INTERNAL MEDICINE ROTATION PROGRAM

LENGTH OF ROTATION

45 working days

The rotation program will be applied twice to both Turkish and English programs.

LOCATIONS OF EDUCATION

Theoretical lectures will be held at the Bilkent Health Sciences Building.

Practical lectures will be held at inpatient and outpatient clinics of Internal Medicine Department at Ankara

Ataturk Training and Research Hospital.

MEASUREMENT AND EVALUATION

Upon completion of rotation students will be evaluated with two methods.

1) Written Examination: Multiple choice, open-ended, true-false or complementary questions will be

applied in this exam. The goal is to measure the knowledge of the students. The written examination

threshold is %50. Students who pass the threshold are entitled to enter practical and oral examination.

2) Practical and Oral Examination: There will be two or three faculty member in each exam

committee. Students will be evaluated based on their skills in medical history taking, conducting

physical examination, making differential diagnosis and appropriate management of patients.

The effect of these exams on rotation grade will be 50% and 50% respectively. Based on rotation grades students

will pass or fail.

AIM OF ROTATION

1) The aim of this rotation is to have students achieve the appropriate education and skills of diagnosis

and management of most common basic and emergent medical diseases based on National Core

Educational Program.

2) To train medical students as ethical, inquisitive researcher physicians with multidisciplinary

approach and effective communication skills.

EDUCATION GOALS

At the end of Internal Medicine rotation students expected to learn;

1) Inquiring the patients and families/caregivers properly

2) Evaluating the general medical status of patients

3) Assessing and monitoring the vital signs of patients

4) Assessing and evaluating the anthropometric measures of patients

5) Conducting systematic physical examination

6) Documenting and presenting the medical history of patients and physical examination findings

7) Ordering the basic laboratory tests (complete blood count, urinanalysis, biochemical tests,

peripheral blood smear, EKG, basic radiologic images) as a first step based on history and physical

examination findings, analyzing the results properly, able to diagnose most common basic and

emergent medical diseases and able to manage and treat these conditions in the primary healthcare

setting.

8) Identifying the diseases that require emergent interventions, being able to refer them to secondary

healthcare setting after stabilizing them with appropriate respiratory and circulatory support.

9) Communicating effectively with patients and caregivers/families.

10) Communicating effectively with other healthcare workers

11) Communicating effectively with trainees

12) Understanding the importance of good communication between physician-patient, physician-

physician and improving the communication skills.

ASSESSMENT AND EVALUATION

Students will be evaluated in 2 exams at the end of the clerkship

1) Practical and Oral Exam: Examination jury made of 2 or 3 lecturer will evaluate students’ skills in

taking anamnesis, doing physical exam, making differential diagnosis and arranging treatment.

2) Written Exam: Multiple choice, open ended, true-false or fill in the blanks types of questions. This

exam is towards measuring student’s knowledge.

The effect of these exams on clerkship grade will be %60 and %40 respectively. Student’s status will

be stated as passed/failed according to the grade.

AIM OF THIS CLERKSHIP

The aim of this clerkship is to teach students necessary knowledge and skills considering patient evaluation in

surgical perspective, taking anamnesis, treating or referring patients properly in respect of surgical diseases of

the gastrointestinal and endocrine systems, diseases of breasts, surgical emergencies and traumatized patient

evaluation.

OBJECTIVES

At the end of this General Surgery Clerkship, 4th year medical students will;

Knowledge:

1) Explain the process of endocrine response to trauma

2) Summarize the biology of wound healing

3) Understand acid-base balance and liquid-electrolyte treatments

4) Tell asepsis and antisepsis applications used in surgery

5) Learn gastrointestinal system diseases that require surgical treatment and symptom and findings of their

treatments

6) Learn endocrine system diseases that require surgical treatment and symptoms and findings of their

treatments

7) Be able to define diagnostic laparoscopy and laparoscopic surgical diseases and their complications and

manage burn patients

8) Determine the differences between blunt and penetrating abdominal traumas

9) Define hemostasis, blood transfusion and its complications

10) Understand surgical treatment of malign melanoma

11) Learn breast diseases, their symptoms, findings and treatment

12) Count down referral criteria of surgical patients

Skills

1) Do normal and acute abdomen examination

2) Diagnose acute abdomen in a patient with abdominal pain

3) Be able to order necessary workup in a patient presented with mass in breasts and put cancer diagnosis

4) Diagnose benign diseases of the breasts and learn necessary treatment plan and referral criteria

5) Learn management of trauma patients

6) Manage patients presented with shock after necessary pre-evaluation.

