meet theneeds of children.

The Bereavement Support Program isa vital part of the continuum of sup-port provided by the Hospice becauseit acknowledges that hospice caredoes not end with the physical deathof the client. Experience has shownthat after the death of a loved one,the needs of the family and friendsmay continue.

Although there are many commu-nity bereavement groups in the citythat caregivers can turn to, thesegroups require a three-month waitingperiod between the death of a clientand admission to membership in thegroup. The Hospice’s bereavementprogram is available to caregivers following the death of a Hospiceclient and therefore bridges this gapby allowing them more immediateaccess to critical support during theirtime of grief.

A small, dedicated group ofbereavement support volunteers provide emotional support for up tothree months following the death ofthe client. While the volunteers arenot professional bereavement coun-selors, they are someone with whomcaregivers can talk openly, share stories and memories, and mostimportantly, someone who can be acompanion in the grieving process.Caregivers and volunteers can meetover a cup of coffee, go for a walk, ortalk on the phone. At the end of thethree-month period, volunteers canassist caregivers with information andreferrals to formal communitybereavement groups or professionalcounselors.

Hospice Toronto’s Executive Directoridentified some challenges associatedwith the growth and success of theHospice. Over the past few years theintroduction of event – drivenfundraising and the increase in thenumber of clients and services thatthe Hospice offers, has meant thatresources were being stretchedbeyond the capacity of both staff andvolunteers.

In an effort to ensure that the volunteers trained especially for careteams, interim support, bereavement,and the children’s work were able todo their direct care work, two issueswould need to be addressed. The first step would be the hiring of afundraising consultant to analyze thepresent process and assist with thetransition to a new fundraising soft-ware program. Then volunteers wouldneed to be recruited to support specialevent activities and more care teamvolunteers trained to ensure that theapproximately 90% of Canadians whowould prefer to die in their ownhomes had that choice.

Hospice Toronto continues togrow and to diversify its source ofrevenue beyond government support

Hospice Toronto gratefully acknowledges the financial support of The Ontario Trillium Foundation, an agency of the Ministry of Citizenship, Culture and Recreation. With $100 million in annual funding from the province’s charitable gaming initiative,the Foundation provides grants to eligible and not-for-profitorganizations in the arts, culture, sports, recreation, environmentand social service sectors.

but the challenge of growth meansthat a more effective volunteerrecruitment strategy is required. The infrastructure to train, supportand supervise volunteers must becontinually improved and developed,and volunteers with the expertise toheighten awareness about hospice,organize and volunteer at fund raising events need to be trained.

The Hospice submitted a successfulgrant application to the OntarioTrillium Foundation to address theseconcerns and fund some solutions. It means a Fundraising Consultant,Special Events Coordinator and aAdministrative Coordinator will behired. Over the three year period ofthe grant 75 volunteers will betrained for funding diversification,development and outreach initiatives.By the third year, revenues fromspecial events will have been increasedby 35% and approximately 140 newcare team volunteers trained.

The Ontario Trillium Foundationgrant ensures that our clients will continue to receive the spectrum ofservices and care given by committed,well trained and supported volunteers.

In our continued effort to providesupport to bereaved families as wellas families experiencing anticipatorygrief, the Expressive Arts Programwas introduced this year. Expressivearts therapists encourage individualsto work in various modalities such aspaint, pastel, clay, voice, drums andpoetry. This allows access to parts ofthemselves they may not otherwisebe in tune with. It also encouragesthe expression of deep inner feelingsand thoughts. Working through thearts is non-threatening and so it is especially helpful when working withchildren. It enables them to acknowl-edge and move through the difficultemotions being experienced duringor after a critical time in their lives.

In February of this year, a numberof groups were piloted in collabora-tion with Gilda’s Club of GreaterToronto. At their facilities we havenumerous groups including: children’sgroups for recently bereaved children,anticipatory grief groups for thosewho have a parent living with a lifethreatening illness, and a group foradults who themselves have the disease. A quilting group called “By My Side” for children with a life

threatening illness and their familiesor for adults with a life threatening illness and their children provides anopportunity for sharing memories,experiences and feelings.

Our continued commitment tothe HIV/AIDS community has beenfurther enhanced with a weeklyexpressive arts therapy group, in collaboration with Casey House andheld in their facilities. This has been avery successful group that was initiallyfor residents but is now being offered to the larger HIV/AIDS communitythrough Casey House’s external clients.

In the fall of 2005, we want toaddress the unique needs of “caregivers” and add two expressive artstherapy groups; one for families andfriends, and one for health care professionals.
