The majority of people at risk for CKD - 60% of patients with diabetes and 90% of patients with hypertension - do not receive recommended testing for CKD. Over 80% of the 37 million Americans with CKD remain undiagnosed in primary care, including almost 50% of people with advanced CKD. These breakdowns in testing and recognition prevent opportunities for early intervention and timely nephrology referral, which has been shown to improve downstream outcomes and reduced costs of care. In partnership with the National Committee on Quality Assurance, the National Kidney Foundation (NKF) has developed the Kidney Health Evaluation for Patient with Diabetes HEDIS measure. The goal of this measure is to improve CKD testing among people with diabetes. This session discusses the current state of CKD testing and the implications of the Kidney Health Evaluation measure for improving CKD care in primary care. This session will also provide a primary care clinician’s perspective on the implementation of a community-based CKD program and the impact of measures designed to improve CKD testing.