7) Diagnose abdominal wall hernias

8) Learn how to apply wound dressings, suturation and suture removal

9) Apply wound care

QUANTIFICATION AND CONSIDERATION

The students are assessed with 2 methods at the end of internship

1) Practical and Oral examination: The students' skill in history taking, physical examination, making

differential diagnosis and ordering treatment are evaluated at the exam performed by the faculty members.

2)Written examination: It is a multiple choice, open-ended, right-wrong and classic written exam that

is directed to measure the knowledge of students.

The impact of these examinations on internship notes is 60% and 40% respectively. The status of students

are determined as successful or failed according to the internship notes.

AIM OF THE INTERNSHIP

To bring phase IV students in up-to-date informations about the etiopathogenesis, clinical and laboratory

findings, diagnosis and treatment methods of cardiovascular system diseases. At the same time, to learn the

evaluation of patients as a whole by understanding the effect of cardiovascular diseases on the other systems of

the body and vice versa. In addition, to bring in basic communication skills with the patients and their relatives.

THE LEARNING OBJECTIVES FOR THE INTERNSHIP

1) To be able to take medical history of patients composed of complaint,background,medical histories and system

inquiry reliably and in detail.

2) To be able to learn and use all basic physical examination methods (inspection, palpation, percussion,

oscultation) in patient's examination.

3)To be able to initiate and sustain a friendly and effective relationship between the patient and doctor and to

perform the ability to provide patient-doctor relationship.

4)To be able to create pre-diagnoses based on history and physical examination findings.

5) To be able to select purposive diagnostic tests

6)To be able to make differential diagnoses by associating the findings obtained from history, physical examination

and diagnostic tests with the physiopathology and clinical science knowledge.

7)To be able to take the informed consent from the patients to direct the patient's treatment.

8)To be able to learn the main task of the physician that is to prevent and cure the diseases.

9) To be aware of the importance of establishing a healthy communication between patient and physician for a

successful treatment.

OBSTETRICS AND GYNECOLOGY INTERNSHIP PROGRAM

İNTRODUCİNG OF İNTERNSHIP

Duration Of İnternship

30 working day

The internship program will be applied three times for the Turkish program and three times for the English

program.

Place of education

Internship theoretical courses will be held in Health Sciences Building of our university at Bilkent.

Practical training courses will be held in ankara ATATURK training and research hospital gynecology and

obstetric clinics and outpatient clinics.

Quantification and consideration

At the end of the internship, students are evaluated in two ways.

1)Practical and oral examinations: Examination juries consisting of two or three faculty members evaluate the

ability of an anamnesis, physical examination, differential diagnosis and treatment arrangement of the student.

2)Writing examination: It includes multiple choice, open ended, true-false or completion questions. It is aimed at

measuring the knowledge of the learners.

The effect of these exams on internship grades is 60% and 40% respectively. According to the internship grade,

the student's status is determined as successful / unsuccessful.

Aim of internship

The topics that are of interest to the students of term 4 in the obtetric and gynecology department are to provide

the ethiopathogenesis of diseases, current information about clinical and laboratory findings, diagnosis and

treatment methods, teaching holistic and ethical approach to the patient, clinical reasoning skills and basic

communication skills and research, and to acquire basic knowledge about the execution, and to apply the learned

knowledge, to teach the patient

Learning objectives

1)To be able to recognize disease groups and causes of diseases (genetic, developmental, metabolic, toxic,

microbiological, autoimmune, neoplastic, degenerative, traumatic, etc. ) and clinical features in some of the

topics related to gynecology and obstetrics branch.

