Self / Pre-Assessment Documents Interested companies are requested to refer to the Applicant Guidelines and send the following documents along with the Self / Pre Assessment Documents as per details below. l Before & after "5S" photographs l 5S manual, if any l Organization structure for 5S l 5S vision statement and objectives l Waste disposal procedures l Implementation steps adopted Tangible and intangible benefits out of 5S initiatives CII would conduct pre-assessments based on the provided documents prior to the final assessment through site visit. This will enable the applicant organization to understand the requirements and address all gaps before the final audit. Refer the Applicant Guidelines. Guidelines to the Applicant while conducting 5S assessment for 5S Excellence Award
1. Last Date for Submission of documents : Friday, 26 August 2016
2. Audit Dates: Make sure that you adhere the dates confirmed sent by CII. Kindly do not change the audit schedule without prior intimation to CII office
3. Audit Duration: September - early October 2016 (Exact Date will be announced later)
4. Award Ceremony: End October - Early November 2016
5. Audit timings: The timing for the audit is normally between 0930 hrs to 1730 hrs and to be covered in one day
6. Please prepare the agenda and send it to assessors, well in advance
7. During the Opening Meeting, keep all documents such as 5S Manual, Cleaning Checklists, Audit Checklists, Red Tagging Procedures, Waste Disposal Policy and Document Retention Procedures ready for examination
8. Please make your 5S Journey/practices presentation for 1 hr only
9. The assessors will decide to conduct combined audit by both assessors or split into 2 streams depending on the area and the number of zones to be covered
10. Facilitate the Assessors to meet the factory manager or head of the unit to understand the role, commitment and knowledge of Top Management in 5S implementation. Assessors may also interact with employees informally and make an assessment on their knowledge and involvement in the 5S activities
11. The audit will be covered all areas/zones of the factory and ensure you make a thorough assessment
12. In the closing meeting, a general feedback on the 5S audit will be given. Please do not ask any assessment feedback in writing or the scores with the assessors. The written feedback report will be sent by CII
13. The actual costs of the visiting Assessors would have to be borne by the applicant company. Please get in touch with the assessors for travel and local hospitality details. The following are to be provided to the assessors:
a. Assessor travel (by train or air, whichever is applicable). CII will intimate the schedule for enabling for booking tickets
b. Local conveyance, local stay and hospitality arrangements c. Professional fee of Rs. 5000/- per assessor to be paid directly on the day of the visit
14. In case, if you need any receipt/invoice request the assessors to present it before the audit date