2)To be able to explain the characteristics of general and genital system symptoms, mechanisms of formation

and symptomatic patient approach principles (history, physical examination, diagnostic procedures)

3)To be able to explain the formation mechanisms of disease processes and to be able to relate molecular,

macroscopic and microscopic morphological changes that occur at cell, tissue and organ levels to

physiopathological mechanisms

4)To be able to define appropriate diagnostic methods by associating basic principles and concepts of diagnostic

approach with pathological processes

5)To be able to recognize and classify drugs used in treatment, to explain the pharmacokinetic properties and

pharmacodynamic effects of drugs and to be able to associate the concepts of pharmacological effect, therapeutic

effect, side effect and toxic effect

6)To be able to define the indications and contraindications of the use of drugs, to be able to relate drugs to

pathological processes

7)To comprehend the mechanisms of gynecological and obstetric diseases and to understand the pre-clinical

basic genital pathology

8)To be able to get detailed and reliable medical history which consists of obstetric and gynecologic complaints,

past history, family history and system questionnaires and to present this information

9)Without ignoring the general examination findings, giving the patient gynecological position, learning the

examination positions, making vaginal touch, applying speculum, taking swab and vaginal culture, detecting the

findings; gain ability to detect and follow normal or pathological course of pregnancy

10)To do dressings, to suture the skin, to put urine plugs, to make leopold maneuvers, to learn to enter the

operations as sterile

11)Using basic communication skills effectively, to initiate a friendly and effective relationship between the

patient and the physician, to maintain the ability to provide patient-pregnant-physician cooperation

12)Be able to make prediagnoses based on history and physical examination findings

13)To be able to select diagnostic tests in the appropriate order for the purpose, to request appropriate, cheap,

priority and most necessary tests, and to learn outcomes assessment

14)To be able to make differential diagnosis by associating the findings obtained from history, physical

examination and diagnosis studies with the knowledge of physiopathology and clinical science, to make the

correct diagnosis

15)To describe health problems, to analyze and interpret findings, to receive informed consent form reliably

from the pregnant patient and the patient to guide treatment

16)Being able to assume that the primary duty of the doctor is to protect human life and health by preventing

diseases and trying to heal diseases, to understand the importance of healthy pregnancy and to comprehend this

to the patient

17)Be aware of the importance of healthy communication with gynecologic patients and pregnant women

18)Using effective conflict-resolution methods in relations

19)To be able to evaluate and monitor the family from demographic social and health perspective

INTERNSHIP PROGRAM OF PEDIATRICS

DURATION: 45 working days

The first 10 days of the internship program lectures and clinical skills/practices are going to be carried out at

Yenimahalle Reseach and Training Hospital.

Days 11-25: lectures are going to be carried out at Bilkent

Clinical skills and practices are going to be carried out at Keçiören Training and Research Hospital, Ankara Child

Health and Disease, Hematology, Oncology Training and Research Hospital; Dr. Sami Ulus Childrens and Maternity

Training and Research Hospital respectively after 25th day till the end of the internship.(5 days for each hospital)

AIMS OF THE INTERNSHIP PROGRAM

1. Can be able to do follow-ups of healthy child from the beginning of the neonatal period till the end of the puberty; to diagnose

and treat common childhood diseases and also to be able to manage life threatining conditions immediately and appropriately.

2. To understand the importance of preventive health care issues in childhood and to gain there quired qualifications to protect

and improve health.

3. To train the students to become doctors who have interrogative and investigator features, empathize with the patient and the

family, and apply the ethical rules of the profession.

LEARNING OBJECTIVES OF THE INTERNSHIP (Knowledge, skill, attitude)

1. Takes history from the child and his/her family

2. Does the physical examination of the child

3. Records and presents the findings of the history and examination

4. Measures and evaluates the vital signs (fever, pulse, tension)

5. Carries out and evaluates the anthropometric measurements

6. Evaluates the growth and development of the healthy child.

7. Describes breastfeeding, complementary and childhood feeding

8. Plans the vaccination program of childhood

9. Diagnosis and treats the acute and common diseases of childhood

10.Supports circulation and respiration in critically ill child and transfers to more advanced health care institution.

11.Interprets basic laboratory tests (whole blood count, urine microscopy, biochemistry, peripheral smear), ECG and basic

radiological tests

12.Explains preventive measurements of childhood diseases

13. Cooperates appropriately with the patient and his/her family

14- Properly communicates with the patient and relatives

15- Properly communicates with other healthcare providers

16- Properly communicates with the teaching staff

ASSESMENT AND EVALUATION

There are three different exams at the end of the internship to evaluate the students:

1- Practice exam: Consists of the evaluation of the student’s ability to take history from the child and/or relatives and

do the physical examination. This exam is carried out with two or three teaching instructors. The effect of internship

grade is 20%.

2- Oral exam: Is carried out by two or three teaching instructor. This exam aims to measure students’knowledge of

prediagnosis, diagnosis, treatment and prevention of the diseases in accordance to the given lectures. Also intends to

evaluate the students’ approach for sick and healty child. The effect of internship grade is 40%.

3- Written exam: This is a multiple choice question exam that is carried out to measure the knowledge of the students

in the scope of the learning objectives. The effect of internship grade is 40%.

PULMONARY DISEASES PHASE IV PROGRAM

QUANTIFICATION & ASSESSMENT

The students are assessed by two methods at the end of phase IV program:

1. Written examination: Examination includes multiple-choice, open-ended, true – false and complementary

questions. This is to measure students’ level of knowledge. The written examination is the barrier. The ones who

written examination is lower than 50 score can not enter oral examination.

2. Oral and practical examination: A grand jury including two or three jurors make test about ability to take medical

history, perform physical exam, evaluate chest Xray, make differential diagnosis and organize treatment.

The contribution to the overall score for each exam is 40% and 60% respectively. Finally, the decision for the status of

success or failure for each student is performed.

THE OBJECTIVES OF PHASE IV PROGRAM AT PULMONARY DISEASES DEPARTMENT

1) The aim is to gain knowledge and skills to phase IV students about ethiopathogenesis, symptoms, diagnosis and treatment

of pulmonary diseases.

2) The aim is to raise as doctors who have quality to investigate and inquire, make professional decision according to ethical

rules, manage patient and his/her family in holistic approach and can empathy.

LEARNING OBJECTIVES

At the end of phase IV program in pulmonary diseases department; students have ability to

1. Get detail and reliable medical history from the patient or his/her relatives.

2. Perform physical examination of respiratory system (inspection, palpation, percussion, auscultation).

3. Perform pre-diagnosis based on medical history and physical examination.

4. Choose the appropriate diagnostic tests respectively based on the initial diagnosis..

5. Identify the causes of pulmonary diseases (genetic, developmental, metabolic, toxic, microbiological, autoimmune,

neoplastic, degenerative, traumatic, etc.) and clinical features.

6. Perform differential diagnosis based on medical history, physical examination and diagnostic studies by using his

knowledge about pathophysiology and clinic.

7. Choose the most appropriate treatment for the disease.

8. Know the drugs used in treatment, explain the pharmacokinetic characteristics and pharmacodynamic effects of the

drugs, also identify the pharmacological effects, therapeutic effects, side effects and toxic effects,

9. Know the physician's primary role which is to prevent and treat the diseases.

10. Be aware of the importance of effective communication with the patient in the treatment success.

11. Get the patient’s consent after detail description of the disease and diagnostic procedures.

12. Identify the health problems in the individual and family level, identify priorities, develop solutions and make

suggestions for family counseling.

The ones who were in the clinic in the first week will go to policlinic in the second week and the ones who were in the

policlinic will go to the clinic. Polyclinic groups will rotate in allergy unit, pulmonary function testing unit, and

bronchoscopy unit.

The written examination is the barrier. The students whose written examination is lower than 50 will not have oral

examination

The final notes will be given after evaluating both written and oral examination.
